Personal Statement

I am a Consultant in Orthopaedic and Trauma Surgery at the Cambridge University Hospitals NHS

Foundation Trust. In addition to my full-time clinical work I have also held a number of other leadership

positions. Past and present additional responsibilities have included of Specialty Lead for Trauma and

Orthopaedic Surgery, Deputy Director for the Musculoskeletal Directorate and Deputy Medical Director

of the Hospital. In addition to leadership, I maintain a strong research and medical education profile.

I have a very strong track record of undergraduate teaching and examining, both in Cambridge and

around the world. I hold an honorary contract as an Associate Lecturer at the University of Cambridge

where I have been heavily involved in undergraduate teaching and examining in a number of different

capacities. I have also been involved in medical education of undergraduates in a variety of different

fields across the world including Malawi, Zambia, Afghanistan, United Arab Emirates and Myanmar. I

love the challenge of helping undergraduate students to link basic science knowledge to practical skills

and to inspire them to develop new applications for this knowledge. Through my training in medical law

and ethics I also have a strong focus on the non-technical skills of a career in medicine including

communication, professionalism and clinical governance.

My medical education roles have also given me considerable expertise in postgraduate medical

training. Nationally, I sit on the board of the ‘Surgical Advisory Committee for Trauma and Orthopaedic

Surgery of Great Britain and Ireland’, the national body that advises the General Medical Council

regarding specialist registration in trauma and orthopaedic surgery.

My research interests are in the sub-specialist area of orthopaedics that I practice: complex lower limb

trauma and infection requiring complex circular frame reconstruction, hip and knee joint replacement

and revision surgery, sports knee surgery and ligament reconstruction. I have over 400 citations in

Google Scholar from 40 publications.

Cambridge University Hospitals NHS Foundation Trust

Addenbrooke’s Hospital, Hills Road, Cambridge, CB2 0QQ

I enjoy my part in leading and working in this major trauma centre as an orthopaedic surgeon. I have a

particular clinical interest in complex trauma, polytrauma, nonunion of fractures and bone infections. I

also have interests in undergraduate teaching, medical management and leadership. With regard to

the latter, I have introduced a number of innovative new ways of working within our department that

has seen our efficiency increase. I have also overseen a significant expansion of the department. My

foremost current management project is working with a team to develop a ‘Musculoskeletal Institute’, a

£50m research, education and clinical hospital on the Cambridge Biomedical Campus.

With regard to undergraduate teaching, I was appointed as an Associate Lecturer on 1st March 2013.

Both before and after that time I have been involved with small group tutorials on a weekly basis for

students learning orthopaedics, giving lecturers on orthopaedics and hosting student selected

components (SSC) and overseas medical elective students with my team for intensive training. I have

also acted as examiner, both script marking and in objective structure clinical examinations (OSCEs) at

various levels. I have also been involved in selecting candidates to study medicine at Cambridge. I am

also a Clinical School ‘Preparing for Patients’ tutor and ‘Professional Practice Group’ facilitator. I have

also worked with the undergraduate Surgical Society to teach some basic clinical skills in local schools

to prospective medical students (part of a Royal College of Surgeons initiative) and to speak at their

annual conference. I have also acted as ‘University Representative’ at regional consultant interviews.

Previous Consultant Experience

1/8/07 – 31/8/10: Consultant Orthopaedic Surgeon, with Cure International

CURE International, 701 Bosler Avenue, Lemoyne, PA 17043, USA

After completing my training, I worked as a consultant orthopaedic surgeon for ‘Cure International’, an

international charity that runs a number of hospitals around the world (www.cureinternational.org). I

had contractual duties at three hospitals over the 3 years I worked for them, with postings in two

different locations – firstly in Africa (Hospital 1) and secondly in Afghanistan (Hospital 2) and the Middle

East (Hospital 3). I was heavily involved with supporting UK medical undergraduates on their medical

elective programmes in the countries where I worked.

