All about Me

Personal File

All about me is a person centred book that can be used on a one to one or group basis to develop an individual’s wishes and aspirations. It is particularly useful with individuals who need time to work gradually on a one to one basis. It can be used as preparation with the individual, for creating a full Person Centred Plan. It is important when using the book to customise it in preparation for working with the individual:

Where ever possible use the person’s photograph, family, and friend’s photos (with permission).

Use photographs of the person’s day centre, college, school ECT.

Make the finished book reflect the individual. As you complete the stages you will eventually collate the person’s wishes and aspirations: it is then important to move through the stages to (stage 4). With all that has been collated how will you then action it? In North East Lincolnshire All About Me has been used with 80 day service users to look at what they want regarding future day services. Collated wishes and aspirations were used to inform the commissioning of community services. *All About Me format is taken from the “Listen to Me “work book from Families Leading Planning – Dept of Health.
