Having suffered from malaria occasionally as a child and during adulthood, and seeing the many tropical diseases that were ravaging the communities in my country Zambia and Africa as a whole I realized that I wanted to make a contribution to medicine either by helping understand the diseases that affect us and by treating people as a medical doctor or participate in vaccine development. I came to the conclusion that research and ongoing education were the best tools that society needs to protect itself from diseases. I therefore started participating in science fairs by making project presentations on different science topics during my high school.

My interest for microbiology and molecular biology began during my undergraduate studies and was the impetus for my desire to do infectious disease research. In my third year at the University of Zambia (2005), I pursued an internship at the Malaria Institute at Macha (MIAM)/Johns Hopkins Malaria Research Institute also known as Macha Research Trust. During this internship I spent a month mastering the techniques of DNA extraction and Polymerase Chain Reaction (PCR) from Plasmodium Jalciparum. In addition, I did field work where I participated in collecting blood samples from people in the communities around Macha.

In 2007, I joined the MIAM research staff as a Laboratory Scientist in the Malaria Molecular biology laboratory with an emphasis on immunity against the malaria parasite in human blood. The aim of the research was to determine how levels of antibodies against P. Jalciparum change due to intensified malaria control measures and decreasing transmission intensity. The second objective was to develop a simple, field-friendly, method to measure immunity to P. Jalciparum and to indentify populations at risk for disease resurgence. I performed Enzyme immunoassay to measure IgG levels using P. Jalciparum NF54 strain schizont cell lysate on dried blood spots (DBS). In this work, I interacted routinely with principle investigators at Johns Hopkins Malaria Research Institute Dr William Moss MD (project director) and Dr Tamaki Kobayashi PhD (co­investigator), and I also participated in weekly lab meetings. The preliminary results from the study were as follows: The Optical Density (OD) values of the IgG antibodies in children younger than 10, increased with age. These results indicated that immunity in young children takes longer to develop and is dependent on frequent exposure. A year after distribution of insecticide treated bednets (ITN s) there was no association between bed net use and seropositivity in children younger than 10, probably due to lack of frequent exposure. In individuals older than 20 years a significant association was observed between seropositivity and bed net use. This could indicate that decrease in exposure to P. Jalciparum should lead to decreased levels of immunity. There was spatial clustering of individuals with increasing IgG antibodies against P. Jalciparum a potential indicator for focal malaria transmission in our study area.

The third objective of the malaria project was to develop a non-invasive way of estimating population immunity to P. Jalciparum using oral fluid (OF) as a surrogate to blood samples. Our data showed that oral fluid is a valid measure for estimating population immunity against P. Jalciparum antigen. The OF assay identified all DBS-confirmed positive and negative samples, resulting in 100% sensitivity and specificity. The preliminary data from this work has been presented at the American Society for Tropical Medicine and Hygiene (December, 2008 and November 2011) and the Zambia National Health Research Conference (September 2009). I presented the work at the 61st Lindau Nobel Laureate conference 2011 in Germany during the Microsoft sponsored lunch attended by Mr. William H Gates. We made a publication on the

Validation of oral fluid samples to monitor serological changes to Plasmodium Jalciparum by Alexis P Chidi et al. 2011. The project is still ongoing.

In addition to my work on malaria, I co-ordinated a project on schistosomiasis both field work and the laboratory work. The research aim was to assess the prevalence of urinary schistosomiasis among school children in Chitongo area, southern Zambia. My team and I collected urine samples from school children from three randomly selected schools. I performed microscopy on the urine samples together with a second reader for verification as routine. I also wrote a manuscript about this project which was published in the Zambia Medical Journal 2008 edition. During 'this period I was under the direct supervision of Dr Clive Shiff PhD whom I reported to on a daily basis via skype or e-mail. I presented this work at the American Society for Tropical Medicine and Hygiene in December 2008 and at the Zambia National Health Research conference in September 2009. I participated on several other research studies and co-authored abstracts and manual scripts on these projects Mkulama APM, et a12008.

In August 2010, I enrolled in a two year Master's program at the Erasmus MC, University Medical Center, Rotterdam, the Netherlands in the Infection and Immunity program under the faculty of Molecular Medicine. I did my first six months internship in the Hematology department in the group of Dr Tom Cupedo PhD, where I worked on a project to identify the location of human retinoic acid receptor (RAR) related orphan receptor C (RORC) innate lymphoid cells (ILC) in situ. In mice, fetal RORyt+ ILC are involved in development of lymph nodes and in adults RORyH ILC through OX40L and CD30L expression provide survival signals to T cells allowing them to select for B cells during affinity maturation and memory responses. We hypothesized that similar to mouse RORyt+ ILC, human RORC+ ILC are involved in fetal lymphogenesis and postnatal immune responses. Our research aim was to identify the location of RORC+ ILC in human secondary lymphoid organs and to determine whether their location reveals their function. Since organogenesis and immunity research cannot be done in vivo in human we worked with palatine tonsil tissue and human fetal mesenteric lymph nodes. During this work I stained the tissue to locate B-cells, T cells and the stromal cells. In addition I learnt how to perform flow cytometry and make cell cultures. Our results showed that RORC+ ILC in human fetal mesenteric lymph nodes (MLN) are first located at the subcapsular site and during later stages of development are more profound at the paracortical region. Interestingly, in inflamed palatine tonsils, the RORC+ ILC were predominantly localized in the interfollicular areas in close proximity to the B cell follicles, at sites where CXCL13 was expressed by stromal cells. Further analysis revealed that these RORC+ ILC expressed the activation marker NKp44, both on tissue slides and by flow cytometry. We postulated that the data further increases our knowledge on how RORC+ ILC may play a role during lymphoid formation and inflammation, which could lead to better insight in their role in lymphoid organogenesis, inflammation and auto immune diseases.

