    Re: Permit # 14134311

    P.S. #99 Stan Makowski School

    1095 Jefferson Avenue

    Buffalo,NY 14209

    Dear Operator:

This letter is being sent to notify you that a significant infestation of rodents was identified in the food

preparation or storage areas of the above facility. The Erie County Department of Health recommends

that the facility should cease preparing food and not resume preparing food until the infestation is

controlled. If you choose not to take this recommendation, you are being advised that you must do the

following until the infestation is eliminated:

[ ] Inspect all areas of the facility for contamination prior to daily opening, especially food

items in storage. If food items are identified as contaminated, they must be immediately discarded and the

facility should cease operation until the extermination is complete.

[ ] Clean and sanitizeall food contact surfaces of equipment prior to the first use every day.

This includes, but is not limited to, food prep tables, cutting boards, grills, food storage shelves.

[ ] Clean up any droppings and/or dead rodents and thoroughly clean and sanitize the

surfaces where found. If droppings or dead pests are found on food prep surfaces, the facility should cease

operation until the extermination is complete.

[ ] Contract with a licensed pest management company to eliminate the infestation

Operation of a School K-12 Food Service with an insect or rodent infestation is a violation of the NYS

Sanitary Code and may result in the non-voluntary suspension of your permit to operate if it is determined

that the severity or location of the infestation could result in injury to the health of the consumer or thepublic .

Questions regarding this matter, or requests for inspection should be directed my attention at 961-6800.

    Sincerely,

    Jeffrey Jurewicz
