AbstractThespreadingofantibioticresistantbacteriaintheenvironmentisathreattohumanhealthbutlittleisknownaboutthetransmissionofextendedspectrumbetalactamases(ESBL)producingEscherichiacolifromthehospitalandmunicipalsewagetothewaterbasinandtotheairattheWWTPs(WasteWaterTreatmentPlants)areaandtheirsurroundings.Accordingly,itseemsparticularlyinterestingtotracethefateofthesebacteriaandtheirgenesencodingantibioticresistanceinbothuntreatedsewagefromhospitals,andinsewageafterdifferentstagesofpurification,andfinallytoexaminethedegreeoftheiremissionstoenvironment.Althoughwastewatertreatmentprocessesreducenumberofbacteriainsewageupto99%,inthepresentedstudyitwasreportedthatmorethan2.710(3)CFU/mLE.colireachedthereceivingwaterandcontributedtodisseminationofresistantbacteriaintotheenvironment.Wereceived395E.colistrainsfromsewageandenvironmentalsamplesandweinvestigatedtheirantibioticsusceptibilityandthepresenceofblageneencodingTEM,CTX,OXAandSHV.Fromamong167and147E.colistrainsisolatedfromhospitaleffluentsandmunicipalsewageinOlsztyn,Poland,upto37.1%and17.7%,respectively,wereESBLpositive.Fromamong38and43strainsisolatedfromriverwaterandtheairupto18.4%and27.9%,respectively,wereESBLproducers.TheblaCTXM(blaCTXM1,blaCTXM3,blaCTXM5,blaCTXM15)geneswerethepredominantgroupoftheplasmidcodedESBLs.Morethan38%outofESBLproducingisolatescarriedseveralblagenes.Themultipleantibioticresistant(MAR)indexesforESBLpositivewerehigherthanforESBLnegativeisolatesandrangedfrom0.45to0.63.TheMARindexesforE.colifromhospitaleffluentsandairsamplesweregreaterthantheindexescalculatedforstrainsisolatedfromothersamples.Presumably,thepreliminarydisinfectionofhospitalsewagebeforeitsinflowintothesewagesystemmightminimizethespreadingofantibioticresistantbacteriatotheenvironment.
