Many have said that 2013 was a year of unprecedented change in healthcare. Our dedication to our mission and vision has not changed. We are a faith-based charitable clinic dedicated to providing high-quality compassionate care. It is our firm belief that all lives are valuable and that every person is inherently worthy of being treated with dignity and respect. Often our patients arrive at our clinic having delayed their care for long periods of time because of their struggle to find affordable health care. Every day our patients are faced with difficult choices such as - can I take my medicine today, how do I get to the doctor, or can I afford rent and food today? It is well documented that those most economically vulnerable suffer from worsening chronic disease.

Since our inception, we have been a collaborative healing ministry. Collectively each of us doing our best each day makes a difference in the lives of our patients. We are able to restore hope when patients thought there was no hope to improve their health and well-being. Every day miracles happen if we only look for them.

A small kindness might not seem heroic. Perhaps the smile you gave today was the only smile that person received or the time you took to listen was the first step in restoring hope for better health. The contributions of our donors, volunteers and other supporters save lives every day and restore hope for a brighter future. We are extremely grateful for your support to help our neighbors in need.

Service OverviewNorth Coast Health (NCH) is a faith-based charitable clinic that provides and optimizes access to health care for the medically underserved. A vital part of Greater Cleveland’s health care safety net since its founding in 1986, NCH provides primary care, prescription assistance, specialty referrals and health education to low-income individuals and families. With the support of 120 volunteer physicians, nurses, other caregivers, NCH is a patient centered medical home; providing preventive care and chronic disease management that has been demonstrated to result in improved outcomes and reduce unnecessary costs.

In 2013, North Coast Health:• Served 2,425 individuals, for a

7% increase compared to 2012

• Registered 664 new patients.

• Provided 14,362 services • 5,210 primary care visits onsite • 409 primary care visits offsite • 2,074 specialist referrals • Additional services including help

with prescription assistance forms, specialty referrals, medication pick-up, nurse visits and lab testing

• Dispensed $4.6 million in prescription drugs:

• $81,283 in drug repository medications

• $2.6 million in medications acquired through pharmaceutical manufacturer patient assistance programs

• $1.4 million in medications through the Astra-Zeneca institutional patient assistance program (IPAP)

• $435,919 in medications provided by medication samples

• $18,493 in AmeriCares prescription programs

2013 Highlights

• Volunteer physicians, nurses and other administrative volunteers at NCH provided services valued at over $241,000.

• NCH became the first free clinic in Ohio and one of the first in the nation to achieve recognition as a Patient Centered

Medical Home, Level 3, by the National committee for Quality Assurance (NCQA).

• In September, Charity Navigator, America’s largest and most-utilized independent evaluator of charities, awarded North Coast Health its

prestigious 4-star rating for good governance, sound fiscal management and commitment to accountability and transparency.

• Expanded our clinic hours to include three scheduled evenings per week and Saturdays – in addition to our clinics held five days a week during regular business hours
