“If you had a choice between receiving

quality healthcare at your home or in the

hospital, which of these would you

choose?”

Todays Learning Objectives

Axxess | 2013 4

There will be 73%

more 65+ year

olds in 2030

than there were in

2010.

That’s nearly 30

million boomers in

20 years!

As Teresa Lee, CEO of the Alliance for Home Health

Quality and Innovation(AHHQI) puts it,

“Not only is home health the least expensive form

of healthcare delivery, home care is the patient’s

preference”.

• Pre admittance counselling

• Improved discharge notes

• Care transitions management

• Care transitions coaching

• Telephonic monitoring post

discharge

• Case Management

• Self Management - education

All these are

workflow/education/and/

management issues

The top five reasons for patient readmissions to the hospital

1) Patients not fully understanding what’s wrong with them

2) Patients being confused over which medications to take and when

3) Hospitals not providing patients or doctors with important information or

test results

4) Patients not scheduling follow-up appointments with their doctor

5) Family members lacking proper knowledge to provide adequate care

A study by the Alliance for Home Health Quality and Innovation took

historical data to forecast potential savings of $10.3 billion over ten

years (2014-2023) by reducing regional hospital readmissions through

the use of home health as the first line to handle post-acute care

episodes.

Axxess | 2013 9

The cost driver for these

savings is the cost of a

patient/day in comparison.

Hospital stays cost 43X more

than home health care in terms

of patient days cost in dollars.

Not 43% (percent) more cost,

43.67 X (times) more costly!

Technology is making a difference.

• Improved medical team communication and

coordination

• Improved communication with the patient

• Shift from “fee-for-service” treatment to holistic

Accountable Care Organization model

• Increased efficiency and competitiveness for even

the smallest home health agencies

The ones who will survive are the tool users. The technology adapters are the ones who will do more with less…and survive the change that is coming.
