Applications are invited in the prescribed form for admission into MD/MS/M.Phil/MPH/M.MEd./MTM/Diploma

courses in affiliated Medical Colleges/ Dental Colleges/ Institutes; and Diploma & MPH courses in BSMMU for

the academic session July, 2014.

For MD/MS Part I Candidates:Candidates intending to get admission in the following courses:MD/MS/M.PhiIjMPH/M.MEd and MTM/Diploma Courses

Application Fee and Method of Payment:Tk. 3000/=(non refundable) in favor of "SB No. 0947101136920, A/C Examination Management & MiscFund BSMMU, Pubali Bank Ltd., Shahbagh Branch, Dhaka" from any Online branch of Pubaf Bank Ltd.

Period of Payment:From 13 February to 03 March 2014

Application submission:After deposition of application fee, candidate can fill the Application Form through online available onuniversity website www.bsmmu.edu.bd

Period of submission:From 16 February to 04 March' 2014 up to 02.30 PMCandidate has to upload his/her color photograph (not older than six months), Bank deposit receipt andsignature. Keep the file size of your color photograph small so that you can upload that easily.

Last date for submission of application:Up to 04 March 2014 till 02.30 pm.

Written Admission Test:28th March, Friday, 2014From 9.30 am to 11.00 am at BUET Campus, Dhaka

(for all MD/MS/M.PhiI/MTM/Diploma/M.MEd courses)From 3.30 pm to 5.00 pm at BSMMU Campus, Dhaka

(for all MPH and M.Phil (PSM) courses)

For MD/MS Final Part Candidates:

1. Candidates willing to apply for Final Part of a selected subject are requested to contact with the

office of the concerned Medical Colleqes/Institutes (where admission in Final part in that subject isapproved) for detail pre-requisites and eligibility criteria.

2. Application form can be obtained from web site www.bsmmu.edu.bd

3. Application Fee and Method of Payment: Tk. 3000/=(non refundable) in favor of "SB No.

4. After filling up, the form is to be submitted along with the Bank Deposit Receipt to the office of the
