Infection:
Invasion of the host organism by microorganisms that can cause pathological conditions or diseases
3
Community:
Elements of residence that characterize a population, for example students in a school or people in a village
Environment:
The external elements and conditions which surround, influence, and affect the life and development of an organism or population.

Community-acquired infection (CAI):
are defined as infections manifesting and diagnosed within 48 hours
of admission in patients without any previous encounter with
healthcare. (Book: Infectious Diseases 4th, 2017)
Hospital-acquired infection (HAI): (Nosocomial infection)
are defined as infection manifesting and diagnosed after 48 hours
stay in the hospital with any encounter with healthcare.

Nearly all clinically important bacteria can be detected/visualized using Gram staining method
The only exceptions being those organisms…
• That exists almost exclusively within host cells i.e. Intracellular bacteria (e.g., Chlamydia)
• Those that lack a cell wall (e.g., Mycoplasma)
• Those of insufficient dimensions to be resolved by light microscopy (e.g., Spirochetes→ Dark field microscope)

Reservoir• A host that carries a pathogen without injury to
itself and serves as a source of infection for
other host organisms
24
Human reservoirs Animal reservoirs
Environmental reservoirs
Incidental hosts
![Page 25: กระบวนวิชาบทน าสุขภาพชุมชน 2 (ICH2) · 2020. 11. 9. · •e.g. Trichinosis, Anthrax, Rabies,Leptospirosis, Plaque Vector-borne disease](https://reader035.vdocuments.us/reader035/viewer/2022070223/6147abd1afbe1968d37a3392/html5/thumbnails/25.jpg)
Human reservoir
• Most of reservoir is human.
• human to human w/o intermediate
• 2 types1. Patients
• Low risk of transmission due to diagnosis & treatment
• Low chance of contact to other people
2. Carriers• High risk to transmission due to
- Incubation period
- Asymptomatic
- Recovery but still having pathogens
25

Animal reservoir
Zoonotic disease
• animals to people, normally exists in animals
• but can infect humans (incidental hosts)
• e.g. Trichinosis, Anthrax, Rabies, Leptospirosis, Plaque
Vector-borne disease
• transmitted to humans and other animals
• If it replicates in human and animal → 2 reservoirs
• by blood-feeding arthropods: mosquitoes, ticks, and fleas
• e.g. Dengue fever, Chikungunya, Lyme disease, Malaria,
Plaque 26

Portal of exit
• The path by which a pathogen leaves its host
• usually corresponds to the site where the pathogen is localized
• e.g. Influenza viruses, TB → respiratory tract
Vibrio chorela → feces
Rubella, Syphilis, Toxoplasmosis → crossing the placenta
Schistosomes → urine and feces
Hepatitis B → cuts or needles in the skin
Malaria, Dengue, Chikungunya, Zika→ blood-sucking arthropods

Mode of transmission
• An agent may be transmitted from its reservoir to
host in different ways.
• Examples of modes
• Direct: contact
• Indirect: contamination
• Droplet: droplet >5 µm in diameter
• Airborne: droplet nuclei <5 µm in diameter
• Vehicle-borne: water, soil, food, blood
• Vector-borne

Ro = The basic reproductive number
• It is the average number of secondary cases which one
case would produce in a completely susceptible
population.
• Parameter for determining whether or not an infectious
disease can spread through a population
• vary considerably for different infectious diseases but
also for the same disease in different populations

52
Ro = The basic reproductive number
R0 > 1 => Each primary case produces more than 1 secondary case,
spread in a population & epidemic
R0 = 1 => Each primary case replaces itself,
continue to persist endemically
R0 < 1 => Each primary case doesn’t produce enough cases to replace itself,
die out in the long run.

