ABSTRACTSTHE EFFECT OF 30 HEAD UP POSITIONING ON CHANGES OF VITAL SIGNS ANDLEVEL OF CONSCIOUSNESS FOR PATIENTS WITH SEVERE BRAIN INJURY POST TREPANATIONQuasy Eksperimet Study In Ruang Observasi Intensif (ROI)3rd Floor of Emergency Care Installation(IRD)Dr. Soetomo General Hospital of Surabaya CityBy:JAJUK RETNOWATISevere brain injury oftenbring death in every patients who suffer of it. Prolonged time before patients achieve medical treatmentwill cause temporary or permanently physical defect. Proper medical treatment and quick response willdecrease risk of having both bad effect. This case can be conservatively treat with surgery indeed. Thisbrain injury patients must receive certain nursing care such haemodynamic monitoring, vital signsobservations and position arrangement beside conservative treatment and certain medicine therapy. Thisstudy was aimed to analyze the effect of 30 head up positioning on changes of vital signs and level ofconsciousness in severe brain injury patients post trepanation surgery and used quasy experimental study,in which the population consisted of all severe cerebral trauma patients in ROI. Total sample was 12respondents, divided into two groups, intervention and control. Intervention group received 30 head up

position and control group was 15. The independent variable was 30 head up position and dependentvariable was 4 variable of vital signs (blood pressure, pulse, body temperature, respiratory rate), Mean

Arterial Blood Pressure (MABP) and level of consciousness. Data were collected by using observationsheet and analyzed with paired and independent t Test with level of significance=0.05.In paired t Testcalculation, result showed that 30 head up positioning had significant effect in major of vital signs,

include blood pressure both systole and diastole, pulse, MABP (p0.05. Yet, 30 head up positioning has ahigh average (mean) and more closed to the normal value, compared with 15 head up positioning. Itmeans 30 positioning gave better effect than 15.Lifting patients head in range 30 willoptimalize oxygen supply to the brain and help decrease or normalize intra cranial pressure (ICP).Furthermore, this development in patients haemodinamic and conciousness status, will bring patientsinto recovering condition and avoid death.Keywords: 30 head up positioning, severe brain injury, vitalsigns, haemodynamic, level of consciousness.