I recently started my final Master's project which will be the subject ofthesis in the hnmunology department under the direct supervision of Dr Mirjam van der Burg PhD and Dr Gertjan Driessen MD. The project involves determining whether (memory) B cell subsets and vaccination responses can be used to identify primary antibody deficiencies (PAD). Due to genetic heterogeneity of PAD disorders, many patients remain undiagnosed or are diagnosed too

late with poor prognosis. Therefore, based on the new concept regarding the origin of six memory B-cell subsets proposed by Berkowska et al. (2011) I am currently performing a retrospective study and will later conduct a prospective analysis of these subsets in healthy individuals, patients with Common Vmiable Immunodeficiencies (CVID), partial antibody deficiencies (SP AD, IgA, IgG2 deficiency) and patients with recurrent infections without antibody deficiency. We have so far linked the B cell subset results to clinical data retrospectively. The preliminary data show that in healthy individuals, IgG+CD27+ and IgA+CD27+ memory B-cells reach adult levels within two years of age. In CVID and partial antibody deficiencies T-cell independent IgA+CD27- memory B-cell counts were normal. In CVID, the other memory B-cell subsets were decreased. A reduction of IgG+CD27+ B-cells showed the strongest association with severe pneumonia and bronchiectasis. This subset was also decreased in 16% of patients with recurrent infections without antibody deficiency, suggestive of a defect in B-cell development. Currently, we are exploring vaccination responses in relation to the memory B-cell subsets. Besides analyzing the B cell subsets I have participated in diagnostics by extraction of B-cell subsets by flow cytometry and Euroflow and gating the B cells subsets. I have also familiarized myself with culturing fibroblasts, designing primers for genes including CD71 and sequencing the DNA amplicons. This work is still ongoing. This research has been incredibly interesting and has furthered my interest in public health, specifically infectious diseases. I have gained an insight into the function of different genes and the human immune system, and I hope to use that information and some of the techniques in tropical disease research.

With a PhD in Molecular Microbiology and Immunology from Johns Hopkins School of Public Health (JHSPH), I plan to participate in research at both national and international levels. Specifically, I am interested in ongoing efforts of the Centers for Disease Control and the World Health Organization. I plan to continue my research in tropical diseases such as malaria, HIV/AIDS and possibly expand into the field of autoimmune diseases such as cancer. My goal is to be part of a solution to improve the outlook for the millions of individuals struck with deadly infectious diseases I have the opportunity to help through research. I eagerly anticipate gaining the additional knowledge and skills that I need to pursue these ambitions through a PhD program at the JHSPH. A PhD program will enable me to contribute, more fully, to our understanding of infectious diseases and chronic illnesses in my home country Zambia and around the world. My research thus far has prepared me well for the graduate education challenges I now wish to pursue.

I am very much honoured to highly recommend to you Ms. Sandra Chishimba, who is one of out most talented students of the Research master Infection & Immunity at Erasmus MC. Ms. Sandra Chfshlmba has been very active and very successful in many areas, including education and training. She was admitted to the Research Master Infection & Immunity in 2010, a programme specifically designed for excellent students, based on her previous study results and the recommendations we got.

During her training in this Research Master programme she demonstrated her intellectual skills, and is one of the best students of her class, Moreover, she developed her curr,ent research partly by herself. RECOMMENDATION FOR SANDRA CHISHIMBA I have worked with Sandra Chishimba in supervisory capacity for more than four years, from the time she was initially a University of Zambia attachment student till she subsequently joined the Malaria Institute at Macha (MIAM), to the present. From continual review of Sandra's work and academic interaction with her in a scientific team environment, I am familiar with Sandra's intellectual abilities and diligence, andl am pleased that she is applying for her doctoral studies with the JHSPH.

At MIAM, Sandra was exposed to hypothesis-driven research, as well as a broad spectrum of basic immunological, molecular, epidemiological and field research techniques. Subsequent to that, she enrolled for her MSc degree with Erasmus University, The Netherlands, where she has been studying for one year. From personal working experience with Sandra, which encompasses field and laboratory research planning, organization, sample and data collection, management, processing, analysis and presentation, I can attest for her diligence and constant drive learn and improve.

