AH accidents are due to:

• Operational reasons – very high

• Maintenance reasons - low

• Technical reasons – very low

AH Fatal Offshore accidents:

• Have a much higher relative Maintenance root cause, compared to the entire fleet

This knowledge enables us to target improvements for different missions

Flight Crew Operations Manual

EC225 has the best in class autopilot:

• Automatic flight envelope protections – Altitude and airspeed

– Prevent CFIT (Controlled Flight Into Terrain)

• Automated power, speed and flight path optimisation

– Maximises performance in the unlikely event of engine failure

• Degraded visibility landing capability

• Allows safe, controlled emergency landings in extreme conditions

Flight Crew Operations Manual (FCOM)

• In the past, the OEM issued a Rotorcraft Flight Manual (RFM) and the helicopter operator issued

an Operations Manual.

• Today Airbus Helicopters has issued the 1st version of a OEM Flight Crew Operations Manual for

the EC225 offshore mission.

• The FCOM provides the OEM’s recommendation on what and how different aircraft features

should be deployed during different phases of the mission, to maximise safety.

Safety Partnership – Flight Operations

Training Standardisation

Despite the best in class autopilot, some of the safety features are not used because they are not

known to the aircrew.

The OEM and the helicopter operators manage their own Approved Training Organisations (ATOs)

to provide initial and recurrent training to aircrew and, over time, the focus and content of the

different ATO’s courses diverge as a result of different in-service experience.

Training Standardisation

• The Training Standardisation initiative aims to minimise operational accidents by ensuring

that all ATOs cover the OEM’s recommended minimum training so that all aircrew know

how and when to employ all (safety) features of the aircraft.

Maintenance Mentor

The AH Maintenance Mentor is located in the helicopter operators’ maintenance facilities to:

• Observe maintenance activities

• Provide advice and guidance

• Interpret and explain existing AH maintenance documentation and instructions

• Act as a conduit through which technical queries can be raised

• Suggest potential improvements in terms of efficiency or effectiveness

• Facilitate the sharing of best practice amongst the helicopter operators

1000 system in all its new aircraft:

• cockpit mounted,

• ruggedised,

• inertial and positioning data, ambient acoustic

data and cockpit imagery

Cockpit Voice and Flight Data Recorders (CVFDR) are fitted to all heavy helicopter types.

The FDR is used for:

•Flight Data Monitoring (FDM) to identify behavioural trends and improve training

•Investigate accidents

Light helicopters typically do not have these safety features

Rig ‘n Fly

Rig n’ Fly

AH has developed the Rig n’ Fly system which provides:

• An automatic, all weather, Instrument Landing System

(ILS) type procedure to within ½ a nautical mile of the

installation in any weather.

• Automated altitude, airspeed and direction adjustments.

• Hands-off approach until the final landing phase.

Conclusion

Despite the continually increasing:

• Fleet size

• Flying hours / utilisation

• Complexity of missions

The AH accident rate is ever improving

More improvement is always necessary until all accidents are eliminated.

As the vast majority of accidents do not have a technical cause (i.e. are not due to the machine),

AH’s focus is now on helping the operators improve operations and maintenance.

The Safety Partnership is an important initiative with that objective.
