GROUP BOOKING AGREEMENT FORM

To guarantee your booking, SkiJapan.com requires a $500 per person non-refundable deposit & signed Group Booking Agreement Form within 7 days of confirmation. Full payment of airfares is required within 21 days of a flight booking or as per the individual airline ticketing conditions applicable to your airfare. Final payment is due at least 90 days prior to departure unless otherwise stated.

By signing the above, I understand and agree to the SkiJapan.com Terms and Conditions displayed on our web site and in the brochure. I agree that all names are correct as per passport (s) and that a fee of up to but not limited to $150 will be charged per ticket for any changes.

IMPORTANT: Some Airlines do not allow name changes or corrections, either prior to ticket issue or after. This may result in a loss of confirmed flights and incur additional costs & a higher airfare. Complete name changes are not permitted.

** TRAVEL INSURANCE ** SkiJapan.com highly recommends that our passengers take travel insurance when paying their deposit. There are two methods to apply for a policy:

1. Visit www.skijapan.com and use our online application form (under Travel Shop > Insurance)

2. Or complete an application form and fax completed copy to your SkiJapan.com consultant. Download the form from the web site location as per above.
