Enrolment Agreement Form This enrolment agreement sets out the responsibilities of the student, parents or carers and the school staff about the education of students enrolled at Benowa State High School.

Responsibility of student to: attend school on every school day for the educational program in which they are enrolled, on time, ready to

learn and take part in school activities act at all times with respect and show tolerance towards other students and staff work hard and comply with requests or directions from the teacher and principal abide by school rules as outlined in the school’s Responsible Behaviour Plan for Students, including not

bringing items to school which could be considered as weapons (e.g. dangerous items such as knives) meet homework requirements respect the school property wear the complete school uniform with no variation.

Introduction to the State School Consent Form (attached) for Benowa State High School This letter is to inform you about how we will use your child’s personal information and student materials. It outlines: • what information we record• how we will use student materials created during your child’s enrolment.

Examples of personal information which may be used and disclosed (subject to consent) include part of a person’s name, image/photograph, voice/video recording or year level. Your child’s student materials:

• are created by your child whether as an individual or part of a team• may identify each person who contributed to the creation• may represent Indigenous knowledge or culture.

Purpose of the consent It is the school’s usual practice to take photographs or record images of students and occasionally to publish limited personal information and student materials for the purpose of celebrating student achievement and promoting the school and more broadly celebrating Queensland education.

To achieve this, the school may use newsletters, its website, traditional media, social media or other new media as listed in the ‘Media Sources’ section below.

The State School Consent Form may, at your discretion, provide consent for personal information and a licence for the student materials to be published online or in other public forums. It also allows your child’s personal information and student materials to be presented in part or alongside other students’ achievements.

The school needs to receive consent in writing before it uses or discloses your child’s personal information or student materials in a public forum. The attached form is a record of the consent provided.

It should be noted that in some instances the school may be required by the Education (General Provisions) Act 2006 (Qld) or by law to record, use or disclose the student’s personal information or materials without consent (e.g. assessment of student materials does not require further consent).

Voluntary There will not be any negative repercussions for not completing the State School Consent Form or for giving limited consent. All students will continue to receive their education regardless of whether consent is given or not.

Consent may be limited or withdrawn Consent may be limited or withdrawn at any time by you.

If you wish to limit or withdraw consent please notify the school in writing (by email or letter). The school will confirm the receipt of your request via email if you provide an email address.

If in doubt, the school may treat a notice to limit consent as a comprehensive withdrawal of consent until the limit is clarified to the school’s satisfaction.

Due to the nature of the internet and social media (which distributes and copies information), it may not be possible for all copies of information (including images of student materials) once published by consent, to be deleted or restricted from use.

The school may take down content that is under its direct control, however, published information and materials cannot be deleted and the school is under no obligation to communicate changes to consent with other entities/ third parties.

Bring Your Own Device Participation Agreement

Parents wishing to participate in the Bring Your Own Device (BYOD) Laptop program should read and understand this document and the Benowa State High School ICT Policy, Mobile and Electronic Device Policy and Responsible Behaviour Plan (found on the Benowa State High School website) before signing the BYOD Laptop Student Participation Form. All Devices must meet the BYOD Minimum Requirements; devices that do not meet the requirements will not be connected to Benowa State High School resources.

Student Resource Scheme It is a requirement of the BYOD program that payments toward the Student Resource Scheme are paid as per the Benowa State High School payment calendar. If payments fall into arrears, Benowa State High School may restrict access to software and resources until you are financial. Lost / Stolen and Damaged Laptops Students are responsible for ensuring laptops are secure at all times, it is recommended that students do not leave their devices unattended in public places. Benowa State High School is not responsible for any damage to student devices and will not be accountable for any lost or stolen student devices or property. Insurance (Own Insurance Policy) When purchasing your laptop please look at options to purchase accidental damage protection (ADP) for your device. ADP covers your device with accidental damage on and off the school campus. Fire, theft and acts of God are usually not covered under these programs and we request you to include it in your personal or home insurance. The insurance can be purchased with your computer vendor or any insurance company. All insurance claims must be settled between you and the insurance company. Statistically, 60% of repairs at Benowa State High School are considered non-warranty. e.g. repairing a cracked screen from a drop. Purchasing insurance for your BYOD is a personal choice.

