Main Purpose of the JobThis is an exciting new role to manage the development of the new social prescribing model in Harrow.
The new social prescribing model in Harrow has been designed in accordance with NHS England guidance released in 2019. The service will be delivered through a voluntary sector consortium model whereby the Link Workers will be hosted by consortium member organisations. The model will be supported by a new digital provider commissioned by Public Health Harrow. The service will be overseen by a new Social Prescribing Board under the umbrella of the Harrow Integrated Care Partnership.
The Service Manager will have lines of accountability to Harrow Community Action, to Harrow Public Health, to the Primary Care Networks and to the third sector consortium lead who will be the employer of this post.
The post holder will: Lead on the social prescribing services as it is developing, to establish an integrated
service model in consultation with Harrow Community Action (HCA) partners, including single point of access, client screening/assessment and support interventions.
Manage and contribute to the operation of the service, including assessment, support planning, onward-referral and case management of people referred to the service who meet the access criteria.
Develop, supervise and support the Link Worker Team.
Represent, promote and report on the service to all main stakeholders.
1.0 Development and Growth
1.1 Represent the Social Prescribing Service and maintain effective relationships with key stakeholders in NHS, Local Authority and Voluntary Sector.
1.2 Work with the Social Prescribing Steering Group to develop and implement a strategy for securing the long-term commissioning of the infrastructure costs to sustain social prescribing in Harrow.
1.3 Regularly review and adapt the delivery plan to optimise the potential return on investment and impact of the service.

1.4 Work with Social Prescribing Steering Group to identify and realise opportunities to develop and grow the service.
2.0 Contract Management2.1 Lead on the development and implementation of the service delivery plan.2.2 Set up and maintain relevant policies and procedures to ensure the service
operates effectively, establishing a quality assurance framework.2.3 Ensure that policies and procedures are compliant with local safeguarding and
information governance requirements.2.4 Work with our digital partner and stakeholders to establish effective systems for
record keeping and monitoring information including client outcomes, quality of service provided and to develop and maintain a directory of services.
2.5 Work with Harrow Community Action and partner agencies to develop and implement effective ICT equipment and management processes that are appropriate to the needs of the project.

Monitor delivery partner performance against HCA service agreement producing regular performance reports.Coordinate quarterly meetings of HCA delivery partner managers and liaise in relation to service management as required.Report to the quarterly HCA monitoring subgroup and HCA Board on partner performance, presenting plans to address any under-performance.Produce monitoring information for the Social Prescribing Steering group and Integrated Care Partnership reporting on performance against the Service Level Agreement targets.
3.0 Team Building3.1 Design induction and coordinate training programmes for Link Workers to ensure
consistent standards of effective practice across the service.3.2 Liaison with Service managers to ensure all staff and volunteers have access to
relevant fundraising training where necessary.3.3 Set and agree standards of performance across the team ensuring staff and
volunteers are supported in achieving these through regular supervision, appraisals and training.
3.4 Provide 1-1 line management support and supervision to the project team as required to support adherence to service quality assurance framework and practice
3.5 Coordinate regular meetings of the Link Workers to develop and maintain a mutually supportive team-working approach.
3.6 Ensure that the project team are supported in working from physical environments that are appropriate to the needs of the service and are compliant with relevant policy and regulation, and in particular good practice in lone working and health and safety.
3.7 Ensure there are effective communications and team development opportunities to support project staff in operating as a ‘virtual’ team across several locations and organisations.
3.8 Support project staff in recruiting, training and supporting volunteers and their contributions to the service specification and agreed operating standards.
3.9 Work with the Social prescribing Steering group to regularly review staff and volunteer needs and to plan for expansion of the service as appropriate.

3.10 Provide 1-1 line management support and supervision to the project team as required to support adherence to service quality assurance framework and practice.
4.0 Marketing and Promotion 4.1 Develop effective working relationships across Primary Care Networks ensuring high
visibility of the service.4.2 Develop efficient referral networks and strong links with the range of community,
statutory and voluntary providers in the London Borough of Harrow.4.3 Liaise, represent and promote the Service to external stakeholders as necessary.4.4 Work with Age UK HHB and HCA to develop and maintain appropriate branding,
marketing resources and communication channels with clients and other stakeholders.
5.0 General5.1 Represent Age UK HHB on appropriate external committees, networks and other
bodies, with other voluntary, statutory and private sector agencies.5.2 To maintain own professional expertise, including attending training as necessary
and be subject to supervision and an annual appraisal.5.3 To attend staff meetings and away days, training and other events.5.4 All staff are expected to undertake their own computer work, both in the
production of correspondence and documents, date recording, e-mailing and internet research.
5.5 It is the nature of the work that tasks and responsibilities are in many circumstances unpredictable and varied. All employees are expected to work in a flexible way, as required by Age UK HHB. Some meetings and other events may be held out of normal office hours and could involve travel away from the local area.
