Learn about neuroplasticity—the brain’s ability to reorganize, rewire and relearn by forming new nerve connections. Discuss how aerobic exercise enhances the neuroplastic process as it positively impacts the progression of PD symptoms.

We will utilize nonidentifying case examples to illustrate common recruitment strategies used by traffickers. We will also discuss our unique collaborative approach that highlights practical tools for responding to trafficking victims.

SESSION ABSTRACT: Bundled payment programs represent one form of value-based care. An example of a bundled payment program is the Center for Medicare and Medicaid Service’s bundled payments for Care Improvement-Advanced Program. Currently more than 1,200 hospitals and physician group practices nationally are participating in this program, which includes BJC Healthcare. We will share learning from our experience in this program to date.

SESSION ABSTRACT: Health happens where people live, work learn and play. Social determinants of health (SDH) describe non-clinical factors such as poverty, housing and transportation that impact health care utilization and health outcomes. With increasing recognition of the importance of these factors, social determinants are being integrated into the clinical setting in new ways including changes in the way these factors are collected and used in patient care. This presentation will describe changes in the national landscape on social determinants, their implications for case managers and emerging tools available in this area.

Neuroplasticity, the brain’s ability to create new circuits, may be the driving mechanism behind positive changes in the aging brain and neurologic diseases. Learn exercise agendas that may sharpen memory, cognition and perhaps impact the progression of dementia/ Alzheimer’s disease.

LEARNING OBJECTIVES:1. Describe how exercise may enhance the process of neuroplasticity and its importance in preserving brain wellness

and efficiency.2. Identify key components of a successful exercise program for people trying to preserve brain wellness across the
