Dear Industry Colleagues:

On behalf of the Pacific Coast Surgical Association (PCSA), I invite you to join us for the 90th Annual Meeting, February 15-18, 2019 in Tucson, Arizona at the JW Marriott Tucson Starr Pass Resort & Spa.

PCSA membership includes many hospital leaders and department chairs from all of the major medical centers on the West Coast, who carry significant impact in their respective organizations. PCSA members are community and academic leaders in surgery who come from one of four Caucuses: Southern California, Northern California, Oregon/Hawaii, and Washington/British Columbia/Alaska.

PCSA holds an Annual Scientific Meeting every February to keep our member surgeons up-to-date on the latest in medical research and clinical practice. High quality presentations cover general and specialty surgery and offer AMA PRA Category 1 Credit and interactive Maintenance of Certification credits.

For those of you who have supported the PCSA in past years, we again thank you for your support and strongly affirm that the success of our meeting and our effectiveness as a profession are intrinsically linked to the technology and innovation that you provide. For those of you who are new to the PCSA, this is a vibrant, very active group of community and academic surgeons with membership from the entire North American West Coast, representing the core of surgical innovation, advanced education, and early adoption in our region.

We intend to match the beauty of the Arizona landscape with great science and fellowship at another superb meeting, and we welcome our industry partners in our efforts. Over 250 general surgeons and surgical educators, with interests in hepatobiliary, endocrine, trauma, vascular, colorectal, cardiac, and transplant surgery, along with many residents, will be in attendance. They look forward to learn both at the meeting and from conversations with you during breakfast, at breaks, and before and after the poster sessions. As in the past, all attendees will be strongly encouraged (and rewarded in a raffle) to visit with you and learn about your company and the technology and innovations that you offer for the care of our surgical patients.

On behalf of all my colleagues in the Washington/British Columbia/Alaska Caucus, we hope to have you join us next February at the JW Marriott Tucson Starr Pass Resort & Spa in Tucson, AZ. We are deeply grateful for your support, which makes our meeting possible.

Exhibit Deposit: A 50% deposit of $1,575 is required with this application. THIS DEPOSIT WILL NOT BE REFUNDED. Full payment is due on January 2, 2019. If an exhibitor cancels space after January 2, 2019 there will be no refunds.

Cancellation and Refunds: Written notification of a space cancellation must be received by PCSA before January 2, 2019 to be eligible for a refund. A $1,575 service charge will be assessed for each cancelled space. No refunds will be given after January 2, 2019.

1. Eligibility to ExhibitCompanies involved in providing education, supplies, and services to the medical profession: pharmaceu-ticals, instrumentation, practice management, publications, computer software, and so on.

All products must be directly related to the practice of surgery or medicine and approved by PCSA in its sole and absolute discretion. Exhibitors shall not exhibit products or services other than those described in the application and approved by PCSA.

A product that is an “investigational device or drug” must be clearly marked as such. Exhibitors must ensure that with respect to all such devices or drugs only permissible preapproved dissemination of scientific and educational information is provided at the exhibit and that appropriate disclosure and cautionary notices are included with the devices or drugs.

All products and services exhibited at PCSA shall comply with FDA rules and regulations. For additional information, contact FDA at 888-463-6332 or www.fda.gov.

An exhibitor shall not in any manner directly or indirectly imply that a PCSA endorsement or approval of exhibitor’s product or service has been given merely because PCSA approved such product or service for display as an exhibit.

Use of the PCSA logo on signs or materials distributed is strictly prohibited without the written consent of PCSA. The use of logos, seals of approval, trademarks, or other similar property rights that are in disuse may not be used in connection with any product or service or advertising material displayed or disseminated at the exhibit.

Applications deemed ineligible by PCSA will be returned with full payment.

Written notification of a space cancellation must be received by PCSA on or before January 2, 2019 to be eligible for a refund. A $1,500 service charge will be assessed for each cancelled space. No refunds will be given after January 2, 2019.

