Supporting TRAC


Providing a Robust One-on-One Experience • Sessions are scheduled around member preference and availability

– Average length of sessions are 15 to 20 minutes

– Frequency—initially weekly, then decrease in frequency depending on acuity

– Most members average 4 to 8 sessions depending on opportunity level

• Early sessions focus on cultivating a relationship and meeting individuals where they are – Establish rapport and assess readiness to change

– Explain program and obtain background—including barriers, personal dynamics

– Collaboratively identify personal goals and develop a strategy to achieve those goals

• Later sessions are tailored to each individual’s “stage of change” – For example, if someone is ready to quit smoking within the next month,

the focus is on preparation. For someone who is still contemplating quitting, the focus is on motivation

• Set a personal “quit day”

• Discuss free nicotine replacement therapy (8 weeks)

• Fagerstrom score to determines nicotine addiction level
