Maximum amount to be on hand, 1 microcurieSealed Americium source to be purchased from Isotope ProductLaboratories - 1800 North Keystone St., Burbank, CA, 91504.Model number of source is #GF241 source to be used as part oftesting process on Advant Tester Model #T3331-B.

4. PURPOSE OF USE

To perform accelerated soft error testing to determine therelative "goodness" or "badness" of ECL SRAM memory devices with ,

respect to alpha particle sensitivity.

Radioactive material to be a sealed source about size ofquarter.

Sealed source, that is in a holder with window on one side, willbe placed within 1 mm of encapsulated /uncapsulated die whilecontinually accessing the part with the SRAM memory tester, inorder to determine the number of times a memory loss occurs.Once placement of radiation source is accomplished, the testwill continue automatically with only minimal supervision andhandling required for a period of two (2) to three (3) days.

9. FACILITIES & EQUIPMENT

| See attachment page 6, number 6 for location of machine in whichthe sealed source is to be used. The machine is part of a testroom which has various test stations that are positioned as farapart as 25 feet and as close as 10 feet. The machines closestto_one using the radiation source are approximately 15 feet tothe left and 22 feet to the right. There is a wall to the rear

and a pole and aisle at the front of machine. Through in-houseventilation, the area has a minimum of four (4) air changes perhour with a minimum of 10% outside makeup air, numbers ofemployees working in area total 18.

10. RADIATION SAFETY PROGRAM

Due to fact that the sealed radiation source will be under one(1) microcurie, the safety program will consist oft

a. Training of users by Anita Taylor on proper handling storageand use of the source. Also procedures of reporting and

'monitoring and what to do in the event of incident.

b. Written procedures for above terminal.

ic. Monitoring of source both at use area and storage location.

Instrument to be used is Trio-Tek Geiger Counter Model#4801952, Serial #7756. The instrument is sent back to thefactory once per year for calibration.

d. Recordkeeping of all activities and monitoring.;

e. Posting of all necessary signs per rules and regulations. '

11. WASTE MANAGEMENTi

The plan is to maintain and use radiation source as long asneeded to accomplish work, maintain the radiation program and