Warranty It is recommended that your device is covered by an extended warranty, statistically a computer will require, on average, 2.5 repairs during its 3-year life cycle. Administrative Account All students are required to have Administrative Access to their BYOD device to allow connection and installation of Benowa State High School resources. Wireless Networking Benowa State High School uses the 5Ghz wireless band as it provides a more stable connection and less interference. All BYOD Devices must be capable of connecting to a 5Ghz Wireless Network. Benowa State High School will be unable to connect devices that do not support 5Ghz. If you are unsure, consult the manufacturers website. (Some lower cost laptops do not support 5Ghz Wireless.) Battery Life / Charging of devices Students will be expected to bring a fully charged BYOD to school each day. It is recommended that when purchasing a device that battery life is taken into consideration. Check device specifications for battery life reference or consult your technical support. Please note there are a limited number of power points installed in classrooms. Case / Carry Bag A strong carry case is recommended to protect your device from accidental damage like drops. It is expected that all students will use a bag or case designed to hold a laptop with adequate padding when moving around Benowa State High School. Repairs and Maintenance All maintenance for the laptop, operating system, software and/or apps purchased by the family are the responsibility of the family. Families should ensure quick maintenance turnaround for student devices. A limited number of loan laptops may be available from the I.T. office for use while repairs are occurring; this must be pre-arranged with Benowa State High School’s I.T. Department. Backing Up Technology devices can fail, be lost or stolen so it is extremely important that students backup their important files to an external device such as a USB Memory Stick or External Hard Drive. Benowa State High School Technical Support Benowa State High School provides minimal I.T. Support for BYOD devices. If you run into a problem, students are advised to see the Benowa State High School I.T. staff who will attempt to diagnose the fault. If the problem is not able to be resolved by Benowa State High School I.T. staff they may recommend a course of action for repair (eg. Re-install windows, warranty claim, insurance claim etc.) Internet Access Benowa State High School provides secured filtered access to the internet via its high speed wireless network. Student access to the internet is governed by the Benowa State High School’s ICT Policy and Responsible Behaviour Plan. Benowa State High School reminds the Parent/Carer of their obligations under this agreement. 3/4G ability e.g. USB Dongles, should be disabled or not brought to school as this function when activated allows students to bypass the EQ internet security filters. Benowa State High School will take no responsibility for the content accessed by students using 3/4G facility on their personally owned devices. Mobile Device Management Benowa State High School enrols all BYOD devices in the Benowa State High School’s Mobile Device Management system to enable remote installation of Benowa State High School resources. Benowa State High School Software Some subjects require the use of subject specific software, all of which have different licencing arrangements. Where required Benowa State High School may load subject specific software where student licences are available. Licencing arrangements for subject specific software will be managed by the Benowa State High School I.T. Department and relevant Head of Department. Microsoft Office is included under this arrangement. Device Monitoring When connected to Benowa State High School’s network all data transmissions sent and received are monitored and logged by the Department of Education. This includes but is not limited to internet history and email transmissions. Benowa State High School may install monitoring software on BYOD laptops. This software is only active while onsite at Benowa State High School and will monitor and log information such as current open applications. Content on Devices The Parent/Carer must ensure that all content on the BYOD laptop is licensed, legally obtained and meeting the guidelines outlined in the Benowa State High School's ICT Policy and Responsible Behaviour Plan. Unique Student Identifier (USI) Registration (Year 9, 10, 11 & 12 Students Only)

Dear Students and Parents/Carer,

From January 2015, the Australian Government required all students undertaking Vocational Education and Training (VET) to have a Unique Student Identifier (USI). There is no cost to the student. The USI will allow students to access their enrolment and achievement record for all VET learning online (From 1 January 2015); and no VET records will be lost. Without a student’s USI, Benowa State High School will be unable to issue statements of attainments or certificates.

