Physicians The American Diabetes Association is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. The American Diabetes Association designates this live activity for a maximum of 8.75 AMA PRA Category 1 credit(s).™ Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Nurses: The American Diabetes Association is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s COA.
Dietitians: The American Diabetes Association is a Continuing Professional Education (CPE) Accredited Provider with the Commission on Dietetic Registration (CDR). Certified Diabetes Educators: To satisfy the requirement for renewal of certification by continuing education for the National Certification Board for Diabetes Educators (NCBDE), continuing education activities must be diabetes related and approved by a provider on the NCBDE List of Approved Providers (www.ncbde.org). NCBDE does not approve continuing education. The American Diabetes Association is on the NCBDE List of Approved Providers.
Successful Completion Statement To Claim Continuing Education Credits Certificates of Completion/Attendance are provided to registered attendees based upon completion of the online evaluation available through April 6, 2018. The evaluation will be emailed to registered participants after the program. *Attention Pharmacists: Upon closing of the online evaluation the American Diabetes Associ-ation will upload the needed pharmacy-related continuing education information to the CPE Monitor within 60 days. Target Audience

2018 Registration Information
Pre-registration is required for the 2018 American Diabetes Association - Wisconsin Area Conference being held on March 16 & 17. The cost of the conference is $175 for professionals and $100 for full-time students. Fee includes breakfast, luncheon, and beverages. Online registration is highly recommended at http://professional.diabetes.org/wisconsin. If you are unable to register online, please return this form with credit card information or your check made payable to American Diabetes Association no later than Wednesday, March 7, 2018. A late fee of $25 will apply if postmarked after March 7th. Fee applies whether attending all or part of the conference. Room Reservations: When contacting Chula Vista Resort for reservations, please mention reference #F85196.A conference room rate is $119/night for single or double and room reservations must be made by Tuesday, February 13, 2018. Contact the hotel for more information at 877-618-0035. Cancellation Policy: If notice of cancellation is received at the American Diabetes Association office by Wednesday, March 7, a full refund will be made. Cancellations received after March 7 are non-refundable. Please inquire when making your hotel reservations about their cancellation policy. Pre- and Post-test Questions: Pre-and post-test questions will be sent to all participants. Therefore, an e-mail address will be required when register-ing.
