A benefit to open Needhams 1st
dog park
When: October 6, 2013 (rain date October 20)
Where: The Quarry at Claxton Field
Time: Check-in begins at 1pm, Events begin at 2pm.
Fee: $20 per dog in advance, $25 the day of the show ($10 for each
additional dog per family
Show Events
The show will have many events including:
Highest jumper Dog looking most like owner
Best trick Best vocals
Best costume Waggiest tail
Best rescue dog Most adorable dog
Grand Finale Parade
Dog Show Rules
All dogs must have valid dog license with them to enter All dogs must be leashed at all times (no flexi leads) Owners must clean up after their dogs All dogs must be at least 4 months old Handlers must carry all props used in the show All dogs must have proper immunizations All entries must be signed No female dogs in season allowed on the premises All children 15 and under must be accompanied by parent

Release and Indemnity Agreement
I acknowledge that I will assume full responsibility for my safety, the safety of my children, and the safety of my pet. I
understand that I am participating in the Needham Family Dog Show at my own risk and agree to release, waive, and
hold harmless the Needham dog park committee, its officers, members, and representatives from any and all liability to
me, my family, legal representative, and/or heirs and assigns, for any and all loss, personal injury or damage and any
claim or damages resulting from on account of injury to my person or property, whether caused by the negligence of the
Releases or otherwise while participating in the Needham Family dog show. I agree in consideration of being permitted
to participate in the Needham Fun Dog Show, I consent to be photographed and release the Needham dog park
committee to use photos for promotional advertising or future events. I verity that my dog(s) has all shots,
immunizations and license.
