Crisis Response Work Group: Update and discussion about next steps
TheobjectiveoftheCrisisResponseWorkGroup,whichmetfromOctober2017-May2018,wastoidentifyproblemsandgenerateconcreterecommendationsfortheimprovingsystemofcareforindividualswithASDexperiencingacrisisandtheirfamilies.The10recommendationsareintendedtoaddressthemosturgentpriorities,andbespecificandabletobeoperationalized:1. AHCCCSshouldincreasetrainingforPCPsandbehavioralhealthproviderstoidentify
individualsassoonastheybegintoshowsignsofaggressivebehaviorandconnectfamiliestoappropriateservices.The10-15year-oldmalepopulationisthegroupmostlikelytoexperiencecrisissituations,includingaggressiontowardothers,self-injury,andsuicide.DDDshouldcontinuetofundpeer-to-peerparentsupportprograms.
2. DDDshouldcontinuetoimprovestafftrainingandprocessestoidentifychildrenatriskforcrisis,assessneeds,andconnectfamiliestoservicesearlyinthechild’slife(beforeadolescence).Approachestoidentifyingchildrenatriskforcrisisinclude:
a. ThetoolthatDDDisdevelopingtoassesscomplexneedsandidentifyandprovideservicestochildrenandfamiliesathighriskforacrisis.
b. Childrenwithhighscores(e.g.,a6)ontheChildandAdolescentServiceIntensityInstrument(CASII)becausethismayindicatehigherlevelsoffamilystress.All

3. TheAutismCentersofExcellenceandDDDshouldcollaborativelyformulateandsharequestionsthatnetworkproviders,DDDsupportcoordinators,andbehavioralhealthcasemanagersandsocialworkerscanaskparentsinordertoidentifythetriggersthatsendfamiliesintocrisis.(Oneexampleofaquestion:Haveparentsconsideredout-of-homeplacement?)
4. Toassessriskofcaregiverburnout,AHCCCSshouldeducateprovidersabouttheuseofinstrumentssuchasAutismParentingStressIndex(APSI),QuestionnaireonResourcesandStress(QRS-F),ParentMotivationInventory(PMI),andChildOutcomeRatingScale(assuminglowratingsovertimecorrelateswithcaregiverstressandburnout).
5. AHCCCSshouldpartnerwithcommunityadvocacygroupsandconductasurveyandfocusgroupswithparentswhohavegonethroughacrisistofindoutwhatinterventionsandsupportworkedandwhatdidn’t.
6. Developamechanismforstakeholders(DDD,AHCCCS,theAutismCentersofExcellence,RBHAs,andcommunityagencies)toidentifythetypesofdatathatcouldhelptoidentifypatternsintheproblemsexperiencedbyfamiliesingettingneededservices.Oncedatatypesareidentified,stakeholdersshouldcollectandperiodicallyreviewthesedatainordertodevelopsolutionsacrosspayorsandproviders.
7. Allprovidersshouldensurethatappropriatepoliciesandcrisisplansareinplaceandstaffhavepropertrainingtorespondappropriatelywhenparentsbringupachild’saggressivebehavior.DCSshouldneverbeusedasathreatinanattempttode-escalatesituationsortocoercecompliance.Retaliationagainstfamiliesshouldneverbetolerated.Allprofessionalshavearesponsibilitytotakestepsinordertoprotectthechild,siblings,andotherfamilymembersfromharm,andalsohavearesponsibilitytoconnectfamilieswithneededservicesthatcandecreaseaggressivebehaviorandpreventacrisiswithoutescalatingtotheneedforDCSinvolvement.AppropriatetrainingshouldbeprovidedinwhatitmeanstobeamandatedreporterandhowtotalkwithfamiliesbeforeacallismadetoDCS.
8. DDDandotheragenciesshouldeducatefamiliesabouthappensduringatypicalpoliceresponse,policeCrisisInterventionTeam(CIT)training,andaskingforaCIT-trainedofficerwhencallingforassistance.MultiplefundersshouldcollaboratetoincreaseuseoftheBESAFEProgram.