1st Aug 2009 – 31st August 2010: Second Overseas Posting

Hospital 3: Middle East: United Arab Emirates and Oman

My primary duty in this post, comprising of 8 months of the 2009/2010 academic year, was to set a

Department of Orthopaedics at the Oasis Hospital in Al Ain, in the United Arab Emirates. During the

year I successfully completed this task. The Oasis Hospital is a charity hospital, located on the border

of the UAE and Oman, catering to all levels of society in both countries. As the first orthopaedic

surgeon at the hospital, my role included planning and purchasing all the equipment and establishing

procedures and policies necessary to run an orthopaedic department. I set about this with much

enthusiasm and during the year we saw over 1500 patients in this new department and performed a

significant number of surgeries. During its inaugural year, I gave the department a good reputation for

high quality orthopaedic surgery. Many patients travelled to our hospital from all over the country and

internationally for specialist surgery. From a professional point of view, as lead orthopaedic surgeon, I

learned much about the management of a successful orthopaedic unit during this year.

To further develop my interest in undergraduate medical education I taught basic clinical skills to

undergraduate medical students in tutorial groups of about 20 students at the University of the United

Arab Emirates as well as giving formal lectures to cover the orthopaedic syllabus of the

musculoskeletal component of the undergraduate course (60 students per year) at the University. In

addition I taught as Faculty on a number of postgraduate AOTrauma Principles and Operating Room

Personnel courses in Dubai and Kuwait.

Hospital 2: Afghanistan

A major component of my role in 2009/2010 was spending over 3 months at the Cure International

Hospital in Kabul, Afghanistan. I travelled there for 2-3 weeks every 2 months, making a total of 6 trips.

I had responsibility to restart an orthopaedic service, as well as to teach residents on the residency-

training programme. I regularly gave lectures to two family practice residency programmes in Kabul. In

addition, I gave lectures to and performed surgery at the Wazir Akbar Khan Orthopaedic Hospital (the

government stand-alone orthopaedic hospital), which is a national major trauma centre.

In Kabul the majority of my practice was complex trauma surgery, including war injuries and neglected

trauma – particularly nonunion, malunion and cases of bone infection. I would normally consult with

around 200 patients per 2 week trip and operate on about 30 patients. There was a significant training

element to the role and wherever possible I would work with local doctors.

Page 10: Alan Norrish - NEWA · trauma and infection requiring complex circular frame reconstruction, hip and knee joint replacement and revision surgery, sports knee surgery and ligament

Curriculum Vitae: Alan Norrish 10

In addition to treating the trauma, one of my goals in Kabul was to introduce knee arthroscopy as well

as hip and knee arthroplasty. At the time, the Cure International hospital was the only hospital for

Afghanistan nationals that performed knee arthroscopy, so many patients were referred. I also

performed a number of hip and knee replacements together with local surgeons, to ensure the

programme will continue. Whilst knee arthroscopy and hip and knee replacements are seen as routine

surgery in developed countries, until recently, they were not available in Afghanistan.

1st Aug 2007 – 31st July 2009: First overseas posting

Hospital 1: Beit Cure Hospital, Lusaka, Zambia

Consultant Orthopaedic Surgeon & Director of Research and Education

In Zambia, I arrived only months after the hospital buildings were opened. As part of my role, I helped

set up the hospital in terms of its referral base from the poor of Zambia and also the income-generating

private arm of the hospital. I was responsible for sourcing and purchasing the necessary orthopaedic

equipment and setting up some of the systems that make the hospital, and in particular the orthopaedic

department, run well.

