First and foremost, I am happy to report that Amistad is in a good position for the future. Last

year I reported on the changes taking place in health care and the various challenges and

opportunities presented by these changes, namely Meaningful Use, Patient Centered Medical

Home (PCMH) and the Patient Protection and Affordable Care Act. Other changes include

Accountable Care Organizations (ACO) and the 1115 Waiver Program.

By upgrading the electronic health record system, Amistad was able to meet all of the

requirements for Stage 1 Meaningful Use and collected $102,000 in incentive payments as a

result. We have started implementation of Stage 2 and two of our clinicians have met these

requirements. Our patients have been the real beneficiaries as a result of the improved service

efficiencies made possible by complying with Meaningful Use requirements. For example, each of

our patients receives a summary of their visit and has the choice to have prescriptions sent

electronically to the pharmacy of their choice.

Amistad has also made significant progress in working toward certification as a PCMH this year.

Our state association (TACHC) devoted the 2012/13 Optimizing Comprehensive Clinical Care

(OC3) learning year to assisting centers in meeting the PCMH standards. The goal is to be

certified as a PCMH by the National Committee for Quality Assurance by the end of 2014.

Amistad is now also a part of a statewide ACO. Selected by TACHC as a first-tier member of

Essential Care Partners, LLP, the ACO is participating in the Medicare Shared Savings Program.

The health center infrastructure developed by this funding can be modified to meet the needs of

other adult populations in preparation for the Medicaid and health exchange expansions

scheduled to occur in 2014 under the Patient Protection and Affordable Care Act.

The 1115 Waiver Program has included Amistad in the Regional Health Plan through

collaborative projects with Corpus Christi Medical Center and the Corpus Christi/Nueces County

Public Health Department. The plans include expanding primary care with an additional

physician and integrating behavioral health into the primary care setting in order to decrease

emergency room visits and preventable hospital admissions. Amistad will also be able to employ

a community health care coordinator to work with diabetic patients through this program.

the residents of Corpus Christi and Nueces County.

I am pleased to report that for four of those years I have served as Amistad’s Chief Executive Officer.

Some members of the Board of Directors have guided the organization since Amistad was

incorporated in 2005, and some of the earlier pioneers have rotated off the Board but continue to

offer support. The accomplishments highlighted here were made possible by the policy decisions

reflected in our Strategic Plan developed by the Board in 2010 and updated in 2012.

Amistad continued to mature both programmatically and financially in 2012. Service delivery

increased from serving 3,688 patients with 10,649 visits in 2011 to 4,430 patients with 14,577 visits in

2012, an increase of 21% and 37%, respectively. Amistad continues to serve mostly uninsured

patients with 57% of the patients served in 2012 being uninsured, with another 27% insured by

Medicaid, 9% by Medicare, and 7% by private insurance. Of the visits in 2012, 49% were for uninsured patients, 34% to Medicaid

patients, 9% to Medicare patients, and 8% to patients with private insurance.

Revenue from patient fees increased from $1,118,852 in 2011 to $1,508,775 in 2012, a 35% increase. Over half (54%) of the

patient revenue is from uninsured patients due to the increase in the number of visits provided to those patients and the diligence

of our receptionists in collecting fees from patients who are able to pay. Our Federal HRSA funding amounted to $705,565 and

other grants, including Methodist Healthcare Ministries and Meaningful Use incentives, were $514,397. Our total net assets

increased by 7% from $1,394,219 in 2011 to $1,498,455 in 2012. Liabilities were reduced by 13% ($53,925) from $428,732 to

$374,807 in 2012.

With funding from the Community Development Block Grant we have renovated three rooms into exam rooms. In addition, we

received CDBG funds to remodel the last unfinished section of the first floor to create four additional exam rooms, a nurse’s

station, and an office for a clinician.