9. Developaone-pageinformationsheetandgivetofamiliesaspartofintakeandtheISP—aflowchartwithclearinstructionsaboutwhattodoinacrisis:whotocallandinwhatordergiventhenatureoftheemergency(e.g.,casemanager,RBHAcrisisline,911askingforaCIT-trainedofficer,etc.).Familiesshouldensurethatthechild’semergencycrisisplanisincludedinthecrisisline’selectronichealthrecord.
10. Buildingonthesuccessfultrainingconductedbysomemunicipalities,theAHCCCSASDAdvisoryCommitteeshouldencouragetrainingforalllawenforcement,fire,andEMTpersonnelthroughoutthestateaboutthespecialneedsofindividualswithASD.

• Question:asthesystemmovestointegratedcare,whatdatacanbetrackedontheeffectivenessofcarecoordination?Dr.SaraSaleksaidthatADHS(ArizonaDepartmentofHealthServices)hasahospitalandemergencydepartmentdatabase.AHCCCScanputindatarequeststotrackindividualswithadevelopmentaldisabilitywhoareheldinanemergencydepartmentforanexcessiveamountoftimepre-andpost-October2018.AdashboarddisplayingkeydataelementscouldbedevelopedandreviewedovertimebytheCommittee.OtherdatapointsmightincludethenumberofindividualsservedinArizonacomparedtothosesentoutofstate.
• Question:Arecrisisservicesincludedintelemedicine?CrisisservicesoutsideofthePhoenixmetroareaareaconcern.Dr.Saleksaidthattherearespecificcrisiscodes.TheRBHAscanprovideinformationonhowtheyarepartneringwithcrisisservices.
Update and Input: Aurora Behavioral Health System’s Specialized Needs Unit
BrianKociszewski,M.Ed.,BCBA,InterimDirector,SpecializedNeedsUnit,AuroraBehavioralHealthSystemThespecialneedsunitinTempetreatschildrenages8-17withdevelopmentaldisabilities.Theonlyexclusionissexuallymaladaptivebehavior.Themaximumnumberofbedsis14;thenumberdependsonwhetherroomsareblocked.Childrencanbetreatedforupto60days.Servicesincludeprimarycare,OT,PT,speech,andchildpsychology.Briansaidtheyareworkingonimprovingcredentialingsothatoutpatientproviders,includingBCBAs,couldworkwithchildrenonsite.Theyareawareoftheneedforimprovedcarecoordination,especiallytosupportthetransitionfrominpatientbacktothecommunity.Theywanttoimprovedata-drivendecisionsandaretrackingreadmissionrates.Dr.SaleksaidthatAHCCCSwantstohelpprovidersmaketransitionstocommunitysettingssuccessful.Therewasdiscussionaboutthelargerissueofprovidersusingexclusionarycriteria,bothtoomuchaggressionandsexuallymaladaptivebehavior.Atthelevelofanindividualprovider,thereareconcernsabout“cherrypicking”andthereductioninthenumberofavailablebedsatafacilityifachildrequiresaseparateroom.Atthesystemlevel,AHCCCSandDDDwanttopushpastbarrierstocreatesystemsofcareforallmembers.Thoseathighestriskareoftentheoneswhoaresubjecttoexclusionarycriteria.Thisissueneedsadditionaldiscussiontoreachworkablesolutionssothatchildrenwiththemostcomplex

Alternative Behavior Strategies Model
DianaDavis-Wilson,DBH,BCBA,LBA,introducedJeffSkibitsky,M.A.,BCBA,LBA,theCEOofAlternativeBehaviorStrategies(ABS).JeffwillofferapresentationattheJanuary2019ASDAdvisoryCommitteemeetingabouttheintegrativeapproachofAlternativeBehaviorStrategies.Theyarecommittedtointegratingacutecareandmedicalco-morbiditiesintoABAprogramsandprovidedatabacktophysiciansandotherproviders.QuestionsabouttheuseofelectronicmedicalrecordsbyABSlettoadiscussionabouttheArizonaHealthInformationExchange(HIE),whichconnectstheelectronichealthrecord(EHR)systemsofprovidersandclinicians,allowingthemtosecurelysharepatientinformationandbettercoordinatecare.HealthCurrent,Arizona’shealthinformationexchange,connectsmorethan500Arizonaorganizations,includingfirstresponders,hospitals,labs,andprovidersofcommunitybehavioralhealth,physicalhealth,post-acutecare,andhospiceproviders.ThelinktotheAHCCCSwebpageabouttheHIEishttps://www.azahcccs.gov/AHCCCS/Initiatives/HIT/HIE.html.ParticipationintheHIEisvoluntary,butthereareincentivesforproviderstosharedata.DDDispartoftheHIE.AfuturegoalisforcarecoordinatorstohaveaccesstotheHIE.Attherequestofmeetingparticipants,wewillarrangeforapresentationontheArizonaHIEattheJanuarymeetingonthescopeandfunctionalityoftheexchange.Itwasalsonotedthatencouragingfamiliestoallowrecordstobereleasedtothehealthexchangeisimportant.