In Zambia I had a very wide-ranging clinical practice within orthopaedics, with a significant amount of

time doing trauma and reconstructive orthopaedic surgery. This included: limb reconstruction work for

large bone defects following trauma, chronic osteomyelitis, tumours or congenital defects. In particular,

I started a circular frame service for problems that were difficult to manage by other means. During my

two years in Zambia I performed a significant number of circular frame surgeries. This was recognised

with an invitation to participate as faculty on a regional circular framing course in Malawi in May 2008. I

also did a wide range of trauma surgery and a significant amount of paediatric orthopaedic surgery –

including surgery on clubfeet, hip and knee problems and congenital hand deformities.

In addition to trauma surgery, I was able to introduce hip and knee arthroplasty to the hospital in

Zambia and I spearheaded a project called ‘Hips for Africa’ that was able to offer 20 free hip

replacements per year to the poorest of the poor who needed to have this operation, mainly young

patients with severe sickle cell anaemia causing necrosis of both hips. We were also able to put

together a knee arthroscopy stack and kit and did many knee arthroscopies. In addition, through a

loan, we were able to purchase a knee replacement kit and we started up an affordable knee

replacement service.

Page 11: Alan Norrish - NEWA · trauma and infection requiring complex circular frame reconstruction, hip and knee joint replacement and revision surgery, sports knee surgery and ligament

Curriculum Vitae: Alan Norrish 11

In Zambia I also performed many hundreds of operations, mainly for trauma, in remote mission

hospitals under difficult circumstances. None of these hospitals had a permanent orthopaedic surgeon

and so the patients were assembled for a regular visit by light aircraft, landing on bush strips with an

anaesthetist pilot. Operating in these circumstances with limited equipment has helped to make me a

versatile surgeon, and also taught me to know the limitations of what can be achieved.

As Director of Research and Education, I was responsible for the teaching hospital element of the Beit

Cure Hospital. We sponsored one African junior doctor to start his postgraduate training in orthopaedic

surgery. I also regularly taught at the weekly lecture meetings at the University Teaching Hospital in

Lusaka, where I was an Honorary Lecturer in Orthopaedics at the University of Zambia. I also set up

and ran a hospital teaching programme for the nurses and other members of the clinical staff at the

Beit Cure Hospital. I also set up and ran a medical elective program for student doctors from the UK

and Ireland. I regularly received students, several of who completed published research projects. Over

the 2 years I was in Zambia I hosted personally hosted over twenty international medical students for

their medical electives in Zambia.

Training Employment Experience Eastern Deanery Specialist Registrar Training Programme

Year 6 Specialist Registrar: Trauma and Orthopaedic Surgery

Mr J Nolan: trauma surgery, complex hip arthroplasty & revision surgery

Mr P Hopgood: trauma surgery, sports knee arthroscopy & arthroplasty

Mr N Walton: trauma surgery, complex knee trauma

Mr J Albert: trauma surgery, complex foot and ankle trauma

At this time the Norfolk and Norwich University Hospital had one of the highest volumes of major

trauma in the East of England. This was the reason that I applied to spend my final, post FRCS (Orth),

year there. I was able to undertake a series of attachments designed to give a high volume ‘fellowship’

style training in complex adult lower limb surgery: particularly trauma surgery but also elective hip and

knee surgery. During this subspecialty training year I was taught to perform complex trauma surgery

Page 12: Alan Norrish - NEWA · trauma and infection requiring complex circular frame reconstruction, hip and knee joint replacement and revision surgery, sports knee surgery and ligament

Curriculum Vitae: Alan Norrish 12

around the knee and foot, in particular the management of periarticular trauma using minimally invasive

techniques, the treatment of os calcis and lis franc fractures and the treatment of complex

periprosthetic fractures. I completed a research project in the subspecialty field during my time there:

A. R. Norrish, Z. A. Jibri, P. Hopgood.. The LISS plate treatment of supracondylar fractures above a

total knee replacement: a case-controlled study. Acta Orthop. Belg., 2009, 75, 642-648.

