What happens now?

Immediately report it to your Supervisor or Manager – By Law, late notice or failure to notify may bar a claim for Workers’ Compensation Benefits! and get medical treatment (if required).

Complete the employee portion of the ‘First Report of Injury’ form.

Cooperate with any investigation by Management.

Return to work (if possible).

You may be contacted by the Corporate Workers’ Compensation Administrator to verify the information provided

If unable to work and medical leave is required, bring a doctor’s written statement with diagnoses and estimated return to work to your supervisor
