    Overview of A1cManager™

    A1cManager™ is for diabetes caregivers and for an elite subgroup of their patients

    with diabetes. A1cManager™ includes supporting algorithms and unique

    animation to guide Elite Users to achieve and maintain Optimal Diabetes Control

    (ODC). To do this, A1cManager™ integrates User data (blood glucose, body

    weight, prescribed medications, diet, exercise, and stress) into a virtualized

    resource to close the circle of care that targets blood glucose and A1c levels.

    Using A1cManager™ Elite Users who do achieve and maintain ODC can expect to

    prevent, slow, or in some cases reverse the eye, nerve, heart, and kidney damage

    of long standing, poorly controlled diabetes.

    Focus on Health

    Remember, healthy people eat 3 well balanced meals at about the same times each day.

    With diabetes, a healthy Breakfast, Lunch, and Dinner are best taken between 7-9AM, 12-2PM, and 5-7PM.

    Portion control is crucial and Snacks are usually not necessary.

    It is generally best to take diabetes medications 5-15 minutes before eating.

    In this way, you do SMBG only 14-16+ times week

    You use A1cManager™ to create a Meal Period Profile (MPP)

    What is a Meal Period Profile?

    A Meal Period Profile (MPP) is a snapshot of your

    current diabetes status. It is created from your

    last week’s structured SMBGs.

    The white circles and surrounding shaded areas are the averages and the scatter of your blood glucose readings. Reading from Left to Right:

    1. overnight (gray rectangle to the left),

    2. pre Breakfast

    3. post Breakfast’

    4. pre Lunch,

    5. post Lunch,

    6. pre Dinner,

    7. post Dinner,

    8. pre Sleep.

    A1cManager™ will support you to first achieve & then

    maintain optimal diabetes control.

    What You do at Home

    At least two times on different days and during the week before returning to the support group or clinic, do the following 7 steps:

    1. On first rising, measure your Body Weight (BW) and your Blood Glucose (BG).

    2. Before breakfast, take your medications if any. Eat Breakfast. Then, looking for the peak, measure your BG again 1 to 1½ hour after the first bite of breakfast. Be precise: use a timer.

    3. Not necessarily on the same day, take the medications (if any) and make the BG measurements before and after Lunch.

    4. Again, not necessarily on the same day, take the medications (if any) and make the BG measurements before and after Dinner.

    5. Again, not necessarily on the same day, take the medications (if any) and make the BG measurements on two typical days before going to Sleep.

    6. Measure your BG at least once overnight, especially if you fear night time hypoglycemia.

    7. Record the results on the MPP Data Collection sheet.

    Though more may be better, you should have at least 12 BGs counting before and after meals and at least 2 before sleep, for a total of just 14 SMBGs in the week before your next support group or clinic visit.

    You start an MPP at home.

    Step 1. Starting an MPP at Home

    Measure your BG.

    Be Precise. Your Data are precious! Wash your hands! Warm your fingers!

    Use a Meal Period MPP Data Collection Sheet as a

    Notebook or Diary.

    Always record Date, BW, time, BG, medications taken,

    and other data as shown below.

    You need a minimum of 14 SMBGs / Week:

    Before Breakfast, on 2 days

    After Breakfast (at ~1 to 1½ hr) on 2 days

    Same for Lunch and Dinner

    Before Sleep on 2 days

    Optionally, Over night (~3AM) on 1-2 days

    Note unusual BGs, diet carbs(CHO), exercise (EX), Stress, etc.

    Record additional readings. Use both sides of this MPP form

    Especially note hypoglycemia.

    Collect & record data in the week before returning

    to your support group or doctor’s clinic.

    Step 2. Preparing to Access A1cManager™

    You will need to,

    1) read the

    i. Agreement to Share Outcome Data,

    2) carefully complete the

    i. New User Registration Demographics, and

    3) carefully fill in the table to

    i. List your Diabetes Medications

    On the next page over, initial, date and keep the extra User’s

    Copy for your records.

    You will need to register in the Cloud.

    Registration provides credentials that allow both

    o you to access your own data, and

    o your health care professionals to access your data.

    Registration places an Icon on your Desktop. The Icon provides

    a link to the Cloud

    Step 3. Logging in to A1cManager™

    You will need to Double Click the BCMCtutor.rdp or the

    A1cManager.rdp Icon on your Desktop to Login and access

    A1cManager™ on the cloud.

    I, _________________________ perform home glucose management

    according to instructions I have received from my health care professionals. To guide me in

    so doing I use the BCMC A1cManager™ Service on the Cloud.. It helps me understand my

    current status and my drug prescription as it relates to my diabetes. My status is indicated

    both by the home measurements I do (body weight & capillary blood glucose) and the results

    from the medical procedures or laboratory measurements done as part of my medical care.

    The BCMC Service is designed to guide me to achieve optimal diabetes control and help my

    caregivers prescribe the best medications and doses for me. My active participation is

    required. I understand that correcting any errors in my treatment and making any adjustments

    in my prescriptions are best done in consultation with my health care professionals who

    themselves use A1cManager™and support my active participation in the global A1cManager

    Initiative.

    Accordingly, I wish to share my diabetes and related metabolic outcomes with the Support

    Group (Diabetes, Optimal Control) and the Company (BCMC, Inc) that provides the Cloud

    Services. There may be disclosure of confidential information. Typically, the information

    shared will be my glucose values, my diabetes treatment, and certain of my life-style events,

    including diet and exercise behaviors. My personal information, including my name and

    demographics will not be disclosed. Even when results will be used for publication, my

    demographics and specific results will be disassociated so that they cannot identify me. My

    outcomes may be used in the teaching and instruction of tutors, including other health

    professionals.

    I have the right to refuse to sign this Agreement and it will not affect my ability to participate in

    the Support Group Sessions here in The Villages.

    Click Start.

    Elite User Authentication requires

    1. Your PIN is your home phone number, eg, 416 233 3996

    2. Your Password will be provided.

    Step 7. Entering your SMBG on the Cloud

    Click My SMBG Your Data set includes:

    1. Date

    2. Body Weight

    3. Time (hhmm) & SMBG (Self-Monitored Blood Glucose)

    4. Meds – Rx/Taken (Rx stands for Prescribed). You insert what was taken

    5. Comments about Life-style carbs(CHO), exercise(EX), Stress

    When a new screen appears, be sure to check the Date that is inserted automatically.

    Type to correct, if necessary

    Key in the date like this: month-day, eg, 4.16

    Press Enter 4-16-2014 Note the computer inserts the Year.

    Cursor goes to weight.

    Key in your weight.

    Press Enter The next screen will appear. See below.

    Key in the time of your meal. Use 8 or 8p; or 915 or 915p. It will be converted to military time: 0800 or 2000; or 0915 0r 2115

    Press Enter Cursor will go to SMBG

    Key in your before Breakfast SMBG. Press Enter Cursor will move to the medication field

    Key in the med doses you took before the meal. Press Enter Cursor will go to the after Breakfast time field

    Key in the 1 to 1½ hour after Breakfast time reading.

    Press Enter Cursor will go to the after breakfast BG field.

    Key in your after Breakfast BG.

    Press Enter The cursor will go to the before lunch time field.

    Repeat the process.

    When you have entered all your readings for the day,

    click Done to save the data. A new blank screen will appear.

    If you click Esc instead the data will not be saved.

    If you have no other days to report just click Esc.

    Some computers hide the .bat extension.