2. Exhibit feesOne single 6’ tabletop exhibit costs $3,150. All applica-tions for space must be accompanied by a non- refundable deposit of $1,575 per tabletop display.

3. Exhibitor RepresentativesEach exhibiting company is granted 2 badges as part of its exhibit space rental fee, provided that the exhibitor representatives are registered in advance. Additional and replacement badges will be available at $400 each in advance and on site. Only exhibitors with badges will be permitted to enter the exhibit area during hours of installation and dismantling.

4. Failure to Occupy SpaceAny space not occupied by 5pm, Friday, February 15 will be forfeited by the exhibitor. This space may be reassigned or used by PCSA without refund, unless arrangements for delayed occupancy have received prior PCSA approval.

5. LiabilityIn the event the exhibition is cancelled, postponed, or relocated on account of fire, strikes, government regulations, causalities, Acts of God, or other causes beyond the control of PCSA, the exhibitor waives any and all damages and claim for damages and agrees that the sole liability of PCSA shall be to return to each exhibitor the exhibitor’s rental payment.

6. Payment TermsA non-refundable deposit in the amount of $1,575 per tabletop must accompany the application. No applications will be processed without remittance of the deposit. PCSA will charge credit cards the full $3,150 per tabletop space requested for 2019 PCSA. All applications received after January 2, 2019 must include full payment. No application will be processed without full payment after this date.

Full payment is due January 2, 2019.

7. Special Restrictions• Giveaways are permitted only if approved in

writing by PCSA prior to the meeting. • Any activity in the exhibit aisles is strictly

prohibited.• Fire hose cabinets and exits must be left acces-

sible and in full view at all times. Decorations and displays must either be of non-flammable material or be treated by an approved fire prevention method. All display material must be flame proofed and is subject to inspection by the Arizona Fire Department. No flammable fluids or substances may be used or shown in booths.

• Displays may not be dismantled nor any packing before 11:30am on Sunday, February 17.

8. SecurityPCSA cannot guarantee or assume any liability against loss or damage of any kind but will endeavor to protect exhibitors by providing a security guard after meeting hours.

9. Signs/AnnouncementsNo signs, posters, or brochures will be allowed any-where in the hotel except within each exhibitor’s booth. No signs or other articles are to be fastened to walls or electrical fixtures. The use of thumbtacks, tape, nails, screws, bolts, or any tool or material that could mark the floor or walls is prohibited.

10. Exhibitor Events/Function SpaceExhibiting companies may not conduct meetings or events that take place during the PCSA program without prior written approval, including all symposia, lectures, focus groups, sessions, staff meetings and social events.

11. Enforcement of Rules and RegulationsThe Rules and Regulations of the PCSA Exhibition are intended to bring order and equity to all parties involved. In addition to being subject to restriction or termination of an exhibit, exhibitors who violate regulations could be prevented from exhibiting at future meetings. PCSA reserves the right to restrict or terminate an exhibit without notice if an exhibitor violates any of the Rules and Regulations contained herein. In the event of such restriction or eviction, PCSA is not liable for any refunds, rentals, or other exhibit expense.

PCSA has secured a block of rooms for attendees and exhibitors. The PCSA Single/Double Rate is $195 single/double per night plus applicable state and local taxes (currently 12.05% and $4.00 occupancy tax) and a $5.00 (normally $29) resort fee per room per night.

The group rate will be honored three days prior and three days post-meeting dates, based on availability. The cut-off date to reserve a room at the group rate is Wednesday, January 16, 2019. Reservations received after the cut-off date, or after the room block fills, will be based on space and rate availability.

EARLY DEPARTURE AND NO SHOW FEES To avoid an early checkout fee of one night’s room and tax, please advise the Hotel at or before check-in of any change in planned length of stay. For guests with confirmed reservations that are a no show, a fee equivalent to one night’s room and tax will be charged to the credit card on file and the reservation will be canceled.