Albeit English is not her native language, it is the official and education language in Zambia. Sandra is fluent in both written and oral expression and has some publications. In my view, Sandra has minimal weaknesses, of which limited exposure to quantitative techniques might be one. However, she learns very quickly and may well have since received training during her MSc studies.

A doctoral degree with JHSPH would be highly instrumental to Sandra's work and the path of her scientific career at MIAM and beyond. I confident that Sandra can successfully undertake her studies and that she will make the most of this invaluable opportunity. Sandra graduated from the University of Zambia in 2006 and worked as a student at the Malaria Institute at Macha. Despite only average grades as an undergraduate, she performed so well at MIAM that she was hired as a laboratory scientist and has continued to do very good work.

I became well acquainted with Sandra during my visits in 2008,2009, and 2010. She was already a very productive laboratory worker and had a first author publication in 2007 Medical Journal of Zambia reporting on urinary schistosomiasis, while supervised by Clive Shiff. Sandra subsequently had coauthorships on reports describing antifolate resistant malaria, supervised by Sungano Mharakurwa. Sandra is currently enrolled as a masters degree student at Erasmus University in Rotterdam.

Impressed with Sandra, I nominated her as a student delegate to the 2010 Lindau Meeting of the Nobel Laureates in Germany. I was delighted that Sandra was selected among the most outstanding graduate students selected from around the world. Sandra was featured alongside Bill Gates and Nobel Laureate Ada Yonath in the opening plenary session. I am delighted to write this letter for Ms Sandra Chishimba, whom I have known for nearly seven years - from the time when she first came to the Malaria Institute at Macha (MIAM) in Zambia as a student on a short attachment.

Sandra was subsequently appointed as a lab scientist for MIAM in January 2007, having graduated from the University of Zambia in 2006. In 2010 she was offered a full scholarship at Erasmus University in The Netherlands to pursue a Master's Degree in Infection and Immunology, so she is currently on study leave from our institute as she pursues those studies.

Her work and personality can be characterized as steadfast, reliable and persistent. Whatever assignment she is given, she puts her whole self into it, and often completes it well ahead of schedule. She does not allow other distractions to divert her attention, and is an extremely task-oriented individual. As a result, she is often given significant responsibilities, since it is well known by her supervisor that she will "do it right"! She learns quickly, but also perseveres if she does not understand something, such that she can achieve most things she sets her mind to do.

Upon joining our research lab in 2007, Sandra not only pursued malaria research work in the molecular biology lab, including polymerase chain reaction (PCR) studies to determine genetic differences in human cases of malaria, but also did work on schistosomiasis with Clive Shiff from Hopkins, including maintaining a snail colony. Before leaving for her master's degree studies, she more recently took a lead role in Bill Moss' ELISA studies on malaria antibody changes over time, as she carried out antibody studies on the specimens collected from local people in this previously hyperendemic malaria transmission area. She presented some of this work at the American Society of Tropical Medicine and Hygiene annual meeting in Atlanta, Georgia, USA in November 2010.

Her first-author publication in the Medical Journal of Zambia (Med J Zambia 34(4):141- 145, 2007) on a study of schistosomiasis, is an example of her commitment after being given an assignment. Not only did she do it whole heartedly, but she then pursued it to make sure a publication resulted from the work.

Sandra Chishimba joined my laboratory in February 2011 to do a 6 month lab rotation as part of the Masters program Infection and Immunity of the Erasmus University Medical Center.

This was Sandra's first lab rotation in this program. Before pursuing a Masters degree she had been working in her home country of Zambia as part of a research team monitoring humoral immunity to Plasmodium falciparum, and joining my lab meant making the transition from fieldwork and diagnostics to a basic immunology lab. Sandra managed to cope with this transition very well. She was a quick learner and within 2 Months was able to design and analyze experiments independently.

Throughout her internship Sandra maintained a strong interest in translational studies ultimately aimed at improving public health. She remains very passionate about this, and is very eloquent in her ways to convey this passion. As a result, she was invited to the 2011 Lindau meeting of Nobel Laureates, where she was invited to take part in a forum discussion on Malaria together with Bill Gates, Nobel Laureate Ada Yonath (Chemistry 2009) and Jonathan Carlson (Microsoft Research).

During her internship we discussed her future perspectives, and at that time she already indicated that she wanted to pursue a PhD in medical sciences, preferably again in another country. Sandra's scientific drive and clear ideas on future directions exceed those of many of her fellow students. She enjoys cultural differences, is capable of handling new situations and environments very well and it is my strong belief that Sandra will develop into a successful independent scientist with a broad interest, yet strong translational focus.

I would very much recommend Sandra for the Johns Hopkins Bloomberg School of Public Health PhD program. I am convinced that she will be a valuable addition to the student body, and that she will progress into a successful scientist.
