Founded and led by a team of therapists, we know firsthand what it takes to run a successful rehabilitation department. Our clinicians treat patients in over 220 facilities across twelve states and cover the entire post-acute care continuum, which includes inpatient, outpatient, home-based care, LTAC, assisted living and independent living.
Based in Wilsonville, Oregon, we serve more than 7,000 patients each month and provide over 1 million patient visits per year. We continue to be the leading choice for contract therapy management services in the Pacific Northwest, Mountain, and Midwest regions.

INFINITY REHABIn 2016, Infinity Rehab implemented proven quality improvement models, based on the Lean approach, to succeed in improving outcomes. We are revolutionizing the delivery of therapy services through science-based research, standardizing practices, and optimizing added value for true patient-centered care.
Over the past year, we gained deeper insight into how therapy is delivered and how it impacts quality outcomes. Through localized, research-based studies, our Quality Implementation Team analyzed data that showed us two things:
1. Increasing intensity of rehabilitation yields positive benefits.2. Enhancing and simplifying patient engagement yields additional positive benefits.
Throughout 2016, several skilled nursing facilities contracting with Infinity Rehab participated in ambulation tests designed to measure individual patient walking performance and provide a means to aggregate patient outcomes data for future study.

Patients Served in 2016 Across the Continuum
Gait Speed Trials and Six Minute Walk Tests are straight forward interventions designed to improve a patient’s self-selected or comfortable walking speed. The strategy we used for both tests was to ask the patient to walk a short distance at a comfortable speed, provide the patient feedback about their time, encourage them to try to beat that time, and then repeat.

Gait Speed Trials improved the average final walking speed of participants far greater than the average final walking speed of non-participating sites. In non-participating sites, we saw an improvement in walking speed of just over three times the minimal clinically important difference (MCID), while in patients participating in Gait Speed Trials, saw an improvement in over five times the MCID.

Six Minute Walk Test
Similar gains were made in walking distance and activity tolerance as measured by the Six Minute Walk Test. For this test, the evaluation results across groups were similar to Gait Speed Trial patients and showed a greater improvement in discharge distance and change score. Participating sites had a 50% improvement in MCID over non-participating sites.
Results of the ambulation tests are significant because a higher MCID relates to greater ability for safe community ambulation and reduced risk of re-hospitalization. The ambulation trials also show when therapists increase the intensity of rehabilitation, patients experience increased cardiovascular, nervous system, and cognitive functioning. In essence, all physiological systems work together to enhance movement and thus improve outcomes.
The next step is increasing patient activation and engagement. We have developed a package of tools that research shows will improve intervention. Occupational therapists engage patients in a card sorting activity for setting specific goals. This translates into creating a goal setting sheet the patient uses to manage progress and direct that day’s therapy activities. The patient is then provided with a simple journal to track progress, which is reviewed with the occupational therapist. Implementing steps to increase patient engagement will potentially improve patient satisfaction and work toward improving quality patient outcomes.

INNOVATIVE NETWORK PARTNERSHIPHospitals and health systems are under increased accountability from state and federal agencies to produce quality patient outcomes. Based on our leadership, innovative approaches to care delivery, and our positive industry reputation, Infinity Rehab was sought out to help a prominent teaching hospital develop longitudinal pathways for patients in Medicare’s Comprehensive Care for Joint Replacement (CJR) model.
Infinity Rehab represented the post-acute care section during the planning process for developing CJR care pathways. We conducted a full literature review, interviewed principle investigators of several of the studies, and from that research, developed an evidence-based protocol for physical therapists and occupational therapists treating patients covered under Medicare’s CJR model. Clear expectations of length-of-stay and specific requirements around treatment were developed to promote consistency so both provider and patient know what to expect in this on-going partnership.
Competencies gained through this partnership helps Infinity Rehab and our network partners further reduce avoidable re-hospitalizations, empower clinicians through standardize practices, and promote healthier communities through positive patient outcomes.

Patty Scheets, PT, MHS, DPT, NCS — Director of Quality and Clinical OutcomesPatty completed her professional and post-professional education at Washington University in St. Louis. She is a board certified specialist in neurologic physical therapy and is the 2017 President-Elect of the Academy of Neurologic Physical Therapy. Patty leads initiatives related to the implementation of outcome measures, implementation of evidence-based practices or knowledge translation, and develops models of care that improve outcomes.
Patrick Hennessey, PT, MPT, NCS — Quality Implementation SpecialistPatrick worked at the Rehab Institute of Chicago (RIC) where he had a split position between clinical care and research in the Locomotor Recovery Lab under the direction of George Hornby, PT, PhD. In this role, Patrick provided clinical care and worked to standardize outcome assessments. Prior to his work at RIC, Patrick worked at Moss Rehabilitation Hospital in Philadelphia. Patrick has presented at national, regional, and local physical therapy meetings and conferences and served as a lecturer to professional-level physical therapy students.
Derek Fenwick, PT, MBA, GCS — Director of Professional DevelopmentDerek graduated with a physical therapy degree from Saint Louis University in 2003 and followed that up by earning a health care management degree from Regis University in 2008. Derek manages our Director of Rehab-in-training program, our Clinical Ladder professional advancement program, and our Student Clinical Internship Program among other exciting projects. Derek led the creation of the Infinity Rehab Leadership Academy to help us develop the next generation of leaders for our company.
The Infinity Rehab Quality Innovations Team leads evidence-based pilot studies focused on standardized practices, increased therapeutic intensity, and examines aggregate patient data for deeper insights into care delivery and outcomes.

In 2016, Infinity Rehab partnered with CEU360 giving our therapists unlimited access to over 1,000 hours of premium online continuing education courses on topics that cover the full spectrum of patient care for older adults. CEU360 assists Infinity Rehab by producing training and education content in areas such as quality and outcomes, operations management, leadership, clinical supervision, and more. This initiative propagates internal ideas of change by turning them into actionable practices.

LEADERSHIP ACADEMY Years of ideas, research, and passion about leadership within Infinity Rehab came together in the creation of the Infinity Rehab Leadership Academy. Scholars enrolled in the academy engage in on-site and distance learning designed to transform Infinity Rehab employees into successful leaders. The curriculum matches our core values with key leadership practices proven effective in navigating real world challenges.
In 2016, our second cohort of Leadership Academy scholars graduated. During the year-long academy, scholars worked on projects that influence their day-to-day work. The results of the projects were presented at the end of the curriculum, and covered topics such as:
