II. Registration 1. User name and emails submitted by the AIM State Coordinator will receive an email from
[email protected] with the subject ‘AIM Data Center Invitation’. a. Click the "Accept Invitation" link.
2. You will then be prompted to confirm your name and email address, and to enter a password to use for future access to the tool (passwords must be at least 8 characters in length).
a. Click the ‘Complete Registration and Sign In’ button to continue.
3. You will then be shown a Data Use Agreement outlining the conditions for using the site and its data. If you agree to the terms, scroll to the bottom and

b. Click the ‘Accept Data Use Agreement’ button
4. Your registration is now complete! You can access the site anytime in the future by visiting www.maternalsafety.org and entering the email address and password submitted during registration.

III. Hospital-level Data Submission 1. Structure Measures
a. When signing in as a single-hospital user, you will be automatically taken to the structure measure data entry page. You will see a list of all the structure measures from the AIM bundle(s) your facility is participating in.
b. Next to each structure measure is a text field where you can enter the approximate date your hospital completed it (in MM/DD/YYYY format). While the date doesn't have to be exact, it does need to be a complete date. Alternatively, you can also click the “Not In Place” button to indicate your facility has not yet completed the measure.
c. After inputting a date and clicking on another question or area on the screen, your response will
automatically be saved and the far-right icon will change to indicate the status of your response. • A completed measure will have a green check mark • An incomplete measure will have an “X” or exclamation point depending on whether it has been
marked as “Not in Place” or no response has been provided

2. Process Measures a. If your collaborative is collecting hospital-level process measure submission, you can click the ‘Edit’
link next to a period on the ‘Process Measure Data Entry’ tab.

b. You’ll then be taken to a page where you can enter or edit process measure question responses.
Make sure to click the “Save” button since responses populated here are not automatically saved. Note: The timeline for the submission of process measures needs to be as timely as possible as agreed upon by AIM National and State Leadership. For more information on your collaborative-specific data submission timeline, contact your designated AIM Collaborative Lead Coordinator.

3. Outcome Measures
a. Hospitals will not be directly submitting outcome measure results to the AIM Data Center. Your State Department of Health or Data Coordinating Body will be providing these aggregate counts to the AIM Data Center.

V. Inviting Additional Hospital Users 1. To invite additional users from your hospital into the AIM Data Portal, from the ‘Admin Tools’ drop down
menu, select ‘User Management’, then click on the ‘Invite a new user’ button.
2. You will be prompted to enter the user’s name, email address, and role (Administrator or Read-only access). The new user will receive an email to create his or her own account after you click on the ‘Send Invitation’ button. Read-only users can't enter data or invite other users. They can only see progress and comparisons.
