Why the Lifecurve? We don’t know where we are intervening in the persons Health &

Well being journey or “state of functional decline” If we as AHPs are to achieve our AILP vision and focus on PREVENTION then

we have to know where we are currently intervening in their Health and Well Being Journey

Agreement that prevention, early intervention and self management are key to delivering H+SC but still no clarity around how we will do this.

National Survey on the Lifecurve will identify exactly where the AHP workforce is intervening on an individuals’ health and well being journey. This will lead to a better understanding of population and will help inform preventative approaches

Articulate the cost consequence for intervention at each Lifecurve stage, and will inform the development of the economic argument for intervening earlier and differently

Has the potential to contribute to meeting some of the strategic objectives

Thoughts for the AHPs in the room

• The hospital costs 16/17 for the 8000 people we had CHI numbers for >80K

• Prescription costs > 8K

• Bed days 154000

• Currently 69% of the population attending AHP services cannot walk 400 yards

• Given our vision … do you think our current service models are fit for purpose?

• 23% of the population attending AHP services are not on the Lifecurve

• Given our vision …… do you think we are doing enough to support them in their health and wellbeing?

• Is the workforce fit for the future?