Update on new ABA codes recently published by AMA
BryanDavey,PhD,BCBA-D,theCEOofTouchstoneHealthServices,reportedtheCPTEditorialPanelapprovedthefollowing:Acceptedadditionofcodes97151-97158foradaptivebehaviortreatment;revisionofguidelinesintheAdaptiveBehaviorServicessection;revisionofcodes0362T,0373T;anddeletionofCategoryIIIcodes0359T-0361T,0363T-0372T&0374T.Theupdatedcodesarecurrentlypublishedinthe2019CPTCodee-book,whichBryanhighlyrecommendspurchasing;itexplainswhatcodescanandcannotbeusedfor.TheCategory1CPTcodeswillgoliveinJanuary2019,butitisuptoplanstomakedecisionsaboutroll-outdatesandexecutingcodeandrateaddendumstoexistingcontracts.Anupdatedcodecrosswalktablewillbereleasedinthenextfewweeks.AlsoforthcomingareaCPTAssistancearticle,apayorpackettosharewithnationalinsurancecompanies,additionalprovidereducation,andmorecommunicationaboutimplementationissues.ItwasnotedthatAHCCCShasbeenproactiveincoveringmedicallynecessaryABAservices.
Status Update: AHCCCS ABA policy and ABA balance billing when private insurance is primary payer
TheCOB/TPL(CoordinationofBenefits/ThirdPartyLiability)WorkGroupmettodiscusscommonscenarios,includingissuesrelatedtoABAandtheconceptof“payandchase.”Thedocumentisinthefinalstagesofreview.ParticipantsattheOctober10meetingsaidtheywouldliketheCOB/TPLWorkGrouptomeetagaintoreviewthescenariosdocumentbeforeitisbroughttothefullAdvisoryCommitteeandposted.OneissueraisedbytheWorkGroupNextwouldrequireanAHCCCSpolicyupdate.Currently,whenprivateinsuranceauthorizesaservice,itstillneedstogothroughasecondauthorizationprocesswiththeMedicaidhealthplan.TheWorkGrouprecommendedthattheMedicaidplansacceptthemedicalnecessitydeterminationoftheprivateinsuranceplan.Suchachangewouldrequireafiscalassessmentandpolicychange.AHCCCSisconsideringtherecommendation.Inresponsetoaquestion,Dr.SaleksaidthatAHCCCSisalsolookingatnetworksufficiencyforprescribedABAservices.AHCCCSreceivedextensivepubliccommentsregardingitsABApolicy.TheWorkGroupraisedissuesrelatedtoa2-tieredversus3-tierdmodel.DDDhas3-tieredapproach.Commentsincludedconcernsaboutfidelitytothemodelandthecapacitylimitationsofa2-tieredmodel.Theteamisstillworkingthroughreviewsandupdates.IfAHCCCSmakessubstantialchanges,theywillthenhavetodetermineifthepolicymustgobackthrough
