Exhibitor Checklist Deadlines Friday, September 7, 2018 – Register exhibitor representatives on-line

Each exhibitor may bring up to two (2) representatives to be present at your booth, and they must be registered by this date. Click here to register your representatives. Additional exhibitor representatives are $75.00 each and should also be registered through the above link (by completing the form a 2nd time). You will be invoiced for the additional fees.

Friday, September 14, 2018 – Tote Bag Gift Item approvals

All tote bag gift items must be approved prior to the Conference. To receive approval, please upload a .pdf (or other file type) of the item to tote bag gift item approvals. You will be notified within 2 business days of the approval status. Please review the Tote Bag Gift Item Guidelines on page 5 prior to selecting your gift item. Plan to ship 450 of the item. Please note that including a gift item in tote bags is available only to: • Conference Sponsors • Exhibitors who committed to exhibit prior to August 10, 2018 • Exhibitors who committed to exhibit after August 10, 2018, and have paid to include a tote bag item.

Between Monday, September 24 - Wednesday, September 26, 2018 – All shipments to arrive at the hotel & Credit

Card Authorization Form sent to FedEx Office at the Loews for Tote Bag Gift Items • Shipments that arrive prior to Monday, September 24, will incur additional storage fees. Costs, which are payable

by the exhibitor/tote bag gift item provider, are included in the guidelines on page 8 of this kit. • All exhibitor booth material shipments should arrive at the Loews Philadelphia Hotel between these dates. Please

use the RED shipping labels (print in color) located on page 12 for all exhibitor booth materials. These labels should be included on each package in addition to your FedEx/UPS shipping labels. Please read the shipping information on pages 5, 7 & 8 in detail to understand the shipping process and costs. All shipments will incur handling fees from the hotel based on weight.

• All tote bag gift items to arrive at the Loews Philadelphia Hotel between these dates. Please use the BLUE shipping labels (print in color) for all tote bag gift items. These labels are located on page 13. Tote bag gift items must arrive at the hotel by Wednesday, September 26, to be included in the bags. Note that the costs for handling fees at the hotel and additional storage fees for tote bag gift items are payable by the tote bag gift item provider. You must complete and return the FedEx Office Credit Card Authorization Form to FedEx Office by Wednesday, 9/26, so that your tote bag gift items can be released to LBBC. See page 5 for more information.

Exhibitor booth numbers will be sent via email by 5:00 p.m. EDT. All exhibit space is on the 2nd floor of the Loews Philadelphia Hotel in Millennium Hall. Exhibitors are welcome to breakfast and a boxed lunch will also be provided.

Friday, September 28 – Saturday, September 29, 2018 – Provide FedEx Office Credit Card Authorization Form on-site; Pick-up Exhibitor Materials If you would like to complete the FedEx Office Credit Card Authorization when you arrive at the hotel (rather than submitting in advance), FedEx Office at the Loews will be open until 6:30 p.m. on Friday, 9/28. They will re-open on Saturday, 9/29 at 6:00 a.m. You may also choose to pick-up your shipped exhibitor materials; handling fees are slightly less for packages that you pick-up.

Saturday, September 29, 2018 – Set-up & Exhibitor Showcase

Set-up and registration begins at 6:30 a.m. in Millennium Hall. The Showcase opens at 8:00 a.m. If you would like your exhibitor materials delivered to your booth by 7:00 a.m., submit the Credit Card Authorization Form in advance or go to FedEx Office at the Loews to complete it. Items will not be delivered to your booth without a completed Credit Card Authorization Form on file. Handling fees are included on page 8.

