2021 USC SONG GIRLS PRELIMINARY AUDITION INFORMATION SHEET

Hello, USC Song Girls candidates! In compliance with University of Southern California regulations and government recommendations from organizations, such as the Center for Disease Control (CDC), we have canceled our in-person 2021 USC Song Girls auditions. Instead, we will be hosting our first entirely virtual audition. The criteria for the process is outlined below. It is important that you focus on exuding confidence, poise, and personality throughout your audition. If the safest place for you to record your audition is a front yard, backyard, or public space, it is okay if people or car noises are in the background. The number one priority for audition videos is that you find a level, well lit, and open space where you can perform to the best of your ability. Please note that all audition skills are expected to be maintained at the same performance level and stamina from auditions throughout the entire 2021 season. Preliminary Audition Submission Process

● On Friday, October 2nd at 12:00pm PST, applications are due. ○ Please submit your application and photos to scsg21@usc.edu before October 2 at 12:00pm PST. ○ Any applications timestamped after the deadline will not be considered.

● On Friday, October 2nd at 5:00pm PST, preliminary audition videos are released.

○ All candidates will receive an email containing preliminary audition choreography for video submissions. Please do not post or share this email or its contents.

○ Shortly after, candidates will also receive a personal OneDrive link to upload their video submissions.

● On Sunday, October 4th at 11:59pm PST, all video submissions are due. ○ Candidates upload all video submissions to their “Preliminary Videos” OneDrive folder. ○ Any video submissions uploaded after 11:59pm PST will not be accepted. ○ Additionally, all video submissions must be unedited. Each piece of choreography must be performed in

one continuous video. Any modified or edited video submissions will not be considered.

● By Tuesday, October 6th at 11:59pm PST, the judging panel will have selected candidates for final auditions. ○ All candidates will be notified of the judging panel’s decisions via email no later than Wednesday,

October 7th at 9:00am PST. All details for Final Auditions will be provided at this time. ○ Once again, please do not post or share this email.

Attire: Top: Fitted athletic top. Bottom: Shorts or a skirt. Shoes: Any type of dance shoes or athletic shoes. Hair: Hairstyles should be in a modern style that does not hide your face and complements your features. Make-up: Please wear red lipstick and make-up that compliments your natural beauty. 2021 USC SONG GIRLS ACKNOWLEDGEMENT AND ASSUMPTION OF RISK AND RELEASE

I am aware of the physical activities and dangers involved in participating in the practices, games, clinics, auditions, related travel and all other activities of the USC Song Girls program. I am aware that such activities can be dangerous and involve risk of injuries, which may be serious, including without limitation, damage to joints, ligaments, muscles, bones, neck, spine, and other parts of the body.

My participation in the above events and in all activities related to the above events is a voluntary act with full and complete knowledge of the risks involved. I hereby voluntarily assume all such risks associated with my participation in the above events. Additionally, I agree to exonerate, save, indemnify, and hold harmless the USC Song Girl Program, its employees and volunteers; The University of Southern California, its officers, agents, and employees-including without limitations, equipment personnel, and physicians and other practitioners of the healing arts-from any and all liability, claims, causes of action, or demands of any kind and nature whatsoever, including without limitation, personal injury which may arise from or in connection with my participation in any activities related to the clinic, games, practices, or auditions.

VOLUNTARY RELEASE AND WAIVER OF LIABILITY FOR PARTICIPATION IN PROGRAM AND VIRTUAL PROGRAMS/TELEFITNESS

I recognize that the use of the University of Southern California’s (“USC”) recreational facilities, Including the Lyon Recreation Center, Uytengsu Swim Stadium, USC Village Fitness Center, HSC Fitness Center, Cromwell Field & Loker Track Stadium, and Physical Education Building (PED), and all equipment and facilities contained therein (collectively, the “Recreational Facilities”), have inherent risks of injury or death. I recognize that there is an inherent risk of injury from participating in any virtual fitness programs/ telefitness that are presented by USC via audio, video or other electronic communications in online sessions, online classes, videos, live-stream or other web-based services. I further understand that I select the location where I participate in virtual fitness programs/telefitness, and that it is my responsibility to ensure that my location is safe for participation and that I will have sufficient access to medical attention in the event of injury.

