centric approach, making extensive use of knowledge and experience gained among early adopters of
integrated eCare in Europe.”

Objectives
• Implement and pilot integrated social and health care services in seven sites across Europe
• Utilise ICT tools (from home health monitoring to shared data records) to enable better cooperation between care professionals, family members and those receiving care
• Base services on care pathways cutting across boundaries which typically separate health care from social care
• Generate multidimensional evidence on impacts of integrated care on all stakeholders concerned through common evaluation approach transferable to follower-regions across Europe

Target Users

Services Provided to Target Users
• Monitoring to detect deterioration in physical/mental health or the occurrence of a health or life threatening situation;
• Domotic tools to maintain independence;
• Timely and appropriate care from health and social care professionals who will be given access to relevant information
• Activation & distribution of care tasks by workflow systems which accord to care pathways;
• Informal careers included among the resources according to defined care paths;
• Contact centre operators acting on alarms received according to defined care paths and agreed handling protocols.

Pilot Sites have:
• Taken organisational and policy steps to integrate care provision across healthcare and social care. Including, in some cases, bringing together budgets and/or establishing decision bodies across the main agencies for social and health care to enable joint commissioning;
• Deployed multidisciplinary teams incorporating health and social care staff to address both clinical and non-clinical independence-threatening conditions;
• Have already (or have planned to during the lifespan of BeyondSilos) deploy advanced ICT infrastructure within their healthcare and social care “silos”, including in particular widespread telemonitoring and telecare

Difference in emphasis
Badalona
• formalisation of the relationship with the third sector care providers;
• improve the knowledge about the benefits of using a telemonitoring platform to maintain stable multi-morbid patients at home.
Kinzigtal
• a better link between the social care database and the healthcare database in terms of a combined electronic patient record;
• facilitating the cooperation of hospitals with other providers.
CPRH
• support formalised cooperation of community services and family carers of older people with at least one chronic disease;
• Joint access to extended electronic care records supports easier coordination of service delivery.

Difference in emphasis
La Fe
• The concept of "disease management" refers to processes based on clinical guidelines grounded in evidence, and supported by the intervention of technology in each and every one of the levels at which such plans are developed.
HSCNI
• Build on NIECR – integrate Telemonitoring NI and eNISAT.
• Build in a new interactive Shared Care Plan

Difference in emphasis
Campania
• based on the existing Integrated Home Care infrastructures, the digital infrastructure will be used to support formalised cooperation, by joint access to an extended electronic care record (eCR).
• BeyondSilos will enable home monitoring of clinically relevant parameters that will be stored in the eCR. During the pilot, service users will have remote access to the personal electronic file.
Amadora
• integration of social and health care stakeholders;
• Introduction of Tele-Assistance, Tele-Monitoring, Home Care Portal, B-Learning Tool to train formal and informal carers.