Year 5 Fellowship: Trauma and Orthopaedic Surgery

Aug. 2005 – Jul. 2006 Overseas Travelling Fellowship in Malawi

Complex Adult Trauma and Orthopaedic Reconstructive Surgery in the Tropics

Trained under Prof Chris Lavy OBE and Mr J Harrison FRCS

This accredited fellowship was based at the Queens Medical Centre, the Malawi national major trauma

centre and also the Beit Trust Cure International Hospital over the road. The latter has gained

international fame for its volume and complexity of reconstructive cases. During the year, I performed

over 400 surgeries and completed a number of research projects. I was responsible for two weekly

trauma lists at the major trauma centre. During this fellowship year I became proficient in the

management of many aspects of complex trauma, particularly with fractures around the knee (18 tibial

plateau fractures performed) and foot. I also became proficient in the use of circular frames,

particularly for reconstructing bone defects after infection and trauma. One of my key projects was in

this area:

A. R. Norrish, C. P. Lewis, and W. J. Harrison, Pin-track infection in HIV-positive and HIV-negative

patients with open fractures treated by external fixation: a prospective, blinded, case-controlled study.

Journal of Bone and Joint Surgery (Br), Jun 2007; 89-B: 790 - 793.

The Beit Trust Cure International Hospital was also the only regional hospital able to do hip and knee

replacements as well as knee arthroscopy. In addition, I helped to set up the Malawi National Joint

Register for Hip and Knee arthroplasties, which boasts being the only National Joint Registry with

100% capture of all joints ever done in the country.

Hip: I have also developed considerable experience in elective hip surgery including hip replacement

and resurfacing, hip revision surgery and periprosthetic fracture surgery, hip decompression for

avascular necrosis, hip osteotomy and hip fusion.

Commitment to Continuing Medical Education

I have ensured that my practice has remained up to date by attending journal clubs, lectures, courses,

meetings and congresses. In all continuing professional development activities I have adopted a

reflective attitude to my own practice and often change procedures and protocols to ensure best

practice.

Clinical effectiveness

My clinical effectiveness is demonstrated through my professional development portfolio and through

annual personal development meetings in my Appraisal Meeting. I have been granted a General

Medial Council licence to practice until 2019.

Evidence-based medicine

I attend a monthly departmental journal club meeting, a number of annual congresses and regularly

read the journals in my field. Using an evidence-base, I have lead on the development of a number

clinical guidelines within my hospital and the region.

Service Development

I have initiated and led a number of services and initiatives, for example:

Virtual Fracture Clinic: all new fracture clinic patients (>1000 per year) receive care in a different way.

It is the single biggest departmental transformation project in recent years.

Taylor Spatial Frame Limb Reconstruction: this cutting-edge, software-guided limb reconstruction

service was built from scratch when I was appointed. It is a regional service and commissioned directly

by NHS England.

Megaprosthesis For Trauma: whole or large parts of bones are replaced for complex traumatic

conditions, to avoid amputation. Over the last 3 years I developed this novel regional service. Joined

by two other surgeons, we have successfully performed more than 20 such surgeries and monitor

outcomes.

Page 19: Alan Norrish - NEWA · trauma and infection requiring complex circular frame reconstruction, hip and knee joint replacement and revision surgery, sports knee surgery and ligament

Curriculum Vitae: Alan Norrish 19

Bone And Joint Infection Multi-Disciplinary Team: For 4 years I have helped lead the development

of a twice-monthly MDT with consultant plastic and orthopaedic surgeons and infectious disease

doctors. Many patients have benefited from this regional service.

Quality and Outcomes

I pride myself on the excellent quality of the clinical service I deliver, both personally and as part of a

department. Our outcomes and patient satisfaction rank amongst the best in both the country and the

world. For this reason, I continue to have a wide regional referral base for many complex and sub-

specialised conditions within the field of complex limb reconstruction.