Ro = The basic reproductive number
Depends on…
• The duration of the infectious period
• The number of new susceptible
individuals contacted per unit of time
• The probability of infecting a
susceptible individual during one
contact
(Dietz K., Statistical Methods in Medical Research, 1993)
Benefits…
• Elimination or eradication an
infection from a population
• Vaccination programs
• Chemotherapy or reduction of
the contact rate (=The number of
new susceptible individuals contacted
per unit of time)
![Page 56: กระบวนวิชาบทน าสุขภาพชุมชน 2 (ICH2) · 2020. 11. 9. · •e.g. Trichinosis, Anthrax, Rabies,Leptospirosis, Plaque Vector-borne disease](https://reader035.vdocuments.us/reader035/viewer/2022070223/6147abd1afbe1968d37a3392/html5/thumbnails/56.jpg)
R = The effective reproductive number
• It is the average number of secondary cases per
infectious case in a population made up of both
susceptible and non-susceptible hosts.
56
R>1 the number of cases will increase, such as at the start of an epidemic.
R=1 the disease is endemic
R<1 there will be a decline in the number of cases

R = R0X
R = the effective reproductive number
R0 = the basic reproductive number
X = the fraction of the host population that is susceptible
E.g. If R0 for Measles is 12 in the population where one-fourth (1/4) of the
population is immune, the effective reproductive number (R) is...?
R = ?
R0 = 12
X = 1- (1/4) = 0.75
R = 12 (0.75) = 9
A single case of Measles would produce an average of 9 new secondary cases.
![Page 60: กระบวนวิชาบทน าสุขภาพชุมชน 2 (ICH2) · 2020. 11. 9. · •e.g. Trichinosis, Anthrax, Rabies,Leptospirosis, Plaque Vector-borne disease](https://reader035.vdocuments.us/reader035/viewer/2022070223/6147abd1afbe1968d37a3392/html5/thumbnails/60.jpg)
60
Herd immunity / Community immunity
• If most get vaccinated … spreading is contained• Vaccinated people act as barriers to outbreak
: the indirect protection of unvaccinated persons, whereby an increase in the prevalence of vaccine-immunity prevents circulation of infectious agents in unvaccinated susceptible populations (Kim et al., 2011).
![Page 61: กระบวนวิชาบทน าสุขภาพชุมชน 2 (ICH2) · 2020. 11. 9. · •e.g. Trichinosis, Anthrax, Rabies,Leptospirosis, Plaque Vector-borne disease](https://reader035.vdocuments.us/reader035/viewer/2022070223/6147abd1afbe1968d37a3392/html5/thumbnails/61.jpg)
61
Herd immunity = 1 – (1/R0)
In case:
- to know the herd immunity in a community after
vaccination or natural occurring
- e.g. R0 = 1.5 Herd immunity = 1 - (1/1.5) = 33%
Vaccine coverage = 1 – (1/R0)
In case:
- to know the proportion of the population that needs to
be vaccinated to prevent sustained spread of the infection
- e.g. R0 = 7.0 vaccine coverage = 1 - (1/7.0) = 85.7%
![Page 62: กระบวนวิชาบทน าสุขภาพชุมชน 2 (ICH2) · 2020. 11. 9. · •e.g. Trichinosis, Anthrax, Rabies,Leptospirosis, Plaque Vector-borne disease](https://reader035.vdocuments.us/reader035/viewer/2022070223/6147abd1afbe1968d37a3392/html5/thumbnails/62.jpg)

4. Environmental Decontamination
• Process by which the healthcare environment is made free from a
harmful level of contamination that may result in the transfer of
pathogens that can cause healthcare-associated infection
• Cleaning – removal of ‘dirt’ from surfaces, which may include
microbial (e.g., bacteria) and non-microbial (e.g., body fluids)
contamination such as manual washing and automatic washers
• Disinfection – killing of microbes on surfaces, via chemicals, heat,
or another process except bacterial spores
• Sterilization – destroy all microorganisms on the surface of an
article or in a fluid to prevent disease transmission associated with
the use of that item 80