Even if students are not studying any VET courses currently, it is likely that they may need to do so in the future. For example, CPR and First Aid Training is competency-based and will be able to be recorded on a student’s USI number.

Due to this, Benowa SHS requires that all senior students create a USI and provide it to the school as soon as possible. Please help us to complete this process quickly and efficiently.

What do you need to do?

1. Students must ensure that they have an email address (that has space for new messages) which is suitable for the USI number to be sent to. This could be their school email address. Students must ensure that their inbox has free space before they start.

Privacy Statement The Department of Education and Training, through the school, is collecting your personal information in accordance with section 51 of the Education(General Provisions) Act 2006 in order to administer the Student Resource Scheme in an efficient, ethical and secure manner. The information will only be accessed by school employees administering the scheme. Some of this information may be given to departmental employees for the purpose of debt recovery. Your information will not be given to any other person or agency unless you have given permission or the Department of Education and Training is authorised or required by law to make the disclosure. Purpose of the Scheme 1. In accordance with the Education (General Provisions) Act 2006, the

cost of providing instruction, administration and facilities for theeducation of students enrolled at State schools who are Australian citizens or permanent residents, or children of Australian citizens orpermanent residents, is met by the State.

2. Parents are directly responsible for providing textbooks and otherpersonal resources for their children while attending school. Inrecognition that these costs can be high, the school operates aStudent Resource Scheme (the “scheme”) that enables a parent toenter into an agreement with the school that, for a specified annualparticipation fee, provides for the temporary use by the student ofprescribed textbooks and/or other resources, and/or the purchase ofconsumables and materials for the student.

3. A Student Resource Scheme is separate to and distinct from a requestfor a voluntary financial contribution.

Benefits of the Scheme 4. The purpose of the scheme is to provide the parent with a cost

effective alternative to purchasing textbooks and/or resourceselsewhere, through reduced prices gained from the school’s bulkpurchasing practices.

5. The scheme also ensures that students have consistent personalresources for their education, and saves the parent time and money insourcing the prescribed materials elsewhere.

6. The scheme is not used to raise funds for other purposes, and revenuecollected through the scheme is applied only to the operation of thescheme.

Participation in the Scheme 7. Participation in the scheme is voluntary, and there is no obligation on a

parent to participate in the scheme. A parent’s decision to participate ornot is based on consideration of the value afforded to them by thescheme.

8. The completed and signed Participation Agreement Form should bereturned to the school by the advertised date to indicate participation ornon- participation in the scheme.

9. If a parent chooses to participate in the scheme and completes theParticipation Agreement Form, a fee will be due and payable by theparent for the items provided by the scheme to the student.

10. Payment of the participation fee is a requirement for continuedparticipation in the scheme. An unpaid fee will be subject to the school’sdebt collection processes.

11. A parent who does not wish to participate in the student resourcescheme shall inform the school by completing the ParticipationAgreement Form and returning it to the Administration Office.Arrangements will be made for the parents of students in Years 7 to12 or of secondary education age to be paid the value of the Textbookand Resource Allowance for each of their eligible children.

12. A parent who chooses not to participate in the scheme is responsiblefor providing the student with all items that would otherwise be providedto the student by the scheme as detailed on the Year LevelRequirements List and/or Subject Requirements List, to enable thestudent to engage with the curriculum.

Parents and Citizens’ Association Support of the Scheme 13. The operation of the Student Resource Scheme is discussed annually

at a meeting of the Parents and Citizens’ Association. Parents areextended an invitation to attend this meeting and provided with anopportunity to express their opinions. A vote is taken at this meeting onsupporting the operation of the scheme.

