Press “<< Prev Step” to go back to step 2Press “Validate” to submit requisition. Once the requisition

is validated, you cannot modify it anymore. You cancommunicate with the Actilab Customer Service departmentif there are modifications to be made to the requisition after

has been validated.Press “Cancel” to abort the current operation

and make a new requisition.

When this window opens, the printdialog will pop up shortly thereafter.

Follow instructions to print.

Select what requisitions youwould like to show.

You have the following choices:“Your Name” : Shows the requisitions you have made

“For the Company” : Shows requisitions made byyou and any other employee from your company

To narrow down your search options, you can choose from theFollowing options:

•“List all requisitions”: This will list all requisitions•“By Product”: To search by product name•“By Lot”: To search by lot number•“By Requisition”: To search by requisition number

NOTE: You may not see this step

By trying access this step you can end up in:

•Results View page Cause: You have chosen “List all requisition” under “Search for” option in step1

•You receive a message telling you that you have no requisitions Cause: You or your company haven’t yet made any online requisitions.

According to your choice in step one, you will have a list to choose from,either: •Requisition number ( this example)•Lot Number•Product Name

You have possibly made a mistake while filling outthe form. Verify the information in “Payment” section

one more time.

If not, there will be empty fields whereyou can add the product identification and version

information, if you wish to do so.

Hit “<< Prev Step” to go backto Quotation Step 1.

Hit “Validate” to submitquotation. (Make sure that all

information is accurate beforehand, as you will not be able to modify it after it is validated)
