Key terms

Copay A flat amount you pay for covered services like doctor visits.

Deductible The amount you pay each year before your plan starts to pay for care.

Your percentage of the costs (coinsurance) After meeting your deductible, this is your percentage of the costs you pay for care.

Out-of-pocket maximum The most you have to pay out of pocket each plan year for health care services. <Copays and deductibles are part of your out-of-pocket maximum.>

How the KeyCare plan works

Copay You pay a set amount for doctors’ visits and services.

Annual deductible The amount you pay each year before the plan starts to pay for care.

Your percentage of the costs You pay a percentage of the costs for most other services, such as outpatient surgery, hospital care or tests.

Out-of-pocket maximum Once you pay the annual maximum for your percentage of the costs, the plan pays 100% of other covered expenses for that year.

What is a Health Savings Account?

•A Health Savings Account (HSA) is an account created for individuals who are covered under high-deductible health plans (HDHPs) to save for current health expenses and future qualified medical and retiree health expenses that HDHPs do not cover.

•Contributions are made into the account by the individual and/or the individual's employer and are limited to a maximum amount each year.

•The account is owned by the employee and is completely portable from employer to employer.

•There are limits on the amount that can be contributed to an HSA each year. For single coverage, the amount is $3,400 per year in 2017. For family coverage the amount is $6,750.

•Note that all IRS regulations related to HSA accounts will be the responsibility of each employee and not ACPS.

How the HSA plan works

Health savings account (HSA) • You can put money into an

HSA, pre-tax, to help pay for covered medical expenses, like office visits, lab work, tests and prescription drugs.

• HSA funds are accessed through the plan-provided debit card or online bill pay.

• Unused HSA funds roll over from year to year.

• This plan puts more decisions in your hands.

Annual deductible

• You pay a yearly deductible before the plan begins to pay a percentage of your covered expenses.

• You can use the money in an HSA to help meet your deductible.

Major medical benefits (your percentage of the costs) • After you meet your annual deductible,

you pay a percentage of the costs for covered care.

• If you still have money in the HSA after you've met your annual deductible, you can use the funds to pay your percentage of the costs.

• Once you pay the annual maximum for your percentage of the costs, the plan pays 100% of other covered expenses.

Key features Things to consider

The HSA plan

• If you visit a doctor who isn’t in the plan, you may be asked to pay for services at the time of your visit and you may have to file a claim.

• If you don't put money in the HSA, you’ll have to pay your deductible and your percentage of the costs during the year.

• Funds can be used during the current year or saved for future expenses.

• The HSA contribution is not taxed, as long as you spend your funds on covered services.

• You get to keep the money you put in the HSA if you change plans or leave the company.

• ACPS will put $600 per year ($50 per month) and cover the monthly fee for your HSA account.
