REQUIRED DOCUMENTS Please obtain the following documents and bring to a registration session:

• Proof of Residency for Parent/Guardian (i.e. utility bill, lease, housing contract)

• Student’s Birth Certificate

• Student’s Immunization Record Proof of immunizations to date.

• Photo ID of the person legally authorized to register the student

• Additional Forms, if applicable (i.e. custody agreement)

REGISTRATION SESSIONS Monday, March 2 or Tuesday, March 10 Drop-in between 3:00 - 7:00 PM Ephrata High School cafeteria Children are not required to attend. No RSVP is needed. KINDERGARTEN ASSESSMENTS Week of May 18 - Exact dates vary by school. More info will be provided.

CALLING ALL

Children who will be five years old on or before September 1, 2020 are eligible to be registered for

kindergarten in the 2020-2021 school year.

ONLINE REGISTRATION Visit easdpa.org mid to late February, click the Kindergarten Registration link, and complete the online registration form. This process will take approximately 30-minutes and should be completed PRIOR to attending registration. If you do not have internet access or an e-mail address, assistance will be provided at the time of registration.
