Requesting Changes

to the Student’s Program or Plan

Examples

1. Student is currently an Accounting major and wants to switch to Business Management.

College = MG

Degree = BSB

See example of how this would look in SIS below.

2. Student’s new status:

College = MG

Degree = BSB

See example of how this would look in SIS below.

3. Since the degree-type remains the same, use a PLAN ONLY CHANGE form to request the change in UAccess.

1. Student is currently a Musical Theater major and wants to switch to Music Education with a Voice sub-plan and Voice Performance as a secondary major.

College = A0

Degree = BFA

See example of how this would look in SIS below.

2. Student’s new status:

College = A0

Degree = BMU

See example of how this would look in SIS below.

3. Since the degree-type is changing, use a PROGRAM CHANGE form to request the change in UAccess.

1. This student is pursuing a BS in Psychology and wants to also pursue a BA in ECOL.

College = A6

Degree = BS

2. Student’s new status:

College = A6

Degree = BA

3. Since a new degree-type is being added, use a PROGRAM ADD form to request the change in UAccess.

1. Student has been NMS in Letters, Arts & Sciences and wants to become a political science major in SBS.

College = LS

Degree = UND

2. Student’s new status:

College = A8

Degree = BA

See example of how this would look in SIS below.

3. Since the degree-type is changing, use a PROGRAM CHANGE form to request the change in UAccess.

UND

If you need to add a secondary program, like in the example below, use the PROGRAM ADD form.

If the student is adding a second major, that belongs to a different college, you use a PLAN CHANGE ONLY form, as long as the second major is in the same degree family.
