AFFIDAVIT OF REVALIDATION

PAG-ASA BIBLE BAPTIST CHURCH, represented by its Deacon, GERAMUND M. CLARITO, of legal age, married, Filipino citizen, and a resident of Blk. 20, Lt. 20, Aguado St., Phase 8, Bahayang Pag-Asa Subd., Imus, Cavite, after having duly sworn on oath, hereby depose and says:

1. That I am the member/deacon of PAG-ASA BIBLE BAPTIST CHURCH buyer of a parcel of land located at Tanzang Luma, Imus, Cavite, containing an area of TWO (200) SQUARE METERS covered by Transfer Certificate of Title No. T-162033 embraced by Tax Declaration No. 09-019-21431;

2. That VIVIAN P. BAYHONAN cause the payment of Capital Gains Tax at Bureau of Internal Revenue (RDO 54) Trece Martires City, Cavite;

3. That on September 15, 2006 the BIR issued me a Certificate of Authorizing Registration with CAR No. 00092514;

4. That on year 2007the said VIVIAN P. BAYHONAN stop the processing of the transfer of the said parcel of land for the reason that VIVIAN P. BAYHONAN travelled abroad and he left the papers at PAG-ASA BIBLE BAPTIST CHURCH;

5. That I am requesting the BIR (RDO 54) to revalidate the CAR No. 00092514;

6. That I am executing this affidavit to attest of the foregoing facts and to revalidate the CAR No. 00092514 and for whatever legal purpose it may serve.
