At Almost Home Kids our Mission is to provide transitional care in a home-like setting to children with complicated health needs, training for their families and respite care.

The Almost Home Kids model is present in any place there is a need without regard to geographical boundaries. All key community and policy stakeholders are fully committed to the AHK model and recognize it as the premiere standard of care.
2015 was an exciting year for Almost Home Kids, our community engagement in Chicago increased by over 70% as new volunteers signed up for recreational opportunities with our children, corporate groups volunteered for story hours, supporters brought in Wish List items and a myriad of other thoughtful projects helped to make our house a home. In Naperville the Margaret M. & Thomas M. Curran Healing Garden became a reality as the garden was transformed into a beautiful serene space that is fully accessible and will be enjoyed by young and old alike. Our international program welcomed children from as far afield as Kuwait and Russia, and our clinical outreach expanded into many more local and national healthcare systems.
This year also brought Almost Home Kids closer to our Vision statement (above) established in 2010. Our plans are moving forward to expand and meet the needs of more children with medical complexities and their families, within and outside of Illinois. Our partnerships with children’s healthcare centers and state systems help us provide the best level of care for all children with medical complexities. Nationwide, the medical community has embraced our model of care and our resolve for the future is strong.
Your support allows us to realize our mission and directly impacts our community’s most vulnerable children and families. On behalf of the more than 1,400 children we have served, we thank you for your dedication. Your willingness to donate your time, expertise and funds really does make an enormous impact, and we are extremely grateful to have so many wonderful supporters.

The Almost Home Kids Model AHK is licensed by the Illinois Department of Health, under the Alternative Healthcare Delivery Act as a Children’s Community Health Care Center. It is the only community-based facility in Illinois dedicated exclusively to the care of children with complex medical conditions. The organization is poised to share its expertise with other institutions therefore increasing the capabilities and capacity of the region to care for children with complicated health needs.
How We Do It
Transitional Care — A bridge from hospital to home
AHK provides a 120-day Transitional Care Program for children who are ready to be discharged from a hospital but for whom preparations at home need to be made. In a comforting environment, children receive 24-hour medical and nursing support from skilled pediatric nurses. AHK trains families on how to both care for their child with medical complexities and how to prepare their home.
Respite Care
The Respite Care Program at AHK offers children a comfortable place to stay with access to medical and nursing support while their parents/caregivers tend to personal or urgent family issues. The Respite Transportation Program provides roundtrip transportation for children from their home to an AHK facility.
Community Support
In addition to training and care, AHK provides community resources and activities that become a support system the family can depend on for years to come.

Program Updates: Keith Veselik M.D. Medical Education Program
AHK’s goal is to improve the quality of life for its children and families, which means, that we offer excellent clinical care, play a role in better training the medical community in knowledge, skills and perceptions that will allow them to competently serve this important and vulnerable population. The Keith Veselik, M.D. Medical Education Program is a prime example. This program improves the lives of high-risk children both now and in the future by training current and future physicians about the medical complexities and social realities of this unique population in the community setting. Thus far this year, residents from ten teaching hospitals have been on-site gaining critical knowledge at both AHK homes.

Children staying at AHK are considered medically complex due to a wide variety of medical diagnoses including prematurity, chromosomal defects, cerebral palsy, respiratory problems that require a ventilator, or victims of physical trauma. They all are at risk for physical disabilities and delay in their overall development due to their medical diagnosis, history of prematurity or prolonged hospitalizations. At AHK one of its goals is to improve the quality of life for its children and families. The Physical Therapy Program, launched in 2014, brings physical therapists onsite several days a week to focus on improving physical abilities and promoting mobility, function and quality of life for the children. They accomplish this through therapeutic exercise, strength building, use of assistive devices and education.

Our Financial SummaryOur fiscal year 2015 runs from September 1, 2014 to August 31, 2015. During this time AHK’s Program Expenses totaled $5.8 million, which included investments toward expansion initiatives and significant capital improvements in Naperville.
This is up 3.7% over last year and the increase is attributed to employee benefit costs. This year, we enhanced the capabilities for our clinical team and increased efficiency in care coordination by converting to an electronic medical record system called EPIC.
After receiving $3.9 million fee for service revenue from the State of Illinois, third party insurance companies and cash payments, AHK turned to philanthropy to close the gap for our medical services.
With the generosity of 1,370 donors, AHK’s fundraising contributions for the year tallied $1.1 million.
The organization remains focused on fundraising efficiency and is committed to providing critical healthcare services for children, regardless of a family’s ability to pay.

Your Support Makes an Impact■ In Fiscal Year 2015, over 2,187 gifts were received at AHK, totalling $1,155,438. These gifts play an essential
role in providing the very best care for our children and support for their families.
■ Volunteers have always been a critical part of the care we provide at AHK. This year, volunteer hours at AHK Chicago increased more than 50% compared to the previous year!
■ More than 150 individuals and organizations supported the Margaret M. & Thomas M. Curran Healing Garden, purchasing commemorative brick pavers or naming sculptures, trees and sitting walls.
In an effort to better steward the donations entrusted to us, as well as the environment, we have omitted the multiple page listing of our donors this year in the Annual Report.