Tote Bag Gift Item Guidelines Our goal is for attendees to receive fun, useful and informative items in the attendee tote bags. If you qualify to provide an item, please consider size and weight in selecting your item. As brochures can be heavy and costly to ship and for attendees to carry, you may consider providing a small gift item that includes your website address for attendees to reference. For the convenience of the attendees, we suggest avoiding any items that may be difficult for air travel. Gift items may include your branding, and we request that you provide 450 of the item. Standard Booth A standard exhibit booth includes a 6’ x 30” table with skirting and a table cloth, 2 chairs and one small wastebasket. Receiving, Handling & Fees All inbound and outbound shipments are handled through the FedEx Office facility located on-site at the Loews Philadelphia. We encourage you to contact FedEx Office at the Loews at (215) 733-0313 or [email protected] in advance of the Conference to answer any questions you may have regarding shipping, receiving, handling or fees after reviewing the information provided. Kyle Weinstein, Senior Store Manager, or another associate, will be happy to assist you. Please carefully follow the package shipping label instructions provided on page 7. In addition to the main shipping label, each package must also be labeled with either the RED shipping label for exhibitor materials or the BLUE shipping label for tote bag gift items. Items that are not labeled properly may be difficult to locate and may not be accessible when needed. All shipments to the hotel will incur handling fees according the fee schedule provided on page 8. You will need to complete the FedEx Office Credit Card Authorization Form either in advance (for all tote bag gift item providers) or when you arrive at the Loews to pay your receiving and handling fees. To use your FedEx Account to pay your fees, include your FedEx account number in the credit card number space. ALL FEES DUE MUST BE PAID TO RECEIVE YOUR PACKAGES. LBBC will not be responsible for receiving and handling fees for any tote bag gift items or exhibitor materials. Tote bag gift item shipments will not be released to LBBC if the provider has not submitted a completed Credit Card Authorization in advance. Additionally, shipments that arrive prior to Monday, September 24, will incur storage fees as noted in the FedEx Office information on page 8. Please check with your shipping carrier to clarify package arrival information. FedEx Office at the Loews will be open on Friday, September 28, from 7:30 a.m. – 6:30 p.m. and on Saturday, September 29, from 6:00 a.m. – 2:00 p.m. FedEx Office at the Loews is a full-service facility that can provide assistance with printing needs as well. Carry-in Exhibitor Materials Exhibitors may carry-in their own exhibitor booth materials through the parking garage entrance on 12th St. Once you enter the hotel through the glass doors from the parking area, turn left and you will be at the elevator bank. Take the elevator to the 2nd floor for Millennium Hall, the exhibit area.

Booth Services - Audiovisual, Internet & Power If you require booth services including audiovisual equipment (such as a monitor), internet and/or power, please complete and return the PSAV Audiovisual Exhibitor Services form on pages 10-11. Please note that your booth number will not be available until the end of the day on Wednesday, September 26th. We will provide booth numbers to the hotel when available so that they may coordinate the services ordered by individual exhibitors. **If you have specific questions regarding audiovisual equipment, internet or power, please contact Tom Magette with PSAV, at [email protected] or 215 231-7348.** Exhibitor Passport At the Conference, attendees will receive a card which lists each exhibitor in an individual box. Attendees will visit your booth and ask you to initial your organization’s box. You will receive a sharpie marker at your booth to initial the cards. Attendees can enter their completed cards in a prize drawing. Return Shipping from the Conference If you will need to ship exhibitor booth materials back to your organization/company after the conference, please bring your packing supplies and shipping labels and make sure that you have completed the FedEx Credit Card Authorization Form. All box(es) to be shipped out will need to be packed, taped and labeled by 5:00 p.m. on Saturday, September 29, 2018 and left in your exhibit area. Additional handling charges will apply based on the fee schedule on page 8 for “Package Pickup or Delivery by FedEx Office”. The packages will be shipped out to your company/organization on Monday, October 1, 2018. Parking The Loews Philadelphia Hotel offers valet parking only (the entrance is on S. 12th St., about a half a block below Market St.) directly behind the hotel. Please be sure to identify yourself as a Living Beyond Breast Cancer conference attendee to receive the discounted rate of $44.10 per night for overnight guests. All guest and event packages being shipped to the hotel must follow the address label standards (illustrated below) to prevent package routing delays. Please do not ship any items to the attention of the Hospitality Manager or Catering & Conference Manager, unless the items are specifically for their use (i.e., hotel specifications, rooming lists, signed documents); this includes any room drops or deliveries to any other area of Loews Philadelphia.

Shipments are held for a limited number of days. If a package has not been picked up and no contact information is provided, the package will be returned to the sender, who will be responsible for all additional shipping fees. For more information on package retention, the Return to Sender process, or to schedule package deliveries, please contact the FedEx Office Business Center at 215.733.0313. Package deliveries should only be scheduled after the recipient has checked into the hotel.