Textbook and Resource Allowance 14. The Queensland Government provides financial assistance to parents

of students in Years 7 to 12 or of secondary education age, attendingState and approved non-State schools, to offset the costs of textbooks and other resources. Assistance is provided in the form of a Textbook and Resource Allowance which is paid once a calendar year through the school. Parents have the option to receive the allowance directly from the school or as an offset of fees associated with participation in the school's Student Resource Scheme. This option is made available to each parent annually by the school. Current allowance rates are available fromhttp://education.qld.gov.au/finance/grants/fund/garp/html/par-stu.html

Payment Arrangements 15. Payment of the participation fee may be made by EFTPOS (Debit

Card; MasterCard; Visa; American Express where accepted by theschool), cheque, or cash.

16. Payment may be made through the QParents portal using a creditcard. Payment will be directed from the QParents portal to BPOINTwhere relevant outstanding invoice information may be selected.

17. When paying by BPOINT, please use the Customer Reference Number (CRN) and invoice number printed on the invoice received from theschool. If unsure of your CRN, please contact the school.

18. Payment of the participation fee may be made in whole or by instalmentas indicated on the Participation Agreement Form or for another amountas approved by the Principal.

19. For payments made in person by cash, cheque or credit/debit card, areceipt will be provided to the payer. Where payments are received byother methods, a receipt will be provided on request. All receipts anddocumentation on the scheme should be retained by the parent forfuture reference and possible taxation purposes.

Parent Experiencing Financial Difficulties 20. A parent wishing to participate in the Student Resource Scheme and

experiencing financial hardship is encouraged to contact the principalto discuss how their financial obligations can be met throughout theschool year, or to negotiate alternative arrangements that may beavailable to accommodate their individual circumstances. Centrelinkdeduction is a payment option offered by the school. All discussionswill be held in the strictest confidence.

21. The onus of proof of being so affected is on the parent, and might includea current Health Care Card or other evidence to establish financialhardship.

22. Any concessions given to the participation fee will be at the discretion ofthe Principal.

Terms and Conditions of Participation in the Scheme 23. Payment of the prescribed fee implies acceptance of the Student

Resource Scheme including the Terms and Conditions irrespective ofwhether or not the signed form has been returned.

24. For the purposes of the scheme, where a student is living independentof his/her parents, reference in the terms and conditions to “parent” is areference to the independent student.

25. Where a parent chooses to join the scheme and completes aParticipation Agreement Form, an annual participation fee will be due and payable by the parent to the Student Resource Scheme.

26. In return for payment of the participation fee, the scheme will provide theparticipating student with textbooks and/or resources, consumables andmaterials as listed on the Year Level Requirements List and/or SubjectRequirements List as being provided by the scheme, when due for thestudent’s use.

27. Payment of the participation fee is to be made according to thepayment arrangement option indicated by the parent on the ParticipationAgreement Form, or as otherwise approved by the Principal.

28. Non-payment of the participation fee by the designated payment date(s)will generate a reminder notice to the parent from the school at intervals of 30 days overdue and 60 days overdue. The Principal may thereafter undertake debt recovery action for the overdue participationfee including, where warranted, referral to an external debt collectionagency. This may result in extra costs being incurred by the parent.

29. Without limiting any other action the Principal may take in accordancewith these terms and conditions, subject to 26 above, the Principal maywithdraw a student’s participation in the scheme due to non-payment ofa participation fee, and/or may require the return of items provided bythe scheme for the temporary use of the student, and/or may withholdthe giving of any further items under the scheme until the debt is paid.

30. The Principal may refuse to admit a student to the scheme whereparticipation fees are overdue from the previous year’s scheme.

31. Where participation fees are overdue, the Principal has the discretionto exclude a student from an optional school activity.

32. The scheme provides the entire package for the specified participationfee, and is not available in parts unless specifically provided for by the school in the fee structure.

33. All textbooks and resources provided for temporary use by the schemeremain the property of the scheme and shall be returned at the end ofthe course or school year or when the student leaves the school,whichever is the earlier. Where an item is not returned, the parent willbe responsible for payment to the scheme of the replacement cost ofthe item. Failure to make payment may result in debt recovery actionbeing undertaken.