Legal Training

2003 – 2007 Master of Laws (LLM) degree, Legal Aspects of Medical Practice

This was a distance learning degree run by Cardiff Law School. The course is made up of students

from both law and medical backgrounds. Initially I passed my Basic English law exams and was then

able to pass on to specialised medical law. I completed all eight modules and then a 20,000 word

dissertation on the medical expert witness, an area that particularly interested me. I was awarded a

distinction for my aggregate mark and a grade A in the dissertation.

I regularly act for the courts as an expert witness in my field, having been instructed to give evidence in

both personal injury and clinical negligence cases.

Research

I am an active participant in well-organised clinical research through the CLRN, as seen below.

Additionally, I have a flair for developing innovative techniques to solve complex clinical problems. I

regularly present nationally and internationally on novel techniques for the treatment of bone & joint

infection and complex trauma, my sub-specialist interest.

My second area of research interest is in the treatment of orthopaedic conditions affecting developing

world countries. I have 9 publications from work that I have carried out on trips to Malawi, Zambia,

Afghanistan and Myanmar over the last 10 years.

PAST RESEARCH ACTIVITIES

1993 – 1994: Steroid regulation of inflammation and Lipocortin

This intercalated BSc project was supervised by Prof Buckingham of the Hammersmith Hospital. Using

protein assay techniques I discovered a dexamethasone-dependent cellular externalisation of a

protein called Lipocortin 1. Lipocortin is a powerful inhibitor of the phospholipase A2 enzyme involved

in inflammation and a potential novel drug target.

1994: The importance of Lipocortin in Rheumatoid Arthritis

This Wellcome Trust Vacation Scholarship was supervised by Prof JC Buckingham & Dr A Taylor of the

Hammersmith Hospital. Using various protein assays, tissue-specific differences in the distribution of

Lipocortin 1 in rheumatoid arthritis-susceptible and resistant rat strains were shown for the first time.

1994: Maternal and infant nutrition in rural western regions of Kazakhstan

Working with the charity World Vision, I was the leader of a team of medical students from all over the

UK who conducted a public health survey into the maternal and infant health, education and health

care services in rural communities. This involved much liaison work with local government officials and

development organisations.

1995 – 1998: World Health Organisation, International Leishmania Genome Project

As part of the project, I was sequencing genetic clones from an amastigote Leishmania cDNA library

(ESTs). I was responsible for developing new techniques to increase the throughput of EST

generation. I was also responsible for liaising with other members of the international network. This

collaborative group published two peer-reviewed papers on the Leishmania genome.

1995 – 1999: Modulating the immune response to infection by genetic engineering

This was my 3-year full time PhD project supervised by Prof J M Blackwell at the Cambridge Institute of

Medical Research. During this research, I gained proficiency in a wide range of molecular biological,

immunological and genetic engineering techniques. I discovered new genes by sequencing. These

were then constructed into expressible genetic constructs and injected into animals that were

subsequently given a clinical infection. The ability of different genes to modulate the immune response

of animals was thus tested. The techniques I developed represented an advance in the field of

functional genomics, an emerging field that, simply put, is designing experimental methods to utilise the

output of the genome sequencing projects. This type of basic research is applicable to a wide range of

clinical areas.

Page 24: Alan Norrish - NEWA · trauma and infection requiring complex circular frame reconstruction, hip and knee joint replacement and revision surgery, sports knee surgery and ligament

Curriculum Vitae: Alan Norrish 24

1999 – present: Clinical Research Projects

I have been involved in a number of clinical research projects throughout my surgical training and

beyond. It can be seen from the list of publications that many of these small projects, without large

outside sources of funding, have had interesting findings and have been presented locally, nationally

and internationally. Many have been published in peer-reviewed journals. It was such projects that

were awarded Annual Cambridge Orthopaedic Prizes (2003 and 2007).

2005 – present: Developing World Orthopaedic Research

I have had an interest in developing world research stemming back to my PhD work on tropical

parasites. More recently however I have been pursuing various research projects related to tropical

surgery.