Meeting organizers and participants are encouraged to contact FedEx Office in advance of shipping their items to Loews Philadel-phia with any specific questions. If you have any special needs such as refrigeration requirements, after hours delivery requests or changes to your meeting dates or rooms, please work directly with your Event Services Manager who will communicate these needs to FedEx Office in advance of your event.

In most cases, FedEx Office will complete delivery or pickup of packages within the conference and meeting rooms, lobby area and guest suites of Loews Philadelphia, but please check with the business center for specific delivery limitations that may exist. In cas-es where a drayage company or decorator is used, FedEx Office team members will release any drayage directly to the decorator if they are onsite when the shipments arrive. If any drayage or parcels require overnight storage, FedEx Office will request handling fees be collected from the decorator. If you are using a drayage company or decorator for exhibitor packages, these packages must be shipped directly to the drayage company or decorator specified address. Please note that FedEx Office team members cannot lend out any moving equipment to a guest, which includes pallet jacks, dollies, and flatbed carts.

In most cases, FedEx Office will complete delivery or pickup of packages to guest suites at Loews Philadelphia, but please check with the business center for specific delivery limitations that may exist. FedEx Office is not authorized to leave packages in guest suites that are not occupied. A guest with authorization to sign for the delivery and approve any charges for handling and delivery fees must be present in the suite.

PACKAGE HANDLING AND STORAGE FEES

Packages will be available for pickup inside the FedEx Office business center (receiving fee will apply). Pallets, crates, display cases and other heavier items may be scheduled for delivery by contacting our staff at the number located on the previous page (delivery fee will apply). Package deliveries should only be scheduled after the recipient has checked into the hotel. In order to maintain the proper chain of custody, FedEx Office requires the package recipient’s signature before a package can be released from FedEx Office. Release signatures are captured at the time of package pickup or package delivery to the recipient.

All outbound packages must have a completed carrier airbill affixed to each package. Packaging supplies (boxes, tapes, and etc.) are available for purchase within the FedEx Office business center. FedEx Office offers pack and ship services in the business center; while packaging supplies are also available for purchase. FedEx Express® shipping boxes and airbill forms are available and complimentary. Outbound packages to be picked up by a third party courier should be coordinated in advance with a FedEx Office team member. Outbound handling fees will be applied to all packages, regardless of carrier, in addition to shipping/transportation fees.

Receiving, delivery and storage charges are payable at the time of delivery. Recipient may be required to present government-issued photo identification and sign for delivery. Shipper must comply with all applicable local, state and federal laws, including those governing packing, marking, labeling and shipping. OBTAIN FIRE, CASUALTY AND ALL OTHER INSURANCE ON PACKAGE CONTENTS PRIOR TO SHIPPING. Neither the Hotel nor FedEx Office and Print Services, Inc. provide such insurance. Neither the Hotel nor FedEx Office and Print Services, Inc. nor the employees, agents or contractors of either firm will be liable for any damages, whether direct or indirect damages, relating to or arising out of any loss or damage to any package or its contents, unless a package is lost after receipt at the Hotel, in which case such liability shall be limited to the lesser of $100 or the liability of the carrier indicated above. By sending your package to the Hotel, you agree to be bound by any additional terms and conditions that the Hotel or FedEx Office and Print Services, Inc. may establish from time to time for receiving and delivering of packages.

A one-time package storage fee will apply to each package received and stored for more than five (5) calendar days. Items measuring over 6.5 feet in size are considered oversize and will be assessed an additional oversize fee if stored for more than five (5) calendar days.

* For inbound/outbound pallets or crates, the receiving and delivery charges are consolidated into asingle fee of $0.75 / lb. ($150.00 Minimum), which is applied to each pallet/crate handled. A labor fee of $70.00 per hour will apply for breaking down pallets, building pallets, or excessive package handling/moving due to a customer’s request. Instructions It is essential that we protect the security of our customer’s credit card data and personal information. Cancellations received on the day of scheduled delivery or “no shows” are subject to the full amount of the order, including installation, drayage and tax.
