Processing Kidney Transplant Referrals More Effectively with a Team-Developed Tool (30 minutes)

This session will discuss the development and use of a scoring grid to stratify transplant patient referrals according to co-morbidities, and easily identify first steps for patient evaluation based on individual needs. The results of a nationwide survey regarding the program’s concern and preparedness for a potential living donor complication, including and most importantly, a living donor death, will be summarized for the audience. We conducted the first randomized controlled trial of a telemedicine-based home monitoring program (HMP) compared to standard of care after liver transplantation. The HMP group had a lower 90-day readmission and improved quality of life in regards to physical function and general health.

10:30 am – 11:30 am - NSQIP Transplant – A Potential Tool to Understand Post-Transplant Outcomes and Complications

Dr. Ryutaro Hirose, MD, FACS will describe NSQIP Transplant, a national transplant QI Program, which is a joint project between the American College of Surgeons and the American Society of Transplant Surgeons that can help programs understand their post-transplant outcomes and complications and provide a national perspective and benchmarks. The ACS National Surgical Quality Improvement Program (ACS NSQIP) is a nationally validated, risk-adjusted, outcomes-based program to measure and improve the quality of surgical care. Recently a transplant specific program has been established. The program was built by transplant surgeons and provides participating hospitals with tools, analyses and reports to make informed decisions about improving quality of care. This session will provide an in-depth overview of the ways some hospitals are using this tool to enhance their quality processes.