34. Textbooks and other resources provided for temporary student use by the scheme shall be kept in good condition by the student. The school Administration Office shall be notified immediately of the loss ornegligent damage to any issued item. Where an issued item is lost ornegligently damaged, parents will be responsible for payment to the scheme of the replacement cost of the item. Failure to make paymentmay result in debt recovery action being undertaken.

35. The parent is responsible for supplying the student with any additionalstudent materials that are not provided by the Student ResourceScheme, as indicated on the Year Level Requirements List and/orSubject Requirements List or otherwise advised by the school.

36. If a student enters the scheme after the first week of school, a pro-rata participation fee will apply, calculated on a 40-week school yearbasis.

37. If a student, having paid a participation fee, leaves the school throughthe year, a pro-rata refund will be made to the parent, calculated on theparticipation fee paid, less the cost of consumed materials and thereplacement cost of scheme items that are lost or negligently damagedor not returned, plus the pro-rata Textbook and Resource Allowancecalculated on a 40-week school year basis.

38. As the scheme operates for the benefit of participating parents and isfunded solely from participation fees, resources provided by the schemewill not be issued to students whose parents choose not to participatein the scheme

Dear Parent/Caregiver 

2020 Sport Program 

The 2020 Wednesday sport program has changed.  Instead of a weekly sport program, students will participate in 5 full day Cluster Days in Term 1, 2 and 3.  They have the opportunity to participate in the Gold Coast 5 (GC5) Interschool competition which is organised into two tiers.  GC5 schools competing in this competition include Palm Beach Currumbin SHS, Robina SHS, Elanora SHS and Pacific Pines SHS.  All information for GC5 will be disseminated via the newsletter, parent email and student letters.  There will also be options for recreational activities and some invitation only special programs.  

Students choosing the option of GC5 will have the ability to choose what sport they play and therefore, be able to choose to play with their friends. The Tier 1 competition will cater to students wishing to participate in a higher level and competitive environment whilst Tier 2 competition will cater to those students who want a slightly less competitive, more social environment.  Tier 1 and 2 teams will be decided after preliminary options have been selected. 

We are currently finalising alternative activities for the recreational sport program and further detail will be provided ASAP. However, students electing to participate in the recreational sport program will be allocated activities and groups to fit in with maximum activity numbers and staffing. Whilst we feel that the range of activities offered in the recreational program will provide both value for money and meaningful experiences, we reiterate that students will not have the opportunity to choose activities and groups and therefore may not have the opportunity to be in the same group as their friends. There will be a small number of students participating in alternative activities organised by various departments from the school. These students will be invited in to the activities and directly liaised with by the program managers.  Given that students will be representing the school in both GC5 and Recreational Sport programs, they are expected to adhere to the schools behaviour management plan and standards.  

Sporting options will be offered online only and a link will be sent in Week 1 to your nominated parent email address provided to the school.  We recommend you log on immediately due to limited spaces in teams to avoid disappointment.  Please ensure your contact details are up to date with the Administration. These options will be required by Thursday 13 February (Week 3) to ensure nominations for teams are submitted on time.  Should teams in the GC5 competition not have enough students to be filled, students may be required to re‐select (however, this will be done in‐person with the Director of Sport). 

GC5 Interschool Sports Program Benowa students will have the option of participating in a wide array of sports (see page 2) for the day in the GC5 competition (formerly interschool sport).

2020 – WRITTEN PERMISSION FOR PARTICIPATION IN SCHOOL SPORTS AND ASSOCIATED TRANSPORT

Throughout the school year, parents/guardians receive many different letters from the school that often require reading, signing and returning to the school to allow their child to participate in aspects of the School Sports Program, that may include transport to and from a number of off‐campus venues, used for the conduct of sports activities. 

In an effort to reduce the amount of paperwork and processing time, you (parents/guardians) will be asked to sign a letter of permission to allow for the participation of your son/daughter in the sports activities that will be held throughout the year as a part of the schedule of activities that occur during the course of the normal school day. This will include the majority of activities conducted as part of the Health & Physical  Education  curriculum;  the  Homestead  Carnivals  in  Swimming,  Cross  Country  and  Track  &  Field  and  the  majority  of  activities conducted as part of the GC5 Competition (5 Sport Cluster Days). 

