Know Your Numbers Campaign

• In order to qualify for the 2019 HealthyMe rate, you and your spouse/domestic partner must

each complete and submit an individual biometric screening and confidential online Personal

Health Assessment (PHA) by July 31, 2018.

• The HealthyMe rate allows colleagues to receive:

*Excludes Hawaii HMSA (Hawaii HMO and Hawaii PPP).

$600 reduction on your 2019 annual medical plan

contributions if you participate and are enrolled in

a Marsh & McLennan Companies medical plan*;

OR if you and your spouse/domestic partner are

both enrolled in a Marsh & McLennan Companies

medical plan*, and only one of you completes the

requirements (this is known as a “Blended rate”).

$1,200 reduction on your 2019 annual

THREE WAYS TO COMPLETE YOUR BIOMETRIC SCREENING

Go to Colleague Connect, click Healthyroads under Tools in Career & Rewards to schedule your

appointment. Your spouse/domestic partner will need to register separately for their own

appointment.

• Sign up for an onsite screening at a Marsh & McLennan Companies office (onsite),

• Visit a Quest Patient Service Center (PSC), or

• See your health care provider and submit your completed Healthyroads Biometric Screening

Health Care Provider Form to Healthyroads.

COMPLETE AND SUBMIT AN ONLINE PERSONAL HEALTH ASSESSMENT

• You and your spouse/domestic partner must each complete and submit a confidential online

PHA by July 31, 2018.

2018 Incentive Changes

2018 Incentive Changes

• If you meet the plan’s eligibility criteria, the Healthyroads Lifestyle Coaching Program (by

telephone, video and/or chat) is available to you and your spouse/domestic partner.

• The $150 gift card incentive will no longer be offered beginning June 1, 2018. Participants

who completed four coaching sessions, an online PHA and biometric screening prior to June

1, 2018 will remain eligible to receive the $150 gift card incentive.

• You will have to log into the Healthyroads website to redeem the $150 Healthyroads® Visa®

Prepaid Card. Any $150 Healthyroads Visa Prepaid Card earned but not redeemed by May

31, 2018 will be automatically mailed on June 30, 2018.

• Each individual $150 Healthyroads Visa Prepaid Card redeemed is taxable to you. Thus, if

you and your spouse/domestic partner each receive a card, you will be taxed on both your

card and your spouse/domestic partner’s card.

• Once you have registered on the

HealthyroadsTM website, obtain your

biometric screening one of three ways

on the HealthyroadsTM home page:

• Click Schedule your Biometric

Screening to schedule at an:

1. Onsite Event.

2. Patient Service Center.

• Click Biometric Screening Health

Care Provider Form

3. To submit a completed Health

Care Provider Form to

Healthyroads, if you are

completing your biometric

screening with your health care

provider.

Scheduling a Biometric Screening on the HealthyroadsTM Website


From the Healthyroads home page, access the

Quest Diagnostics Health & Wellness

appointment system:

• Click Schedule your Biometric Screening.

If you are a New User:

• Enter MMC in the Registration Key and click Register

Now.

• Accept the Quest Terms and Conditions.

• Enter your Employee ID, birth date and select if you are

an employee or non-employee from the relation drop

down menu. (If you are a spouse or domestic partner,

select non-employee.) Your spouse/domestic partner will

enter your Employee ID plus “S” and their own birth date

to register.

• Enter a username and password and click Continue.

• Review your personal Information and click Save.

If you are a Returning User:

• Enter your username and password and click Log In and

accept the Quest Terms and Conditions.

• Review your personal information and click Save.

Scheduling a Biometric Screening


• Once you have completed logging in, you arrive at

Wellness Screenings dashboard.

• Complete the Healthyroads Coaching Authorization

questionnaire by selecting Yes or No to the

authorization question.

• Select how you would like to attend your screening: at

a Patient Service Center or at an Onsite Event for a

screening appointment.

• Follow the prompts to create an appointment. A

confirmation of your screening appointment location,

date and time will appear. You can add an Outlook

appointment to your calendar by selecting Back to

Dashboard then clicking the Download to Calendar

link.

• You will receive a confirmation email of your screening

appointment details.

Note: Your spouse/domestic partner will need to register

separately and schedule an appointment under their own

username.

Personal Health Assessment (PHA)

Personal Health Assessment

• Free confidential online PHA provides a detailed picture of your health status.

• Includes questions relating to wellness and lifestyle health factors, as well as behavioral health

factors and skills.

• Receive an instant overview of your overall health status, including a personalized action plan

identifying potential risk factors.

• All wellness resources are private and confidential:

• HIPAA policy laws prohibit the Company from receiving specific employee data. Marsh &

McLennan Companies only receives aggregated data reports that are not personally

identifiable.

HealthyMe Rate

• The HealthyMe rate allows colleagues to receive:

• Biometric screening numbers that are completed and submitted between August 1, 2017 and July

31, 2018 can be used for the 2019 HealthyMe rate. In addition, you MUST complete and submit

your annual online PHA between January 1, 2018 and July 31, 2018, in order to receive the 2019

HealthyMe rate effective January 1, 2019.**


**Healthyroads Biometric Screening Health Care Provider Forms must be submitted via fax or email

to Healthyroads no later than July 31, 2018.

and the HealthyMe rate will be automatically reflected during annual enrollment.

Absence from June 1, 2018 – July 31, 2018, will be automatically eligible to receive a

New: Connected! and Active&Fit DirectTM Programs


Connected! Program

• If you meet the plan’s eligibility criteria, the Connected! Program is available to you and your

spouse/domestic partner.

• The Connected! Program provides you with a portal for tracking your activity.

• You will have the opportunity to participate in online competitions and colleague-created

challenges.

• A common platform to consolidate data from different fitness apps and activity trackers.

Active&Fit Direct Program

• If you meet the plan’s eligibility criteria, the Active&Fit Direct Program is available to you and

your spouse/domestic partner.

• The Active&Fit DirectTM Program provides you with access to over 9,000 fitness centers across

the US at a cost of $25 per month, plus applicable state taxes. Upon enrollment, you pay $75 at

registration, plus applicable state taxes ($25 enrollment + $25 membership fee for existing

month + $25 membership fee for next month, plus applicable state taxes).

• The Active&Fit DirectTM Program is 100% employee paid.

• You and your spouse/domestic partner may enroll in the Active&Fit DirectTM Program. Go to the

Healthyroads website at healthyroads.com and click on the Active&Fit DirectTM link under the

HealthyMe Toolkit. The link will direct you to the Active&Fit DirectTM website to register and pay.

• Once you are enrolled in the Active&Fit DirectTM Program, you can print your Fitness Center

card and gain access to any of the Fitness Centers in the Active&Fit DirectTM network.
