HOW TO APPLY:

Please send your completed application form, supporting materials, and application fee to WHITE WAVE.The Application Deadline is April 2, 2012 (Early Bird Special: March 20, 2012).

1. Application (Can be downloaded at whitewavedance.com and appears at the end of this letter)2. DVD Work Sample

Please submit a recent performance-quality DVD of the work you propose to show.

Please note which chapter(s) you wish us to view. If you want us to view a specific workexcerpt, please note the times. If the choreographer is performing in the work, pleaseidentify (i.e. color of costume or second from left, etc.)

If you do not have a recording of the work you propose to show, submit a performance-quality DVD of another work and note on the application form that the videotaped pieceis not the proposed work.

Do not submit promotional montages of many works.

Make sure your DVD is labeled with your company name.

Please note, we will NOT accept an online link via YouTube or other sites for yoursubmission

3. Choreographers resume (1 page) and/or company profile (1 page)4. Additional Promotional Material (such as press kits or high quality photos [3 maximum])5. A self-addressed stamped envelope if you wish your materials returned to you (make sure to

include proper postage). Materials in Applications without SASEs will be discarded after our panemeetings.

6. Application fee: All checks or money orders should be payable to WHITE WAVE.Early Bird Special fee: $75 (postmarked or hand-delivered by March 20th)Regular Application fee: $90 (postmarked or hand delivered by April 2nd)

IMPORTANT GUIDELINES:

Each choreographer/company may submit up to two applications for consideration (eachapplication requires a separate completed application form, DVD, and application fee).

Length: your proposed program must not exceed 30 minutes, (one full-length piece, or severalshorter works), including set-up and strike.

Hand-delivered applications must be at WHITE WAVE by no later than5pmon April 2nd, 2012;mailed applications must be postmarked by April 2nd, 2012.

E-mailed or faxed applications are not acceptable, nor are late applications.

All applications will be reviewed by a panel of impartial adjudicators. If your work is selected, youwill be notified by e-mail by May 16th 2012.
