MEDICAL AND DENTAL COVERAGE
Coverage under your CCC medical plans remains the same. There is no action that you need to take if you would like to keep your coverage in place. BCBS of Illinois does offer a variety of services available to you as a member that can assist you in seeking services and promote a healthy life style:
• 24/7 Nurseline at 800-299-0274 is available for assistance in deciding where to go for care.
• The Provider Finder at bcbsil.com can help assist in finding a doctor or hospital. The Customer Service number on your ID card is available.
• Blue Access Mobile (available on iOS or Android) and BlueAccess for Members provides access to claims, coverage and deductible information. It can also assist in locating services.
You have access to the Well onTarget Wellness portal. It provides wide range of wellness information available at your fingertips:
• Health assessments
• Self-directed courses
• Health trackers and calculators
• Health information on a variety of topics
For the month of November, BCBS will be offering discounted gym membership through their Wellness onTarget Fitness Program. Log into BlueAccess for members (www.bcbsil.com), click on “Fitness Program” in the Quick Links and use Coupon Code: CCCNOV to receive a waiver of the $25 enrollment fee and discounted membership fees for over 9,500 fitness locations and 35,000 health and well-being providers.

VISION COVERAGE
Your VSP vision coverage for 2018 remains the same, but. VSP offers several discounts that you may want to explore:
• The VSP Premier Program offers a wide selection of featured frame brands, bonus offers, and more. To locate a Premier Program location, visit http://www.vsp.com.
• Eyeconic is an easy-to-use, in-network, eyewear option that allows you to shop online using your VSP benefits. You are also able to use your FSA benefit to cover expenses. For additional information visit http://www.eyeconic.com.
LIFE, DISABILITY AND CRITICAL ILLNESS PLANS
City Colleges has partnered with Prudential to provide disability and critical illness benefits which help safegard you and your family in the event of a long-term disability or illness. You may access a number of videos and tools to determine which benefits may make sense for you in 2018. Visit www.prudential.com/citycollegeschicago for additional information.
Note: Increases to coverage may require that you complete the Evidence of Insurability process. Coverage may be placed in a pending status until approval is received.
FLEXIBLE SPENDING ACCOUNTS
Health Care Flexible Spending Accounts allow employees to save on a pre-tax basis for health care expenses that your medical plan does not cover up to an annual limit of $2,650. Dependent Care FSA accounts allow employees to save on a pre-tax basis for child care or elder care expenses incurred in order to maintain employment up to an annual limit of $5,000. Enrollment is not continuous – you must re-enroll each year. As a reminder, BenefitExpress is our new FSA administrator. Any expenses for 2017 must be submitted by March 15, 2018 for expenses incurred by December, 31, 2017. Remember, FSA plans are governed by the IRS. Funds must be used within the calendar year or they will be forfeited.
EMPLOYEE ASSISTANCE PROGRAM (EAP)
City Colleges of Chicago provides an Employee Assistance Program (EAP) to all active employees and the family members residing in the home.
In addition to benefits provided through health insurance coverage, EAP services include consultation and support for everyday challenges of life that may affect your health, family life, or desire to excel at work.
To utilize EAP services, call (800) 311-4327 24 hours a day, 7 days a week. Care can also be accessed online via: www.guidanceresources.com
To register, click on the 'REGISTER' tab and use the following information:
CCC's 'Organization Web ID' - MGR311 (not case sensitive)
CCC's 'Company Name' (first 5 characters)- Board (not case sensitive)
Click 'Submit' - Enter your personal information to register.

MAKING CHANGES IN HR SELF-SERVICE
To enroll in benefits, log onto HR Self Service.
ü Log into the portal using your CCC username and password.
ü Once you’re logged in, click on “Self-Service.”
ü Under the “Benefits” tab choose the “Benefits Enrollment” link to begin making elections.
ü Follow all prompts and navigate through all plans. Once all elections have been made, click the “Submit” to complete the enrollment process.
ü You may view all elections made by clicking on the “Benefits Summary” link located under the main Benefits section.
DEPENDENT VERIFICATION
City Colleges will require that employees adding dependents for the first time provide proof of their relationship beginning with Open Enrollment 2018. As a reminder, eligible dependents includes:
• Legal spouse, civil union spouse, or domestic partner • Eligible child(ren) including step, legally-adopted and children under legal guardianship until
age 26 (or until age 30 for military dependents). • Physically or mentally handicapped children (regardless of age), who have been added prior
to age 26
The following are considered ineligible dependents:
• Employee’s parents • Divorced spouse • Common law spouse • Children who are not under employee’s legal guardianship
Examples of acceptable documentation can include:
• Marriage Certificate • Civil Union Certificate • Birth Certificate • Court Order
ARE YOUR BENEFICIARIES UP-TO-DATE?
Beneficiaries for life insurance are kept in Employee Self-Service. For your basic and voluntary employee life coverage, you will want to ensure that your beneficiaries are current. SURS as well as our 403(b)/457(b) vendors keep separate beneficiaries on record. Make sure that your records are updated with them as well.

GET FAMILIAR WITH YOUR RETIREMENT PLAN OPTIONS
If you are a benefited employee, 8% of your pre-tax pay is contributed to the SURS retirement plan. Upon your hire, you were given the option to choose your plan design option and if you elected the Self-Managed Plan, the vendor which would receive your contributions. If you are unsure of your election, reach out to SURS via the web at www.SURS.org or by calling 800-275-7877. There are some additional items which you may want to discuss to ensure that you’re ready for retirement:
• SURS has several videos available related to plan options, reciprocity with other agencies as well as retirement planning which may help you navigate your options.
• As your life changes – family changes, retirement, termination, be sure to make sure SURS is kept in the loop. Their life events page will guide you on making any necessary changes.
Webinars and seminars (in Naperville) are offered from time-to-time. Check the current schedule to see when seminars are available.
BENEFIT FAIRS AND OFFICE HOURS
Should you need assistance during Open Enrollment, there will be several opportunities to have your questions answered:
Don’t forget your flu shot! Cold and Flu season is upon us. You should get a flu shot as soon as it's available as flu season can begin as early as October. The vaccine is covered under the plan and is recommended by the Centers for Disease Control and Prevention for all people ages 6 months and older. Check with your doctor to find out if the flu vaccine is recommended. Many times it can be administered within their office. BCBS PPO plan members may also visit any CVS Pharmacy location. Just bring your prescription card and valid photo ID.
