>70% participation excluding valid waivers(Except 11/15/15 – 12/15/15)
>75% Employees must reside in Meritus service area
>Employer must contribute 50% of premium cost for all plans
>Meritus offers all FFM-SHOP group plans OFF Market>Meritus will accept Husband/Wife Groups as long as they meet the requirements found on page 78 of broker handbook >Meritus offers up to 3 plan options on any size group>Meritus offers composite or age rates on any size group

To quote a group, click on the Group tab and select New Quote. Fill in the information and click Next.

This is where you will enter the employee census information or import the census you created from the Meritus census form. If you want to import the census, click Import from the drop down box from the Add Employee box.

Choose your census file to import or if you need a blank census file, click on the green “sample” link to get the census spreadsheet. Once you have chosen a file, click Import.

Enter the employer contribution amount. You can switch from a Percentage to a Dollar amount by clicking the orange button. Once complete, click Next.

Review the proposal and edit any information if necessary.

Proposal review continued. At this point, if the information looks correct, either Save As PDF, Email to Employer or click Apply. If you click Apply, make sure that you have selected no more than three (3) plans that the
employer will offer their employees.

To email the proposal to your client, complete the missing fields. You may edit the Subject and Message fields to fit your needs. If you would like to copy yourself or another party on the proposal email, click the small up-arrow
in the bottom right corner of the Send button. Once complete, click Send.

This is the Group Application – this is the first screen you will see when you click Apply at the bottom of the Proposal Review screen. Enter the information requested, then click Next to take you to the next page.

If any information is incomplete, you will see a red box and be able to edit the information.

Once all the information is completed, select the Submit button. Meritus will review the information and either approve, ask for additional information or reject the application within 24-48 hours.
