40 th Anniversary Celebration Agrace celebrated its 40th anniversary of being founded by a group of volunteers who believed in the mission of quality, compassionate end-of-life care. An event to celebrate this milestone was held in August with more than 100 staff, donors, volunteers and community members in aendance. Non-Medical Service Helps Seniors “Age at Home” In September, Agrace began offering a new non-medical home care service to help people that are struggling to remain safe and independent at home as they grow older. Age at Home by Agrace offers a menu of services designed to help Dane County seniors remain living in their own homes as they age: personal care, housekeeping, meal preparaon, laundry, medicaon reminders, errands, transportaon and much more. With specialty services, such as overnight and live-in care, clients can personalize support to their needs. Proceeds from this private-pay service are reinvested into programs and services that support Agrace’s nonprofit mission. Volunteer Program Focuses on Supporting Veteran Patients When a hospice paent is also a veteran, their military service can affect their end-of- life goals. In order to best serve our veteran paents, Agrace developed a detailed, new volunteer training process for Vet-to-Vet visits and are seeking new volunteers with military service. Agrace’s Vet-to-Vet visits support veteran paents by giving them me with a volunteer who can relate to their service and is a willing listener. These visits oſten build trust and give veterans more personalized care. Agrace Receives Award for Diversity in the Workplace Agrace received the 2018 YWCA Rock County Dorothy Height Award for Diversity in the Workplace. The honor reflects our broad commitment to diversity and inclusion, including innovave interpreter services, Advancing Inclusion and Equity Commiee, and recruitment and retenon pracces. Residential Hospice Care Becomes Available in Janesville Once dedicated to short-term, acute care, the Agrace Center for Hospice & Palliave Care in Janesville began offering a new opon: residenal hospice care. When patients who need Agrace’s palliative care or hospice services are not covered by Medicare, Medicaid or insurance and cannot otherwise afford their care, the Agrace Foundation is here to help. Thanks to community donations to the Foundation’s Care for All program, Agrace can offer free or discounted care to patients who demonstrate financial need throughout our entire service area, including Dane, Sauk and Rock counties in Wisconsin as well as counties in northern Illinois. Because of this program, we’ve never turned away a patient for inability to pay. Those who donate to the Agrace Foundation don’t expect to get anything in return—and yet there is a return on their investment. In 2018, Agrace provided a community benefit of $3.9 million through support of vital Agrace services and programs like these:

Our Grief & Bereavement Clients• In 2018, Agrace provided grief services to 2,367

individuals—both children and adults. Although providing grief support is required by state and federal regulations, grief support services are not covered by insurance, Medicare or Medicaid reimbursement. Agrace provides these services because the organization believes they are critical to helping families return to functional living after their loss.

• In 2017, Agrace opened the Agrace Grief Support Center in Fitchburg, Wis., to host grief support groups and one-on-one support. During 2018, grief support services were provided to more than 500 people at the Center.

• Agrace’s grief support groups are open to everyone in the community who has experienced the death of a loved one, even if the person who died did not receive hospice care.

Age at Home Senior Care:In September 2018, Age at Home by Agrace was created, offering senior care services throughout Dane county. This service is private-pay, with all proceeds reinvested in Agrace’s nonprofit mission. In 2018, Age at Home caregivers served 107 clients and provided more than 4,000 hours of in-home care.

Grief Support GroupsAgrace’s grief support groups are open to everyone in the community who has experienced the death of a loved one, even if the person has no prior experience with Agrace services. VolunteersIn 2018, 922 specially trained volunteers provided 83,000 hours of service to Agrace in a variety of roles, including direct service to patients and families in their homes, and in the Agrace inpatient units. Volunteers also supported agency efforts by volunteering in many capacities including in our offices, kitchen, gardens, at our special events, community presentations, the Agrace thrift stores and the Agrace Grief Support Center.

EducationIn 2018, more than 150 students from the areas of nursing, social work, child life, clinical counseling, pharmacy and medical school, as well as residents and fellows, participated in clinical rotations at Agrace. Since the Agrace Rotation Program began in 1995, approximately 2,300 students, interns, residents, fellows and working health care professionals from the University of Wisconsin, Blackhawk Technical College, Edgewood College, Madison College, Herzing College, Aurora University and other institutions have completed an educational experience at Agrace. Agrace receives no reimbursement for these rotations, exception for a stipend from the UW School of Pharmacy. Agrace receives no reimbursement for these rotations with the exception of an annual stipend from the UW–Madison School of Pharmacy.