FOR AND IN CONSIDERATION OF USC PERMITTING ME TO USE THE RECREATIONAL FACILITIES AND ACCESS VIRTUAL FITNESS PROGRAMS/TELEFITNESS, I, MY SPOUSE, ASSIGNEES, HEIRS, GUARDIANS, AND LEGAL REPRESENTATIVES HEREBY VOLUNTARILY INDEMNIFY, RELEASE FROM LIABILITY, AGREE TO DEFEND AND HOLD HARMLESS USC AND ITS OFFICERS, TRUSTEES, EMPLOYEES, AGENTS, REPRESENTATIVES, AND ANY DEPARTMENT, ORGANIZATION OR GROUP AFFILIATED WITH USC (COLLECTIVELY THE “USC PARTIES” AND INDIVIDUALLY A “USC PARTY”) FOR ANY ACCIDENT, INJURY, ILLNESS, DEATH, LOSS, THEFT, DAMAGE TO PERSON OR PROPERTY, OR OTHER CONSEQUENCES SUFFERED BY ME THAT ARISE OR RESULT DIRECTLY OR INDIRECTLY FROM MY USE OF THE RECREATION FACILITIES OR PARTICIPATION IN VIRTUAL FITNESS PROGRAMS/ TELEFITNESS, INCLUDING BUT NOT LIMITED TO CLAIMS ARISING FROM OR RELATED TO ANY USC PARTY’S NEGLIGENCE AND/OR PRODUCTS LIABILITY, INCLUDING STRICT PRODUCTS LIABILITY.

IT IS MY INTENTION BY SIGNING BELOW TO EXPRESSLY ASSUME ALL RISK OF PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE UPON MYSELF ARISING OR RESULTING DIRECTLY OR INDIRECTLY FROM MY USE OF THE RECREATION FACILITIES OR PARTICIPATION IN VIRTUAL FITNESS PROGRAMS/TELEFITNESS, TO THE EXCLUSION OF THE USC PARTIES.

By signing this agreement, I waive my right to bring any legal action against the USC Parties now or at any time in the future to recover compensation or obtain any other remedy for any injury to myself or my property or for my death, however caused, arising out of or resulting from my use of the Recreational Facilities or participation in virtual fitness programs/telefitness. I further agree that I, my spouse, assignees, heirs, guardians, and legal representatives will not make any claim against, sue or attach the property of the USC Parties for any loss or damage arising out of or resulting from my use of the Recreational Facilities or participation in virtual fitness programs/telefitness.

I understand that USC requires that ALL participants have current medical/health insurance in order to participate in any physical activity at the Recreational Facilities or virtual fitness programs/telefitness. In the event that I am injured, I agree to assume any financial obligation, either through my health insurance, or through some other means, for any medical costs that I incur. I understand that the USC Parties assume no responsibility for any medical expenses, injury, or damage suffered by me in connection with my use of the Recreation Facilities or participation in virtual fitness programs/telefitness. I understand that USC encourages me to have a physical examination PRIOR TO PARTICIPATION in any physical activity at a Recreational Facility or in any virtual fitness program/telefitness. I understand that USC assumes no responsibility for any existing health conditions that make it inadvisable for me to participate in physical activity at the Recreational Facilities or virtual fitness programs/telefitness. By signing below, I acknowledge that I have current medical/health insurance coverage.

I am aware that this document is a release of liability and a waiver of my legal right to collect damages from the USC Parties in the event of injury, death or property damage as stated above. I am aware this document constitutes a legally binding contract between USC and me, and I sign it of my own free will. I expressly agree that this document is intended to be a release and waiver of liability as broad and inclusive as California law allows and that if any portion is held invalid, I agree that the balance shall, notwithstanding, continue in full legal force and effect. I certify that I am 18 years of age or older and am permitted to sign this form. I have completely read, understand, and voluntarily accept the terms of this agreement.