For all these activities, you are asked to sign a ‘once off’ form that will give permission to allow for the participation of your child in sports activities  for  the  year  and  transport  to  and  from.  The purpose of  this  approach  is  to  significantly  reduce  the number of  forms  that  your son/daughter bring home for signing. There will still be a regular communication with you as to the schedule and nature of activities being conducted.  

Activities that involve contact sport (such as water sports), plus activities with an assigned High Risk rating (from Education Queensland) and also any participation by your child in tournaments that involve overnight accommodation and/or significant involvement in out of school time, will still require specific and signed permission forms (which will be distributed after sports selection). 

In the majority of circumstances for the School Sports Program, transport is by bus. Venues include other high schools, community and Council sport grounds and stadiums, swimming pool and athletics tracks. In some cases, maxi‐taxis will be required rather than a bus. For venues close to school (for example; Sir Bruce Small Park, the Environmental Park, Benowa Road Netball Courts, Ice‐skating etc.) students will walk under teacher supervision to and from the venues.

Activity risks and insurance Please note that the Department of Education does not have personal accident insurance cover for children/students. If your child is injured as a result of an accident or  incident while participating in the activity, all costs associated with the injury,  including medical costs are the responsibility of the parent/carer. Some incidental medical costs may be covered by Medicare. If you have private health insurance, some costs may also be covered by your provider. Any other costs must be covered by parents/carers. It is up to all parents/carers to decide what types and what level of private insurance they wish to arrange to cover their child. Please take this into consideration in deciding whether or not to allow your child to participate in this activity.

Dear Parents & Citizens

Benowa State High School’s Parents & Citizens Association welcomes you and your student(s) to Benowa State High School.

The Parents and Citizens Association (P&C) is a committee which informs and offers valuable family-based feedback on School programs and facilities, and which fundraises by various methods to support the School in those endeavours.

Together, we (the P&C) operate the School Canteen and Uniform Shop – with all profits from these enterprises directed back to the school to provide equipment & services, and to enhance facilities for our students.

We support and recognise the higher achievements of students by paying for the Gold Day In each year and making a donation to students who reach Regional, State or National levels of excellence in their chosen sport.

Families can support the School via the major fundraising initiatives of:

• The Parent Contribution Scheme - a voluntary, contribution of:o $50 per student, $80 for two students, or $100 per family

• Contributions to the Benowa State High School P&C Association Building Fund - Tax deductible, parent voluntarycontribution payments can be made at the Cashiers office or through internet banking.

These parent voluntary contributions are essential for Benowa State High School to continue its delivery of a first–class educational environment for your children. The P&C and students greatly appreciate your active support.

Through these strategies, over the past 5 years the P&C has wholly financed the air-conditioning of the entire school. We have also financed the refurbishment of the Collaboration Centre, the purchasing of new outdoor seating and sourced grant funding to assist school music programs.

Your contribution towards such projects is greatly appreciated and is vital to resourcing Benowa State High School, its student programs and facilties – to maintain its outstanding performance and build upon it into the future.

You can also support your school as a parent with your time, perspective, knowledge or expertise – by joining and attending the P&C Association, and making a positive regular contribution as a parent voice of the Benowa State High School community.

Our meetings are held on the third Wednesday of each month at 6.30pm in the W Block Conference Room - you are warmly invited and encouraged to attend our meetings where your input will be genuinely welcomed. W Block is the multi-storey building on the school site in Mediterranean Drive, Benowa.

SCHEDULE 2 – Code of Conduct for P&C Association This Code of Conduct clarifies the expected standards of all P&C Association members. It has been adopted to aid all members of our school community to work as a cohesive group in partnership with the school.

The Code is intended to promote and maintain the highest standard of ethical behaviour by P&C Association members. Members of a P&C Association should adhere to the Code of Conduct at all times.

This Code applies to each member of a P&C Association.

P&C Association members are to:
