NCH rewards you for being healthy! With the NCH Win! Wellness Program, you and your

spouse can earn a wellness reward by completing certain health actions.

To be eligible to receive the Reward you earn, you must enroll in a 2019 Medical Plan option

and the Reward will be given as a credit to your medical plan premium each month in 2019.

Participation in NCH Win! is voluntary. However, to earn the Reward, you will need to

participate in the program activities and earn the required percentage points.

The time period for completion of activities is September 1, 2017 to August 31, 2018.

The deadline to complete eligible health actions is August 31, 2018.

Health Numbers (Associate only)

The Rally Health Survey is an online health assessment. You MUST complete the

Rally Health Survey before you can earn any Wellness Rewards at all! If you are enrolled in a UHC medical plan, you can access Rally by logging into your account at

www.myuhc.com

If you are enrolled in the ImagineHealth/UMR medical plan, you can access Rally by logging into your

account at www.umr.com

If you do not currently have medical coverage through NCH, you can access Rally by visiting

https://www.werally.com/client/nchcorp/register

Some questions on the Health Survey ask for your health numbers. If you have already had a biometric

screening, these numbers will be pre-populated. If you have your health numbers handy, enter them in the

appropriate places. If you don’t know your health numbers at the time that you complete the Health

Survey, it is okay to skip those questions. Skipping this section will give you a less accurate estimate of

your “Rally Age” but will not affect your eligibility to get credit for the Health Survey completion.

A Biometric Screening is a test that measures your height, weight, and blood pressure. It also includes a blood draw to determine your blood glucose or Hemoglobin A1c level. An onsite biometric screening event is available to DFW area associates who are enrolled in a UHC or

ImagineHealth/UMR medical plan. If you participate in this event, your health numbers will be submitted

to Rally for you within 3 weeks.

If you are unable to attend the onsite event (i.e. schedule conflicts, not in the DFW area, not enrolled in an

NCH medical plan), you MUST print the Physician Form (see pages 10 and 11 for instructions), have your

doctor complete the form, and fax the completed document to the number on the form.

Only the Physician Form will be accepted for offsite biometric screenings

Spouses must use their own Physician Form to get credit for this activity.

The Biometric Screening will give you your Health Numbers. To get credit for this activity, your numbers

must fall within the following ranges:

Body Mass Index (BMI) <27.5 — 10%

Blood Glucose <100 or Hemoglobin A1c <5.7 — 10%

Blood Pressure <130/85 — 10%

Documentation (Associate only) Completion certificates are emailed to associates when they complete the Step2It Challenge, Live

Healthy Lifestyle Challenge, Naturally Slim program, NS4You program, NS4Life program, Eat

Right For Life program and required tracking of physical activity in the Live Healthy NCH website.

Once a completion certificate is received, the associate can verify completion of the activity in the

“Rewards” section of the Rally website by completing the self attestation for that activity.

Earning Your Points

If you claim credit for an activity in the Rally website for which you

have not received a completion certificate, that credit will be

deleted from your record during audit.

Your health plan is committed to helping you achieve your best health. Rewards for participating in

the wellness program are available to all benefit-eligible associates. You must be enrolled in a

2019 NCH Medical Plan to receive the Wellness Reward if you qualify. If you think you might be

unable to meet a standard for the health numbers and are medically unable to complete any of the

alternate activities described in this booklet, you might qualify for an opportunity to earn the same

reward by a different means. Contact us at NCHWellness@nch.com and we will work with you to

find a wellness program with the same reward that is right for you due to your health status.

Rally—Quick Start Guide

UHC Members

RETURNING USERS If you completed the Health Survey last year, you already have a Rally

account. You will need to complete a new Health Survey before you can

access your dashboard.

NEW USERS You will need to create a Rally account. If you currently have a medical

insurance plan through NCH, you will access Rally using the same steps listed

on the previous page based on your carrier.

Rally Sign Up Process Click the “Sign Up” button on the Rally Welcome Page

Enter an email address and password

Check the box to accept the terms and conditions, then click “Next”

Check the box to acknowledge HIPAA Authorization, then click “Next”

UHC and ImagineHealth/UMR members: Enter your Member ID number as shown on

your medical insurance card in the field labeled “Alternate ID”

Non-Covered Associates: Enter the 9-digit Employee ID number as shown on your

cover letter in the field labeled “Alternate ID”

Fill in the remaining fields, then click “Let’s Go!”

Choose an avatar and create a permanent username for your account, then click

Rally—Quick Start Guide

NOTE: Only associates who DO NOT participate in an onsite biometric screening

event will need to print the Physician Form. This is the only form that will be

accepted for offsite biometric screenings. Forms will be available on April 1, 2018.

STEP 1: Log in to your Rally Account. On the Dashboard page, click on the “View

Details” link in the Employer Rewards section

STEP 2: Scroll down to find the Reward activity to “Complete a Biometric Screening”

and click the green “Register” button on the right

STEP 4: You will be redirected to the Quest Diagnostics website

STEP 3: Click the “OK” button to leave the Rally website

(Quest is the vendor that will process all biometric screening forms and coordinate with Rally)

Click “Accept and Continue” on the Terms and Conditions page

Enter your personal information (be sure to complete all fields), then click “Save”

Under “Self-Reported Nicotine Status”, click “Take Custom Questionnaire”. Answer the

question about tobacco usage and then click “Continue”

STEP 5: In the Wellness Screening section, click the “Order Form” button

STEP 7: After the form is downloaded, print it and take it to your Primary Care

Physician to be filled out

up the NCH Win! Wellness Reward Program, please visit www.livehealthynch.com

and click the Wellness Resources tab at the top of the page. Find the box labeled

“Wellness Reward Resources”.

There you will find:

***ATTENTION**

If you plan to get an annual physical, you do NOT have to wait a full 12 months to do so!

UnitedHealthcare and ImagineHealth/UMR plan members can have an annual physical exam

once every medical plan year, regardless of when the exam was done in the previous plan

year. NCH’s medical plan year is January 1, 2018—December 31, 2018. You can schedule your

appointment with your doctor anytime within this time frame and your plan will cover the cost.

If your provider indicates that you must wait, please be advised that this may be a provider

policy, as it is not an insurance policy. If you want to verify this information, feel free to

contact the customer service phone number listed on the back of your medical ID card.

If you plan to get your Biometric Screening done during your annual physical exam, you are

encouraged to schedule your exam prior to August 1, 2018 to avoid delays in processing due to

increased volume of submittals as the wellness deadline approaches.