As indicated in our President’s Message, we have made significant progress in

implementing several initiatives: Patient Centered Medical Home, Meaningful Use, being

part of an Accountable Care Organization, and collaborating with Corpus Christi Medical

Center and the Corpus Christi/Nueces County Public Health District under the 1115

Waiver Program.

The past year has further positioned Amistad for the future, and I am confident that 2013

will be an even better year. We look forward to continuing our top-notch patient care

and offering the community a solution for quality, affordable primary health care.

Patient Spotlight: Amistad is a 10!

Adelita Garcia has been a patient of Dr. Ruiz for four months now. After a

recent follow-up visit with Dr. Ruiz she talked about her experience as a

patient at Amistad. “Dr. Ruiz treats me very well. The whole staff at

Amistad treats me with dignity and respect.” Ms. Garcia started coming to

Amistad when her previous family practitioner stopped accepting her

insurance. Her insurance company referred her to Amistad, and she’s

been a satisfied patient ever since. Ms. Garcia said that she’s so happy at

Amistad that she would recommend Amistad to her friends and her entire

family. “Everything here is a 10!”

Exercise Available to Amistad Patients and Staff on Newly Completed

Walking Path

In 2012 Amistad added an outdoor walking path in the backyard with the purpose being to provide affordable

exercise opportunities for patients and staff. With funding from the Coastal Bend Diabetes Initiative and volunteers

from New Braunfels Presbyterian Church Youth Group , the walking path was completed last summer. Patients and

staff are encouraged to participate in walking groups that regularly exercise on the path. The walking path is located

next to Amistad’s community garden which was built in 2011 to offer patients and staff access to healthy fruits and

vegetables.

Medical Report Amistad 2012—More of the Best That We Can Give

Another year at Amistad Community Health Center has seen us grow, both in size and in the way in which we impact our Corpus

Christi population and the surrounding community. We have doubled the number of patient visits to almost 15,000 patient

encounters per year. We expanded our services to include prenatal care for OB patients, and active screening and treatment for

women with abnormal Pap smears who need further attention. We are performing more well baby checkups, well person exams, and

cancer screening exams for all of our patient population. We are managing chronic diseases such as diabetes and high blood pressure

so that our patients can avoid severe problems and hospitalizations. Amistad continues to be a patient-centered medical home where

our population receives their healthcare in a compassionate and respectful environment, without regard to the ability to pay for the

services that we are privileged to offer. Amistad is becoming the place to come to in Nueces County for quality, affordable healthcare.

An important part of the growth and maturation of our health center is found in a very special place. It is in the hearts and souls of the

people at Amistad, who work here because they are dedicated to continuing the healing ministry of Jesus Christ, and doing so one

patient at a time. At Amistad, we are committed to doing all that we can for as many people as we can, and doing it the best that we

can. Sometimes, this means easing the pain and suffering for diseases that we cannot cure completely. Sometimes, this means

screening for bad problems like cancer, and having to pass on bad news to patients who are learning for the first time that they have

great challenges ahead in their life. Sometimes, this means performing a routine physical exam, which can be so important to many

people to reassure them that they are in good health. It always means doing whatever we can do to improve the overall health of our

patient population.

We are also in the process of growing physically. With the help of the City of Corpus Christi, we are expanding our clinic size to

accommodate more patients and healthcare providers. In 2013, Amistad plans to add another physician as well as a specialist in

behavioral health. We have plans to remodel existing space to make our healthcare services more efficient. We are adding new,

upgraded medical equipment to keep up with advances in medical technology. Once again, we continue to strive to offer more of the

best that we can give to as many residents in need as we can properly care for in a high quality manner.

At Amistad, 2012 was a growth and learning experience for all of us. The commitment to excellence in providing quality, affordable

healthcare in the likeness of the healing ministry of Jesus Christ is alive and well at Amistad. The physical, fiscal, and emotional

environments here are as healthy as they have ever been. Even so, in 2013, all of us at Amistad look forward to giving more of the best

that we can give to the deserving patients of Nueces County and the surrounding area.
