2012 has brought many changes to the healthcare industry with the enactment of the Affordable Care Act. Hospice Partners continues to provide end of life care for patients and families when they need us most by offering comprehensive and progressive programs through our state licensed, Medicare and Medi-Cal certified agency. Because we are mission-driven we believe everyone deserves the right to comfort and dignity at the end of life; your donations have enabled us to ensure that each patient that desires hospice care, regardless of ability to pay, has the opportunity to benefit from that philosophy.

On the following pages you will see a snapshot of the clients we served in 2012 and the Hospice team members who provided this specialized care and guidance. Our interdisciplinary team of professionals - Registered Nurses, Home Health Aides, Dieticians, Spiritual Counselors, Music Therapists, Social Workers, Physicians, and Bereavement Counselors collectively made 26,960 patient visits last year. We are honored to have been invited into the lives of 875 patients and families during this period. The time we have with patients, whether brief or extended, is always filled with empathy and dedication, and proves to be a very deep and fulfilling experience for our team.

This 2012 Annual Report is an opportunity to salute and applaud the generosity of our donors that enable Hospice Partners to provide expert care and support services to all families in the community, regardless of financial constraints. We are very pleased to share that not a single patient who qualified for and desired care

from our agency has ever been turned away because they could not afford to pay for the service. We are honored to have 104 active volunteers who collectively gave 6,656 hours of their time in 2012 to serve patients and families, administrative, fundraising and bereavement programs.

In 2012 Hospice Partners received its largest stock contribution from a visionary benefactor who has shared, “You can give and feel really good about it.” This humble gentleman who is featured below in this newsletter, worked hard all his life choosing to live modestly. He has chosen to create a lasting legacy by contributing to an organization that provides patient centered care.

Hospice Partners deeply appreciates the generosity of all our supporters. Collectively we received over $1,000,000 in gifts, grants and pledges in 2012 from 2,400 contributors. We have made every effort to ensure the 2012 Honor Roll of Donors is accurate. However, if you find we have made an error please accept our apology and notify us so we can make the correction. You are important to us and we want you to know how grateful we are for your generosity.

It is only through the combined efforts of the entire community of supporters, volunteers and our interdisciplinary team of hospice professionals that we can provide compassionate end of life care for those who need us. Many people ask how can we do this work and honestly we could not do it without all of you; it truly “takes a village.”

Visionary BenefactorJohn Boruk was raised in Texas and moved to California for a position with Kaiser. After retirement he became a volunteer with the American Cancer Society for 21 years and a Caring Caller at Wilshire Health & Community Services for 15 years. After becoming a hospice patient in December, John made an incredibly generous $400,000 gift to Hospice Partners.

He commented, “People will be surprised that I accumulated that much and chose to give it away, but I want other people to have the hospice experience like I have.” John explained he worked hard all his life and had a life

changing moment many years ago when he was involved in an auto accident. Upon quick assessment it was evident the young mother with children that caused the accident was not of means. He assured her there was nothing to worry about and that he would pay for the damages. “The relief and gratitude on her face gave me such a wonderful feeling I will never forget. It is better to give than to receive. You can choose to give your savings away and feel incredibly good about it. I am glad I donated to Hospice Partners and wish I had done it sooner.”

paying it ForwardHospice Partners of the Central Coast received a fabulous surprise last fall, a $12,000 contribution from VMware honoring Tony Dunn for 12 years of service, In Memory of Wallace Dunn (1920-2009), his father.

Founded in 1998 and headquartered in Palo Alto, California, VMware provides cloud computing and virtualization software. The company philosophy of giving extends to its support of the local communities and speaks volumes about their care for fellow humans.

Tony Dunn explained that he selected Hospice Partners of the Central Coast and its employees because they offered tremendous care and sensitivity during a very difficult time for his family. He wanted to reach out and thank the organization with this contribution so that other families of the Central Coast may have these services when needed as well.

Tony explained Wallace Dunn was a remarkable father, husband, businessman and friend to many people. After serving as an honored Army Captain in WWII, Wally built a successful career

of sales and real estate management, real estate development, and consulting. Besides having a passion for life, his biggest accomplishment was building a great family. Wally is survived by his wife Lois, son Tony and daughter Kathy who have been SLO County residents for over 3 decades.

Hospice Partners is honored to be selected to receive such a generous contribution. A charitable gift of this size will provide one of the following benefits to patients and families right here on the Central Coast.

• Fifty patient and family in-home visits from a member of the hospice medical interdisciplinary team of nurses, dieticians, spiritual counselors, and medical social workers.

• One hundred fifty in-home visits from a Hospice Partners Therapeutic Palliative Musician.

• One thousand hours of volunteer training, supervision and consultation for in-home visits.

• Training, support and supervision for fifteen in-home volunteers annually.

2012 contriButions in Honor of

January 1, 2013 Congress reinstated the IRA Charitable Rollover provision allowing individuals 70 and older to make tax-free charitable contributions directly from their IRA up to $100,000 annually.

The IRA Charitable Rollover allows people aged 70 and older who are required to withdraw a minimum amount from IRAs and similar retirement plans, to distribute up to $100,000 to a 501(c)(3) nonprofit, such as Hospice Partners, to help meet the required minimum distribution. These distributions to nonprofits are not subject to taxes.

2013 distributions must be made by December 31, 2013. Contact your financial advisor for more information on how this may help your tax situation.
