Requirements:• The IBO can qualify if they acquire 5 New Orders starting January 1 for the same type of service (Digital Phone Service, Local & Long Distance or Flash Wireless).

• Customers must be separate accounts, therefore accounts with family plan lines or accounts with multiple video phones, wireless phones or local lines count as one customer.

• The 5 new customers must be separate accounts with separate credit card information, outside of the IBO’s home.

• A credit for the monthly service charge will be applied each month as long as 5 active customers (acquired on or after January 1, 2012) are maintained:

o Flash Wireless – a credit of up to $60 will be applied to the monthly service

o Digital Phone Service – a credit of $29.99 will be applied if the IBO has a video phone or $24.99 if the IBO has a phone adapter

o Local and Long Distance – a credit in the amount of the local monthly fee plus the long distance plan monthly fee will be applied. Monthly fees for international plans and features do not apply

• Credit toward total bill amount; credits may take up to two months to be applied.

• Tax and surcharges on credit amount still applies, where applicable.
