Service Overview: Community Optometry
Evolutio Care Innovations Limited have been awarded the contract by NHS Barking and Dagenham CCG, NHS Havering CCG and NHS
Redbridge CCG (BHR CCG’s). Some of the service objectives include:
• Providing care closer to home• Reducing referrals to secondary care• Ensuring a more equitable and sustainable service• Maximising community care though a multi-disciplinary workforce
Service includes:
• Enhanced Optometry Services (optometry delivered)• Stable Glaucoma/OHT Management (ophthalmology led/optometry delivered)• Minor Surgery/Oculoplastics (Consultant/GPwSI delivered)
Referral routes include: Walk-in; self referral; central management and triage (via Evolutio)
Community Optometry now has the opportunity to provide EOS under the new contract to your patients. Each community service provider must:
• Use eVonnect (our free bespoke software) for central patient record management• Work to standardised protocols• See patients within 2-6 weeks depending upon the pathway
