Exhibitors should follow their veterinarians’ recommendation to assure their dogs are free of internal and external parasites, any communicable diseases, and have

appropriate vaccinations.

Please Note: The minimum age for eligibility to enter an AKC trial is 15 months. Any entry of a dog less than 15 months on the day of the trial will be returned.

If a dog is going to be handled by a junior the Jr. Handler # must be included on the entry form.

There are no hook ups and a completed registration form, signed release and $10 registration fee are required. Agility RegulationsTelegraph, telephone and unsigned entries or entries without fees cannot be accepted. Entries

NOT on Official AKC Agility Entry Forms or photocopies of Entry Forms without the Agreement and Rules on the reverse side of the Official AKC Entry Form are NOT ACCEPTABLE. Express Mail entries must include a signature authorizing them to be left at the Secretary’s address without the recipient’s signature. If no authorization is given for express mail, or the entries arrive after the closing time and date, entries will be returned to the sender.

Injured dogs: Any exhibitor with a dog that is injured prior to the trial date will be refunded half of the amount of each entry for that particular dog, upon written certification by a veterinarian. Notification must be in the hands of the Trial Secretary not later than 9:00 a.m. on the day of the trial..Make check or money orders payable to Central Indiana Shetland Sheepdog Club (CISSC). Canadian exhibitors must make fees payable in U.S. funds. CISSC assumes no responsibility for cash sent through the mail. Post dated or returned checks do not constitute a valid entry fee. The secretary will add a collection fee of $25.00 to the amount of each returned check. Failure to make good on returned checks will result in rejection of future entries for any dog owned by the that handler

Entry fees shall not be refunded in the event the dog is absent, disqualified, excused by the judge, or banned from competition by action of the Agility Committee. If because of riots, civil disturbances or any other acts beyond the control of the management of the trial it is impossible to open or to complete the trial, no refund of entry fees will be made.

Declination of entries: CISSC may decline any entries and may remove any dog on account of disease, viciousness, or other cause and no one shall have any claim against CISSC or any official thereof. No entry shall be made or entry accepted which specifies any conditions as to its acceptance. Error on the entry forms: Owners are responsible for errors and legibility in making out entry forms,

whoever may make such errors, and no entry fee will be refunded in the event of such errors or cancellation of entries after the published closing date. Please type or print carefully the information on entry forms to eliminate errors.

Bitches in season are not permitted to compete in Agility Trials. Please note, Refund of entry fees for bitches entered in the trial that come in season after the closing date of this show may be obtained by sending a veterinarian’s certificate to the trial secretary before the start of the show. Any exhibitor with a bitch entered who will be in season on the trial date will be refunded the entry fee less a $3.00/ for each entry/run for that bitch. Notification must be in the hands of the Trial Secretary not later than 9:00 a.m. on the day of the trial.

Exhibitors alone are responsible for the behavior of their dogs and/or children. Any exhibitor whose dogs and/or children create unnecessary disturbances or repeatedly engage in unsafe or disruptive behavior may, at the direction of the Agility Committee, be asked to leave the trial site. In such a case, no refund of any entry fees paid will be made.

Dogs must be ready for judging. CISSC will not be responsible and is not obligated to furnish any service for notifying exhibitors or their agents of class judging. Judges need not wait for any dog holding up a class. Owners or agents alone are responsible for the presence of their dogs in the judging ring. All dogs present must be held on leash or confined to their crates except when being judged or in the warm-up area.

Course familiarization and Warm-ups

Handlers in all classes are permitted to walk the course without their dog prior to the start of the class. The walk-through allows handlers to plan their strategy for running the course. A judge’s briefing will be held prior to each class.

A warm-up area with a regulation jump will be provided.

Prizes and Awards Rosettes will be awarded to the 1st through 4th place dog in each jump height in each class, provided they have earned a qualifying score.

Qualifying ribbons will be awarded to each dog earning a qualifying score. A title ribbon will be presented to each dog finishing a title.MACH and PACH bars and rosettes will be presented to each dog earning a MACH or PACH title. (MACH, MACH 2, MACH 3, etc.)

One toy (value ≤ $3.00) per weekend for each dog entered.THIS CLUB DOES NOT AGREE TO ARBITRATE CLAIMS AS SET FORTH ON THE OFFICIAL AKC ENTRY FORM FOR THIS EVENT.

