2013 Northeast Kansas Football League Registration form

The Northeast Kansas Football (NKFL) is now taking registrations for youth tackle football. Space will be limited. Preference will be given to players who have previously played in NKFL. The early deadline to register individual players is May 18, 2013. Teams must have a minimum of 11 players registered and paid for by July 15, 2013 and the entire team must be registered and paid for by Aug. 1, 2013. Absolutely no registrations will be accepted after Aug 1, 2013 unless approved by the NKFL Board. Players will be assigned to teams according to the school or district they attend whenever possible. Final Rosters will be decided by the NKFL Commissioner. All coaches will be approved by the NKFL Commissioner and NKFL Board. Most games will be played at the SportZone Sports Complex located at 3909 SW Burlingame Rd in Topeka. Additional games may be played in other communities within the area. Each team will play a minimum of seven (7) league games (weather permitting). Some teams will advance to a league Superbowl tournament or a Bowl game. League play will begin on Aug. 24, 2013. Practices with pads cannot begin until Aug.1 2013. The League maximum of players will be 25 players per team. The NKFL will provide helmets and shoulder pads, referees, and a well maintained facility. Each player will provide their own mouthpiece, shoes, jersey, football pants w/pant pads, and athletic supporter. Players will be divided into divisions by grade. Divisions will be combined when necessary. The NKFL League Commissioner, selected by the NKFL Board of Directors, will rule on all issues not clearly stated in league rules. All players must have a current athletic physical (completed in 2013), proof of 2013 grade, and a copy of their birth certificate turned into the League Office prior to the first practice. A separate check for $100 (Check or Cash only) dated December 15, 2013 must be submitted at time of equipment

check-out or registration. This check will be destroyed or returned at time equipment is returned.

League Fee: $150.00 (Includes $40.00 Fundraiser and Player Id card)

My signature acknowledges I am the parent or legal guardian of the above listed minor. I understand medical insurance is not provided with NKFL programs. I release

the NKFL from any and all liability whatsoever resulting from participation in NKFL activities. I authorize those in attendance to act according to their best judgment in

emergency situations requiring medical attention. I hereby waive and release the SportZone and NKFL, it's staff, agents, sponsors, and/or coaches from any and all

liability that may occur from accident, injury or illness sustained by my son/daughter during participation in these activities. I understand that no refunds will be applied

within two weeks of the beginning date of a program. I understand behavior resulting in removal from a program does not constitute refund criteria. I understand that

refunds, when applied, will have a $20.00 handling fee accessed. I understand that if equipment is issued in conjunction with any program, failure to return said

equipment within 2 weeks of the end of the program will result in legal action. I understand that any photographs, medals, awards trophies, etc., associated with

programs may be held for 30 days after the end on the activities at which time, if not claimed, will be disposed of. I understand returned checks will be accessed a $20

processing fee. I understand that photographs of all SportZone, NKFL, MEGA, activities and activities conducted by leased tenants will be taken and may be used for

brochures, promotions and advertising without permission. I acknowledge all information and waivers contained herein.
