4 Correct Patient = Correct Procedure

Aim To develop an QI and patient safety project in four strategic areas at the Hospital General 450:

1 Specialty Medical Services 2 Laboratory 3 Pharmacy and 4 Radiology .

Background In April 2013 staff were not trai-

ned in quality improvement and

patient safety.

Members of the board concerns

to create a culture of quality im-

provement (QI) is needed to im-

prove the reliability of health ca-

re and thereby bridge the gap

between possible STEEEP TM

care and actual usual care.

The IOM´s defined High quality

health care as care that is safe,

timely, effective, effcient, equi-

table, and patient centered.

Intervention 1 Multidisciplinary Team

Integration of leading teams in each

strategic area

2 Educate

Educate hospital staff through quality

and patient safety sessions a time prior

to the end of the workday

3 ABC HG450 Course

Training a four-month course for develo-

ping a project with rapid cycles of QI

4 Graduation

Celebrate the achievements with a gra-

duation ceremony in which staff receive

an acknowledgment of the Board and

the Department of Quality Improvement,

inspired and supported by STEEEP Aca-

demy, Baylor Health Care System.

Results: International Patient Safety Goals Im-

plementation in each strategic area :

1 Specialty Medical Services Project:

ID patient to safety care.

2 Laboratory Project:

Read back of the critical lab results .

3 Pharmacy Project:

Improve the safety, verification and

drug conservation of High Alert Medi-

cation.

4 Radiology Project :

Correct Patient = Correct Procedure

Project Design

Next Steps/ Lessons Learned 1 Capacity, assure that the policies

have been establishing.

2. Available, to train the personnel

about the changes.

3. Revenues, manage the equipment

purchase.

4. Evolution, to provide strategies

that assure the implementation

and learn new ways to do the job
