ADDplus (anterior distraction deviceplus) ist ebenfalls ein Wirbelkörper-ersatz, der für die vordere zervikaleWirbelsäule und für den zerviko-thorakalenÜbergangeingesetztwerdenkann.EntstandenistADDplusdurchdiekonsequenteWeiterentwicklungunsererbeidenSystemeADDundosmium.Derentscheidende Vorteil ist die direkteKombination von Wirbelkörperersatzund ventraler Verplattung, d.h. zweiSysteme –Cage und Fixationssystem–ineinem.

ADDplus bewirkt die Rekonstruktion,Abstützung und Stabilisierung dervorderen Säule. Die Fixierung anden Wirbelkörpern erfolgt mittelsSchraubendesosmium-Systems,wobeisowohleinemono-alsauchbikortikaleSchraubenverankerung möglich ist.Durch die zusätzlichen Platten werdendieMigration sowie einmögliches Ein-sinken des Implantats verhindert. Daskombinierte Implantat bringt somiteinen Gewinn an Sicherheit. Im Ver-gleich zu einer ventralen Platte ist dieFlächereduziert,sodasseineguteknö-cherneFusionerreichbarist.

Beide Implantate, ADD und ADDplus,sindinsitustufenlosdistrahierbar,somitist eine optimale Überbrückung desDefektesgarantiert.DiegroßenAuflageflächenmitDornengewährleisten eine sichereAbstützungund Verankerung. Die vorliegende OP-Technik beschreibtdieArbeitsschritte für dieAnwendungvon ADD und ADDplus. Außerdemwerden die Implantate und die zurImplantation notwendigen System-instrumentevorgestellt.

DieOP-Technik ist als alleinigeGrund-lage für die erfolgreiche Anwendungder Wirbelsäulenimplantate ADD undADDplus nicht ausreichend. The implant ADD (anterior distractiondevice)isavertebralbodyreplacementfor the anterior cervical and thoracicspine.ADDcanalsobeused for inter-somaticfusion.ADD is used for the reconstructionof substance defects, support of theanterior spineaswell as restorationofthespinalprofile.Anadditionalanteriororposteriorstabilizationbymeansofarod/screw system or a plate system isnecessary.

ADDplus (anterior distraction deviceplus) is also a vertebral body replace-mentwhichcanbeused for theante-rior cervical spine and for the cervico-thoracic transition.TheADDplus is theresultoftheconsistentandcontinuousdevelopmentofour two systemsADDand osmium. The decisive advantageis the direct combination of vertebralbodyreplacementandanteriorplating,twosystemsinone–cageandfixationsystem.

ADDplus effects the reconstruction,supportandstabilizationoftheanteriorspine. The fixation on the vertebralbodies is effectedbymeansof screwsfromtheosmiumsystem,withamono-cortical as well as a bicortical screwanchoringbeingpossible.Theadditionalplates prevent a migration as well aspossible sinking inof the implant. Thecombinedimplantthusprovidesagainin additional safety. In comparison toa ventral plate the surface is reducedsothataimprovedbonefusioncanbeachieved.

Both implants, ADD and ADDplus,are continuously expandable in situ,thusanoptimumbridgingofthedefectisguaranteed.The large support surfaces withspikes guarantee secure support andanchoring.The consolidation of introduced bonematerialissupported.Theeasyhandlingand few operating steps provide forshort operating times and low bloodlossforthepatient.

ADD Implantate (CS2250-X) stehen indrei verschiedenen Durchmessern (12,14 und 16mm) zur Verfügung. DieDistraktionsbereiche reichen von 10 bis65 mm. Preparation

The implantation of anADD or ADDplus implantis demonstrated on a cer-vical model. Depending onactual segment height, ananterior or lateral approachcan be selected for the useofADD.ADDplus isusedviaan anterior approach. Theaffected vertebral body isexposed. Next, the adjacentvertebraldiscsandtheverte-bralbodyarefullyorpartiallyresected.

Fitting the locking screw

The lockingscrew(CS2259)can be pre-fitted outsidethe situs into a bore belowthe expansion ring. In thecaseofADDplus the lockingscrewmust be used on thefront of the implant usingthescrewdriver (CS1328). Ifinclined ADD implants areused, ensure that the lock-ing screw is located at theapproach end. The lockingscrewisnottightened.

Assembling the depth stop

Tapping for cancellousbone screws

Use the same techniqueas with the monocorticalscrews.However,usethetapfor cancellous bone screws,Ø4mm(CS1326).Important: It is optional tousethetap(CS1326)beforeapplying the screws. UseC-armcontrol!

Implanting the cancellous bone screws

The appropriate length ofthe cancellous bone screwmay also be determinedusing the depth gauge(UT1068-23). Inserting thecancellous bone screws bymeans of the screwdriver,hex2.5mm(CS1328)underC-armcontrol.

Vorbereitung derSchraubenimplantation