Dogs Eligible to Enter This trial is open to all dogs fifteen (15) months of age or older that are registered with the American Kennel Club or that have AKC Limited Registration, Purebred Alternative Listing/Indefinite Listing Privileges (PAL/ILPor an AKC Canine Partners listing number, or approved Foundation Stock Service (FSS) breeds. Bitches in season, aggressive, blind or deaf dogs or dogs suffering from any deformity, injury or illness which may affect the dog’s physical or mental performance are ineligible to participate. Mixed breed dogs that have been listed with AKC Canine Partners participate provided their listing number has been recorded on the entry form.

Jump Height Card ProcessWhen filling out the entry form, there is a new section at the top of the form for exhibitors to check if your dog needs to be measured. Please check the box so that the Trial Secretary knows whether to include your dog on the Dogs To Be Measured List. It is no longer required that you submit a copy of your dog’s jump height card with your entry form or show it during day of show check-in.

Move-ups will also be allowed during the trial If your dog qualifies to move-up preceding the trial, you must fill out the form below and send either by regular mail or e-mail. The Trial Secretary must have the completed form by 6:00 PM on the Monday, November 12, 2018.

From Chapter 1, Section 13: Dogs may be moved up to a higher class at the request of the owner as a result of qualifying for an agility title, provided that the request is made to the Trial Secretary. Entry move-ups are valid only if the dog was already entered at the event and only if approved by the Trial Secretary. Dogs having obtained their first AKC Agility Title (Std. or JWW) must be moved to the Novice B Class for all trials for which entries have not closed. Once a trial has closed its entries, a dog cannot be moved from Novice A to Novice B.

Crating space will be limited so please be mindful of that when you set up your crating areas and arrange them so as to use as small a space as possible. We will be unable to allow exercise pens unless multiple dogs in an x-pen can fit in less space than they would in their crates. Also, you will need to bring a waterproof floor covering to place underneath your crates, chair and everything else you bring.

CISSC is offering a Wait List to fill openings created by withdrawn entries after the trial's closing, November 3, 6:00pm A maximum of 20 entries will be placed on the club's wait list. All entries, including wait listed entries, must be received by the Trial Secretary prior to November 3, 2018 at 6:00pm. Any entries withdrawn that are received by November 3, 2018 will be replaced by a wait listed entry if any are available. If a withdrawn entry is replaced by a wait listed entry, a full refund will be issued for the withdrawn entry. Any wait listed entry that does not move off the wait list into the trial by Monday, November 12, 2018 will receive a full refund. A dog withdrawn from the wait list before Monday, November 12, 2018 will receive a full refund.

AGREEMENT I (we) agree that the club holding this event has the right to refuse this entry for cause which the club shall deem

sufficient. In consideration of the acceptance of this entry and of the holding of this event and of the opportunity to have the

dog judged and to win prizes, ribbons, or trophies, I (we) agree to hold the AKC, the event-giving club, their members,

directors, governors, officers, agents, superintendents or event secretary and the owner and/or lesser of the premises and

any provider of services that are necessary to hold this event and any employees or volunteers of the aforementioned

parties, and any AKC approved judge, judging at this event, harmless from any claim for loss or injury which may be

alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or about the event

premises or grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such

claim; and I (we) further agree to hold the aforementioned parties harmless from any claim for loss, injury or damage to this

dog.

Additionally, I (we) hereby assume the sole responsibility for and agree to indemnify, defend and save the aforementioned

parties harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by law upon

any of the aforementioned parties for damage because of bodily injuries, including death at any time resulting there from,

INSTRUCTIONS Handlers may opt to enter in a higher height division for all Regular titling classes, but not in a division lower than their proper height division. Dogs entered in the Preferred classes must compete at the required jump height and may not compete in a jump height division higher or lower than their proper height division.

All dogs that have not been officially measured for an Agility Jump Height Card may still compete; however, they

must be measured by a Judge of record, an Agility Field Representative, or a Volunteer Measuring Official, prior to

running. If measured by the Judge of record, that measurement will be valid for this trial or back-to-back trials only. If a measurement is necessary, it is the exhibitor's responsibility to have their dog(s) measured prior to running.

Dogs listed in the AKC Canine Partners SM program may be eligible to enter all-breed AKC agility trials at the club’s option. These dogs should be listed as All American Dog and must include their AKC number on the entry form and check the AKC No. box.

Junior Handlers should enter their Junior Handler number on the front of this form. Should you not have your Junior Handler number, it may be obtained from the American Kennel Club (919) 233-9767. If the Junior Handler is not the owner of the dog identified on the face of this form, what is the relationship of the Junior Handler to the Owner?
